Approximately 9:25am, two survey techs discovered a rib slough while performing scan of the bench. Area had been recently mined with autonomous loader and ops group was in backfilling cycle. The slough was about 115' from closest mining activity. Due to current mining activity (backfilling sill) access to slough was limited but was estimated to be between 600-800 tons of material

Approximately 7:30am, techs were conducting routine shaft inspection when they noticed 120 volt cable had become unsecure in shaft. The plastic tie wrap used to secure the cable had broken loose because it was old. After performing JHA, the cable was secured with nylon strap and techs continued shaft inspection. Shaft inspection was complete and back to normal operations at 10:08am
This is being filed in protest. Approximately 22h30, haul truck operator noticed flames coming from diesel particulate filter area of truck. Operator was able to extinguish the fire. Operator notified TROC of incident and evacuation orders were given for precautionary reasons. Mine was evacuated and all personnel accounted for at 23h23
A fall of ground was discovered in the LC3830 SD334 on 7.5.19 around 01:00 hours. This most likely occurred at shift change, durning blast, on 7.4.19 at around 19:00 hours, but is unknown. The heading was previously shot at the end of night shift 7.3.19, & was mucked on day shift 7.4.19. From the time the heading was shot until the fall of ground was likely only 12 hours.
Ee was seated behind front passenger seat in mantrip. Ee was closing the compartment door to the right from a seated position, and placed right hand in the door frame near the hinges. Ee kicked the door shut, pinching right middle finger in the door. Amputation of the tip of the right middle finger.
Ee was cleaning an area and lifting a wheel barrow full of rocks when EE felt pain in EE's abdominal area. EE was diagnosed with an inguinal hernia and referred to a surgeon. EE was released to restricted duty.
Ee was doing regular rounds, taking reading in pond #1. EE slipped and fell, landing wrong on left foot. No fracture, but diagnosed with contusion and sprain to left foot. Given boot and crutches and site is unable to accommodate restrictions.
Ee was walking to haul truck, slipped and fell, landing on EE's knee. Seen at the ER, diagnosed with pain in knee. Seen again at the clinic and put off work pending MRI.
Ee was starting EE's shift. EE put a lithium battery in EE's pants pocket and it came in contact with EE's keys, causing a fire. Ee burned both hands and upper right thigh. Transported to UofU burn treatment for care.
Ee was replacing oxygen/acetylene tanks and the crane hook came down on their left thumb. EE was seen at Renown ER and surgery was required on the thumb. EE is off work pending surgery follow-up.
Ee was working on haul truck. Ee lost footing on some broken concrete & fell, landing on right elbow. Ee was seen at the clinic on 08/12 & diagnosed with a contusion & strain with possible radial head fracture. *EE seen on 9/4/19 repeat xrays showed fracture at the radial head. Follow-up on 9/11/19 with orthopedist confirmed fracture, ee sent to PT, restrictions not accommodated.
EE was descending haul truck when they slipped and fell approximately 6 feet, landing on the ground. EE was flown to University of Utah hospital and diagnosed with multiple fractured vertebrae. EE underwent surgery on 08/09/19.
EE was changing a drill bit. When they repositioned the bit, the drill steel came down and pinched their right ring finger. Laceration required stitches, no fracture seen in xray. Employee originally released to full duty then put off work on follow up.
Ee was changing a breather and walking under a haul truck. Ee hit head on the steering cylinder. Ee complained of immediate head and neck pain. Seen at ER and diagnosed with closed head injury and neck sprain. Put off work for 1 day. Follow-up on 08/05 resulted in return to full duty.
Ee was descending haul truck and EE's back pack caught, causing EE to lose EE's balance, twisting to the left. EE caught self with EE's right arm. EE was seen at the ER and put off work pending an evaluation with an orhto. Seen by the ortho on 08/09/19 and released to restricted duty, MRI pending.
EE was operating haul truck and hit a rut/hole in road, jarring them. EE was seen at Golden Health and returned to full duty. On 12/13 EE asked to be seen again and was released to restricted duty.
EE put hand in pinch point resulting in laceration to hand.
Employee was using a 2.5 pound hammer to loosen up prill in the silo. While swinging the hammer the head caught on the silo handle causing employee's arm to stop suddenly. Employee felt a pop in left shoulder.
Ee was replacing a filter cloth when EE bent down to pull the cloth EE reports pain on right side of low back and right shoulder. Seen at clinic, diagnosed with a right trapezius strain and right para spinal lumbar strain. Released to restricted duty.
Two employees were changing cutting edges on the 988 surface loader in the east bay of the Pete Bajo surface shop. The new cutting edge was resting on top of the old cutting edge on the floor, supported by a bolt on one side. The cutting edge slid, crushing the ee's left hand. Diagnosed with a fracture to the left tip of the thumb, and laceration. Put on restricted duty.
Ee was driving haul truck, it started to slide down a ramp, turned 180 degrees, and hit a dry spot of ground, coming to a sudden stop. Ee's left arm was jerked by the steering wheel. Seen at the clinic and put on modified duty, no use of the injured limb.
EE stepped over a hose, the hose blew apart and whipped around, hitting them on the back of the lower right foot, hyperextending right knee. EE reported on the date of injury, but did not seek treatment until 01/03/20. EE was put on modified duty and an MRI was ordered.
Ee was changing out a pump. The ee pinched EE's thumb between the pump and the tank mount. EE was seen at the ER, diagnosed with a fracture to the tip of the thumb, received stitches, and was released to modified duty.
Employee pinched finger between drill rod and drill mast.
Ee reports feeling pain in the shoulder when pulling self into mill. Diagnosed with a strained shoulder and released to full duty. Subsequently released with restrictions on follow-up.
Ee reports EE was inspecting heading and scaling when a rock struck EE's leg. EE reports left leg pain. Seen at the ER and clinic, released to modified duty, use crutches as needed. Diagnosed with abrasion to left thigh and contusion to left calf. Site was able to accommodate restrictions.
Ee was replacing a belly liner in the mill and the liner fell onto the thumb. The ee was seen, the wound was cleaned and stitched, and the ee was released to restricted duty.
EE was operating a haul truck when it started to smoke, and the employee was exposed to some of the smoke. EE was seen at the ER and diagnosed with bronchospasm. EE was released to restricted duty for two days.
Ee was putting a baghouse cage into its holder and the cage fell, pinching right middle finger between cage and holder. Ee seen at ER, diagnosed with fractured fingertip and laceration requiring stitches. Released to restricted duty.
A maintenance technician was struck by metal shrapnel while working on a hydraulic shovel. The employee received a laceration on the lower back, requiring stitches.
Ee was opening a bay door and a pulley fell from 15 ft above, striking them on the hard hat, then the right shoulder. Seen at the clinic, diagnosed with a laceration to the back of the head requiring 3 stitches, and a contusion to the right shoulder. Released to full duty.
Ee was lifting a box of crucibles from one cart to another and strained lower back. Seen at ER and released to modified duty on next scheduled shift.
Operating a haul truck, employee struck left leg against dash of unit when employee drove over a pothole. Employee was working in tight quarters. Employee reported on 12/12/2019 as for medical on 01/07/2020 as area was irritated. No restrictions.
Ee was operating loader 50 in cut 06-6040. The floor in the area was reported to be rough, and the ee was jarred while operating the loader. Ee reported to the supervisor and was seen at the ER. Diagnosis on follow-up was grade 1 acromioclavicular joint separation. Ee was released to full duty.
Ee untied a roller table that was used to put liner in the mill. It was leaning against a handrail, it fell, striking the ee on the right lower leg. Reported to supervisor and was seen on 09/25, x-rays negative for fracture. Ee followed up on 10/29 and was diagnosed with an infection making this reportable for treatment.
EE was running a 1/2 inch impact gun and screwed the thread into their right palm. Xrays negative for foreign body in hand, stitches required. EE released to full duty.
Employee was working from a scissor lift cutting an existing 1/2"" all thread hanging from the electrical room back. The porta-bandsaw broke away from the material and fell onto the Team Members left hand cutting through the glove and injuring hand. Laceration to Left Pointer Finger
Ee was unplugging a sump pump in the lime slaker area. Air broke the plug free and lime dust and debris was blown under the employees safety glasses into eyes. Seen at the ER and diagnosed with chemical conjunctivitis. Ee prescribed medication for the eyes. Released to full duty.
EE was tacking a liner on a loader bucket when a piece of weld broke off and struck EE on the right side of the forehead. Seen at clinic and stitches were required to close the laceration. Released to full duty.
Tripping out of the hole, driller hoisted plug up and towards collar, assistant was using rod feeder and guided plug towards collar and the other assistant reached for hoist plug while in motion towards head and finger was pinched between head and hoist plug.
Ee was turning a manual valve to open a knife gate when EE felt a pop and pain in EE's right shoulder. Reported to supervisor and continued to work. Seen at clinic on 08/01/19 and diagnosed with a right shoulder rotator cuff strain. Released to full duty but required to attend physical therapy.
Employee was working in the old float area of the mill scaling a tank. While using a small pry bar the employee was striking and prying the scale away from the side of a tank. The employee contacted a sharp piece of bracing that was partially hidden in the scale on the side of the tank and employee's right forearm contacted the sharp metal causing a laceration.
Ee reports feeling pain in elbow and upper arm after putting crucibles in furnace. Ee was seen by doctor on 12/04 and diagnosed with epicondylitis and released to full duty. Determined work related and accepted on 12/27/19.
