A regulator at the open pit guard shack malfunctioned causing two 120KV fuses to malfunction. This caused a power bump on the underground side causing the 4703 service cage to lose power.

At approximately 8:30 AM ground fall was reported in the FED 2464_1270 by the geo techs and the SMD shift supervisor. Approximately 300 tonnes of material fell.
Maintenance work being done on shovel, and smoke was identified. Work was was stopped and area was evacuated. Flames noticed, and source unknown at this time.
After employee was finished mucking out a short 6 foot round in the LA 4010 230, the employee was cleaning up the ribs and the back. The Mucker operator started to place bucket on the ground when a fall of ground occurred causing the mucker to be partially covered in material.
On night shift of August 22, 2024 a ground failure was discovered by NGM shifter on the MXD ramp. The dimensions of the failure: 15 ft. w x 12 ft. t x 6 ft. deep and started at the sill and did not quite reach the spring line. A cross-shift blast in the MXD 2853 West induced the failure. The ramp was barricaded and closed down.
A ground fall occurred at the 4472-117BC intersection.
At approximately 7:50pm a ground fall was reported in the MBD 2804. The ground fall occurred during shift change and approximately 300 tonnes of material fell.
The #3 hoist 4705 went down at 3:10 AM for ""brake issues"", the electricians were called up from underground. Troubleshooting was completed and found the bottom brake sensor was out of adjustment. Electricians backed the sensor out and the brakes released. At this point the sensor was adjusted to its proper location. The hoist was back up and operational at 5:14 AM.
4703 hoist alarmed to having a DCB low voltage fault. During the troubleshooting phase, it was found that the loop contactor wiring was brittle causing the wiring to short out on the metal framing of the loop contactor box.
Safety on-call received a call at 3:58pm from TROC Dispatch that there was a brush fire at the mill standpipe by the tails. Mayday issued, on shift supervisor was notified, and fire truck and water truck dispatched to location. At 4:40pm brush fire was extinguished. The power pole with the broken line was repaired by Western Line Builders contractor.
Hazardous level of natural gas was discovered in truck shop 5. No damage, injury or illness was caused by the natural gas.
All Nevada Energy line power was lost to the mine caused by utility poles being covered in wet snow but not damaged, resulting in the 4701 hoist losing power. No personnel were on the cage at the time. Escape was available via the 4315 service hoist. At 0810 am power was restored via generator power. Nevada Energy Line power restored at 10:39 AM

The geologically unpredictable slope (GUS) was identified July 2023 and monitoring started. On January 25, 2024, GUS area was affected by potential runout (5% probability) closed area to all mining activity. February 7th at 8PM, as a precaution, 7C south mining operations relocated from behind pit crest above GUS. On February 11th GUS highwall collapsed at 4:12PM.
An employee loaded two dumpsters on the bottom deck of the service cage and the top deck with a dumpster and steel tote. The employee shut shaft door and sent the cage to surface. A minute later, personnel heard a loud noise come from the shaft. When the hoist heard bells coming from the console, EE stopped the conveyance. The Supervisor inspected the cage and noticed damage.

Ee was putting chock blocks back on the truck when one fell between the hand rails and the drum. EE went to get the chock and the hatch hooked the chock, pinching EE's right thumb between the hand rail and the chock. Seen at ER, diagnosed with traumatic amputation of the tip of the right thumb.
The employee was lifting a scaffold plank overhead to set for upper level. the employee felt a pinch in stomach, but did not think it was serious. Later it was determined to be a small umbilical hernia. Late reporting from employee. Work Status: May perform regular duties with attention not to aggravate.
Ee was moving the vacuum to clean work area. When ee pulled on the cord, the vacuum pulled, and ee was also pulled backwards and the lid slammed on ee's finger. Initial diagnosis was ""ring finger injury"". Follow-up with ortho on 1/27/25 resulted in restricted duty, making this reportable.
Ee was using a pry bar to help guide a cylinder into place while standing on the third rung of a 6 foot step ladder. As another operator moved the cylinder into place, it caused the pry bar to ""kick back"", causing the ee to lose balance and fall from the ladder, landing on left hip. Ee transported by ground ambulance to hospital, orthopedic surgery done on pelvis.
Ee reports getting into the cab of equipment, startled by loud noise, fell into EE's chair. Ee reports unsure how, but EE's knee popped out. EE was diagnosed with a non-occupational condition that contributes to dislocations etc. Following medical investigation, on 9/18/24 cumulative trauma accepted by workers' comp, strained knee, prompting this report.
Ee was walking to work area. Ee reports feeling knee pop and having instant pain, then fell to ground. No uneven ground, no work related condition reported. Seen at ER, put off work pending ortho evaluation. WC claim denied, but reporting per MSHA regulation.
Ee reports walking up stairs at work. Felt pop and pain in left calf. Reports inability to bear weight. Put off work pending follow-up on 7/16. On 7/19 determined not compensable by WC, however, reportable under MSHA.
EE was working at the 4070 lube 321, filling up with fuel, reports being hit in the mouth by the door of the lube truck. Initially NGM only aware of false teeth/bridge being impacted. On 6/24/24 we found out the EE had 3 real teeth impacted and they also lost time.
EE went upstairs to get parts. On the way back down EE remembers feeling like they were going to fall. EE remembers being on the floor, trying to get up. EE reported incident, worked rest of the shift. The next day they asked to see doctor. EE also reported having blood pressure over the past week. Seen at clinic diagnosed with sprain left knee and ankle. Restricted duty.
Ee got off the grader and walked around the equipment to open the drain. EE reports falling forward onto both knees. The ground was wet and snowy. EE felt a pop in the right knee. Seen at ER, x-rays positive for fractured patella, released with restrictions. Ortho consult scheduled for 4/10.
EE was passenger in the shift change bus on the way to the time shack. The bus hit a pothole and it jostled the EE in the seat. Seen at clinic, diagnosed with a contusion to the left knee and on follow-up complained of wrist pain as well. Released to restricted duty.
Employee had just dumped haul truck load and got out of the cab to clean the pad. Upon re-entering the truck, the door became hard to close. As they put more force on the door, it accidentally closed and pinched their thumb near the door hinge.
EE reports while ""spinning off"" filters while laying on their left side and reaching with their left arm, they felt a pinch in their shoulder. They reported the next morning that they had pain. Seen at the ER and diagnosed with a rotator cuff injury. Put off work pending MRI and ortho consult.
EE was loading pallets and boxes into the back of the truck. When they shoved on the pallet they felt sharp pain in abdomen. Continued to work, pain got worse. Reported to EMT, taken to ER, diagnosed with a hernia, admitted for surgical repair.
Ee loading round, just finished scaling prior to loading. Some rock was coming down, ee tried to scale down again, nothing came. Continued loading, a few pieces of rock came down, one brushed ee's left arm and then rolled onto outside left calf. Seen at ER, referred to ortho and is having surgery 2/2.
Ee was carrying a chair into the UG batch plant office. While stepping up onto the platform, ee tripped, dislocating and fracturing, left ankle. Seen at ER, put off work pending orthopedic consultation.
EE was changing cutting edges on a loader bucket. EE reports a beam weighing 140 pounds dropped from about two feet above onto EE's right foot. X-rays negative for fracture, referred for ortho evaluation. On restrictions site can accommodate temporarily.
Ee was lifting a blue drum that ee reports was half full. Ee felt pain in right bicep while lifting. MRI on 11/15 was positive for a tear making this reportable. Claim accepted by workers' comp on 11/18. Referred to orthopedic surgery.
Ee reports trying to get out of haul truck and somehow ee's foot got stuck and ee twisted knee. Seen at a health clinic, diagnosed with a sprain, and on follow-up was released with restrictions. Currently working modified duty.
Ee was walking along pump alley when EE stepped on a rock and rolled EE's left ankle. Seen at clinic, diagnosed with pain in EE's left ankle, given a boot, and released to modified duty that site can provide.
Ee reports feeling a pop/tear when ee picked up ee's backpack out of the truck bed when being dropped off at ee's truck to start shift. Arm went numb, and then sharp pain in the arm/elbow. Seen at ER, ee released with restrictions and referred to ortho. Seen at ortho, diagnosed with torn biceps tendon, will need surgery.
EE was switching skid steer trailers. EE reports shifting all EE's weight to EE's let leg and hearing a pop in EE's left knee. On 7/12/24 NGM received confirmation that this is a work related injury. The EE was put off work pending surgery.
Cable to dumpster caught behind no parking sign. EE went to get cable unstuck, the cable came free, lid shot up, catching their left hand between cable and sign. EE put off work pending surgery on 6/4. Released with restrictions effective 6/10.
Ee stepping up on to ladder of piece of equipment. Felt pain in knee when EE stepped up. Seen at clinic, diagnosed with strain. Determined occupational on 5/28 following site and medical investigation. Reportable for lost time.

Ee was preparing to replace a tire on a haul truck and was pulling the adaptor on a hydraulic jack. Ee pushed the adaptor up with left hand and felt a pop in left shoulder/upper arm. Ee was seen at the clinic, diagnosed with a strain and referred for an MRI. Released with restrictions site can accommodate.
EE was walking and their foot got stuck in the mud. They tripped and fell forward, landing on both knees and hands. Seen at ER, diagnosed with a strained knee, and released with restrictions site can accommodate.
Ee was operating a guzzler truck. Ee grabbed the lever to open a discharge valve, lifted it, and felt a pop and tingling in right wrist. Seen at clinic, diagnosed with a strain, and on follow-up referred to ortho and put on restrictions.
Ee was installing a 90 degree elbow on the back of a sprayer. The elbow fell, landing on the employee's left hand. Diagnosed with fractures to the index and middle finger and put off work pending orthopedic surgery.
EE was getting out of skidsteer and they slipped and fell, landing on their wrist. Seen at clinic, diagnosed with a left wrist distal ulna fracture and a contusion. Placed in hard splint by ortho. Released to restricted duty site can accommodate.
EE was working on a charge hose. While positioning the hose, they felt a sharp pain in left elbow that radiated up to left shoulder. Reported and continued working. Seen at clinic 2/6 and released to full duty. MRI was ordered, done on 3/6 and reviewed on 3/26. EE diagnosed with torn rotator cuff and put on restrictions. They will be having surgery.
EE felt dizzy so they took a 5 minute break and ate some jerky; then decided to take lunch break, had soup and water. Walked over to garbage following lunch, put water in fridge, blacked out and fell on face (according to witnesses). Cause of syncope unknown. EE fractured maxillary sinus.
Ee was in back seat passenger side of personnel carrier. Ee was holding scaling bar with one hand to allow other passengers to enter front seats of personnel carrier. Ee's right hand positioned between door jamb and door. Other ee closed door, crushing the injured ee's right index finger. Ee released to restrictions site can accommodate.
EE was doing a post-shift inspection and slipped on ice in the chock ditch, falling and landing on their right side. Seen at ER, diagnosed with right rib fractures and right elbow contusion. Put off work pending follow-up on 1/22/24.
IP was removing drill pipe out of presenter on a DU431. IP noticed pipe was hotter than usual and went to place pipe on rack and caught finger between pipe racks ear and pipe. Seen at U of U on 12-26-2024 Diagnosed with Longitudinal fracture of the middle phalanx involving the PIP joint on right index finger requiring 11 stitches and 2 Pin. Surgery scheduled 12-27-24
Ee was working with another ee to remove angle iron. First bolt removed, and when second bolt removed, the angle iron fell, landing on ee's hand. Seen at ER, diagnosed with a bruise/contusion. No fractures/dislocations identified. Released to RTW full duty 12/26.
EE reports large boulder hit bed of truck, making cab shake. EE reports lower left back pain. Seen at clinic on date of injury, released to full duty. On 12/3 EE was seen again, put on restrictions. On follow-up on 12/9, released to full duty.
EE reports getting out of truck, slipping on ice, and feeling pain in knee. On 12/2/24 EE saw a provider at the clinic who diagnosed EE with pain in the knee, subscribed physical therapy, and released EE to restricted duty.
EE was using a safety knife to cut a piece of hose when the blade broke on the knife, causing the hose to slip and the knife to cut their hand. Seen at health clinic, diagnosed with a laceration that was closed with stitches, and released with restrictions. Working modified duty.
Ee reports stepping off bus at queue area to go to ee's haul truck. Ee reports rolling ankle and falling to ground. Seen at clinic, diagnosed with ankle strain. Released with restrictions site can accommodate.
Ee was changing out cutting edge on a grader. The magnet used to hold the cutting edge in place failed, allowing the cutting edge to slip and fall on the ee's thumb. Seen at ER, diagnosed with a fractured thumb, and referred for surgical consult. Released to restricted duty.
Ee was walking down stairs into pit at the shop. EE slipped on last step, fell back, and caught self with right hand. Felt a pop. Seen at clinic, x-ray positive for fracture. Ee put on restricted duty site can accommodate.
EE reports feeling difficulty moving neck (pain/stiffness) after using a wrench to tighten a bolt. Pain and stiffness continued. No specific incident, site cannot recreate incident. As of 10/2 workers' compensation has not accepted claim, however due to EE's report of pain, NGM is reporting to MSHA.
Ee was adjusting shovel cable. EE lost EE's balance and went to catch self and landed on EE's hand wrong. Seen at ER, diagnosed with a dislocation, and referred for further care. Released with restrictions that site could accommodate. EE's one day of restricted was a paid fatigue day. Ee released to full duty 8/14.
Ee was using a pry bar to move a pin. The bar slipped and smashed/pinched employee's finger between the bar and a ladder. Seen by doctor, diagnosed with a fracture to the right long finger, and released with restrictions site can accommodate.
Ee reports rolling up truck window manually, when EE felt a sharp pain in lt thumb radiating up EE's hand, into EE's forearm. Hand spasmed, and as ee tried to straighten it, the pain was intense. Seen at clinic, diagnosed with strain and hairline fracture. Referred for ortho consult, scheduled for 8/14.
EE started climbing up the ladders and scaffolding platforms to help at the top of the ball mill discharge. EE got to the 4th scaffolding Deck and turned body away from the ladder and towards the deck. EE proceeded to step off and heard a loud pop from EE's left knee as EE stepped to the deck.
Ee was dropping the 3rd belly pan on a piece of equipment in order to gain better access to inspect a leaking hose. The pan dropped onto EE's left hand. Seen at Golden Valley, diagnosed with fractures and laceration, and referred to ortho. Released to restricted duty.
EE was opening door on a tank. Door fell off the hinge, landing on EE's foot. EE was seen at the clinic, diagnosed with a fracture to right foot. Released with restrictions and referred to ortho.
Ee was working on a shovel when EE struck EE's hand with a 5 pound hammer. Seen at ER and clinic, diagnosed with a contusion and a non-displaced fracture. Released with restrictions site can accommodate and referred to ortho.
Ee raking down grit in slaker 4 bin. Reports feeling pull in rt shoulder, continued to rake, pain got worse. Reported incident. Seen at clinic the following day as pain was worse, released to return to work with restrictions site can accommodate.
Ee was putting filters away when a filter press fell, striking the ee on the back. Ee reported head, neck, shoulder and right hand pain. Seen at ER and clinic, released with restrictions site can accommodate.
EE was moving swellex bolts. They picked one up and was looking at other items in the tray when the bolt they were holding slipped from their hand, falling on right middle finger. Seen at clinic, diagnosed with a fracture, wound was sutured, and referred for a consult with a hand specialist. Released to restricted duty.
Ee was doing pre-op inspection on equipment. Ee walked in a sump area that had slurry in it, ee reports it was up to ee's knees. Mud went inside the ee's boots. Ee continued to work, and reports getting slurry in boots 2-3 more times throughout the day. Ee reported the incident that night. Diagnosed with bilateral chemical burns to lower legs.
EE working in containment area reports slurry was ""thigh high"". Trying to walk through it, right foot sank, stepped back, and their left foot stuck. They fell counterclockwise and into a squatting position. Strained right knee. On restrictions site can accommodate.
Ee was changing a drill bit. Ee lifted the drill bit out of the bit basket and felt pain in lower back. Seen at ER, diagnosed with a strain and spasm and lumbago. Released to restricted duty that site can accommodate.
EE was going to sit in haul truck and misjudged the distance to the seat and fell, hitting their side against the seat. EE was seen at Health, diagnosed with a fractured rib, and put on restrictions site can accommodate.
Ee driving truck when hit a bump/pothole, causing the visor to fall and strike ee's head/neck. Seen at clinic, diagnosed with strain, released to full duty. On follow-up on 4/17, ee was put on restricted duty that site could accommodate, making this reportable for restrictions.
EE stepped in containment pond with lime/water mix, causing a skin irritation to EE's ankle on 03/29/24 9:00pm. Miner first aid of flushing ankle/apply antibacterial cream. When offered additional medical attention, refused and would selfcare. EE called 4/4/24 12:00pm complaining about foot pain. Took EE to dr who thought potential infection, prescribed antibiotics and pain meds.
Ee was working with another ee trying to maneuver a dumpster lid. The lid fell, catching the ee's left pinky finger. Hand surgeon determined no bone amputation, small chip fracture, no surgery required. Released to restrictions, ee worked last day 3/28. EE had given notice two weeks prior, moving to Utah for another job. Completing RTW info as ee resigned, last day 3/28.
EE was getting out of light vehicle. EE stepped out and into a parking ditch. The ditch was deep, and EE twisted right ankle. Seen at Golden Health 3/24, released with restrictions. X-rays on 3/25 were positive for a small avulsion fracture.
Ee was walking to the work area when a ""burst of sneezes"" caught EE off guard. EE reports hunching EE's back and pulling a muscle. EE was seen at Golden Valley and diagnosed with a non-occupational back strain. There was no environmental factor or work-related condition identified that caused EE to sneeze. Released with restrictions.
Ee was tightening bolts with an impact gun when the bolt spun against ee's glove, cutting the glove and cutting ee's palm. Seen at ER, laceration closed with steri-strips and glue, and on follow-up ee placed on restrictions site can accommodate.
RAM UG Elect team was being escorted from their UG work heading to the cage via UG UTV's. The escort pulled into a muck bay leaving the RAM buggy in direct line with the mucker. Before the buggy was hit the RAM team members jumped out of the buggy, the injured RAM employee fell and slid on the ground scraping hands and bruising left leg and ankle.
Ee was lowering a man basket on Sh017 for fueling. Ee had to push hard on the release pin to get the basket to move. The basket dropped down, and the ee reached out to grab the handrail. Ee contacted the man basket safety gate bracket which was sharp, and lacerated right hand.
Ee was sitting on a tire on a piece of equipment. Ee went to dismount to get a tool, ee's foot caught on the tread of the tire, ee fell, landing on left wrist. Seen and released to full duty. Orthopedic consult on 2/27 led to restrictions and medical treatment, making this reportable.
Ee was walking outside at Mill 6 by the shack for RTO (oxidizer). Slipped on ice and fell. Seen at clinic, diagnosed with fracture, released with restrictions site can accommodate.
The employee was walking upstairs carrying materials when EE slipped on the top step and fell forward landing on left arm. The contact resulted in a fracture to the arm just above the wrist.
Ee was chipping slag off door with a chipping hammer, and a piece of slag flew up under EE's safety glasses and got in EE's eye. First aid was provided and ee seen at the ER, released to full duty, and referred to an eye doctor. Eye doctor also released to full duty. Diagnosis corneal abrasion, prescription given.
An employee was installing a steel tank liner inside a tank. While aligning the liner by hand, it slipped and pinched the employee's middle finger, causing a laceration.
On Tuesday, November 5th, around 3:30 p.m., an iron worker helper injured their right middle finger while adjusting a steel I-beam. During the bolting process, the worker's finger got caught between the reaction arm of a pneumatic tool and the I-beam, causing a laceration.
Employee was conducting welding operations on work platform. The hand rail post broke and the employee fell to the lower level. Miner cut knee requiring sutures to close the wound.
Ee reports pressure washing a piece of equipment when debris shot behind ee's safety glasses into left eye. Ee was seen at Humboldt General, diagnosed with a corneal abrasion and given prescription medication. Released to full duty.
Ee was moving crucibles to brush and clean table. Dust or other foreign body got into EE's eye. EE rubbed EE's eye, and initially it felt ok, but then it became irritated. Seen at eye doctor, diagnosed with a corneal abrasion, given prescription antibiotics and steroid, and released to full duty.
Ee was traveling down 1600 haulage when loader bucket made contact with pillar causing an abrupt stop. Ee hit left side of face on cab window. Seen at clinic, diagnosed with contusion/swelling, and released to modified duty. No restricted days worked as ee had fatigue night. On follow-up 10/4 released to full duty.
EE was using rap wrench to break bonds on a bundle of spiling (pipe). Pushed wrench on last bond, bond broke, causing the pipe to roll down, pinch ee left middle finger. Seen at ER, xray negative for fracture, sutures required to close the wound.
EE was removing hose from a machine with cutting wheel. EE was wearing a face shield, gloves, and safety glasses. A piece of debris got in EE's right eye. EE was seen at health clinic, referred to eyecare clinic, diagnosed with a conjunctival abrasion, prescribed antibiotics, and released to full duty.
EE was shoveling dirt out of the truck in the shop. They threw a shovelful of muck and felt their neck pop. They report feeling pain and numbness. Seen at ER, diagnosed with pain, released to full duty. On follow-up 10/24, EE referred for physical therapy making this reportable for treatment.
Ee in hot change bus that hit a pothole, jostling ee in the bus. Seen at ER, scans negative for fracture, diagnosed with head injury, released to full duty. On follow-up 10/07 reports soreness/aching in neck, referred for physical therapy, making this reportable for treatment.
Ee using box cutter to cut trailing cable shielding. Knife tip broke and went through cut resistant gloves, cutting the ee's thumb. Seen at ER, sutures required, ee released to full duty.
EE and coworker were moving EE's toolbox. EE's arm was caught between the toolbox and the wing guard on HTK104, causing a laceration to the EE's right forearm. Seen at ER, laceration was closed with staples, EE released to RTW full duty.
Employee was taking bolts out of the torque converter flywheel from an engine. EE's coworker was turning the engine over by hand to locate the bolts. Once stopped, EE stuck EE's finger in the hole to retrieve bolt but did not tell EE's co-worker. EE's co-worker started turning the engine over again and employee's finger accidentally got caught between the hole and the flywheel.
EE was bolting, and a bolt fell out of EE's fingers. The employee powered down to reinstall the bolt and was heading back to the bolter's cab when EE tripped and fell on a rock, striking EE's knee.
EE hooked a 4 way chain to component with crane. When they put tension on the chain, it spun and hit them in the mouth. Broken front tooth. Seen at clinic, referred to a dentist, and released to full duty.
EE was retorquing lug nuts. EE was moving the gun to the next lug nut and struck their thumb on the edge of the wheel. Seen at ER, laceration was closed with sutures, and released to full duty.
During the demolition and removal of 30"" HDPE piping, IE utilized a Sawzall hand tool to cut piping. The Sawzall blade broke and became lodged in the pipe. IE was not aware that the broken blade was protruding from the pipe and rose from EE's position. As EE rose, EE's thigh above EE's left knee came into contact with the broken blade, causing a laceration.
Ee was working on machinery in the SXEW plant. Ee reports a foreign object getting behind ee's safety glasses into right eye. Seen at clinic, diagnosed with right eye burn. Given erythromycin drops and released with restrictions for one shift to work indoors. Seen on 7/5, released to full duty.
EE was using the fuel reel on the back of the truck. As EE was rolling it up, it was going fast, and popped back and broke EE's tooth. Diagnosed with a broken front tooth, seen at dentist, released to full duty.
Ee was tying leads on det cord when some material fell, striking the ee on the head, shoulders, and back. Seen at ER, diagnosed with fractures to the sinus. Released to full duty. Follow-up with ENT confirmed no further care required.
A RAM belt technician was performing a finger splice on a conveyor belt, the OLFA blade broke in the rubber of the finger splice. The employee obtained a laceration on right thumb, the broken blade punctured through employee's level 6 cut glove. The employee received 4 stitches, tetanus shot, and given a full duty work release.
An LHD tire came off the Forks of a Telehandler during transport and rolled down the Range Front decline ramp. The tire contacted a CUSA Minca Paus 18.1 personnel carrier near crosscut 6, with two employees in the Cabin of the Paus. The Paus passenger sustained a laceration on the top of EE's head requiring staples to close.
Hydroblast cut and injection of water in the left hand between thumb and pointer finger
The injured worker used a 24"" pipe wrench to loosen a joint. The joint was near eye level, and the employee placed the pipe wrench on the joint at a vertical angle and began to pull downward. In the process of pulling downwards the wrench slipped and hit the employee in the lip. There was a small laceration on the lip that only required first aid; however, the tooth was fractured.
IE was team-lifting a 95lb grout bag when EE's right shoulder popped and started to ache. EE was seen 4/22/24 at urgent care, where EE was given a full release with no restrictions. EE returned to a specialist on 07/02/24 and was put on restricted duty.
Employee was climbing into the cab of the nipper flat deck (open cab). As EE was entering, EE's mine belt accidentally got caught on the door, causing the door to be closed behind EE. EE's thumb was caught between the door and the door frame as EE turned to sit in the seat.
Ee was working on equipment that had a film of lime on it. Lime got in EE's right eye. Seen at the ER and released to full duty. NGM received the workers' comp claim form on 4/4 with diagnosis of scleral abrasion secondary to chemical exposure. Ee prescribed antibiotic for the abrasion making this reportable.
Ee was making lunch, pulled a hot plate out of the microwave, dropped it, and cut hand on a piece of glass. Seen at clinic, the cut was closed with stitches, and ee was released to full duty.
Employee was cleaning and replacing a discharge flange seal. The employee rested the bull hose flange on top of the discharge flange to begin bolting the flange into place. The bull hose flange vibrated off of the discharge flanged and dropped approximately 2 ft and crushed/cut the employees hand as it landed on the platform.
EE was hand stabbing a bolt into the face to perform a resin test. Their right hand was in the line of fire inside the centralizer clamp and the clamp came down on their hand. The operator released the clamp, releasing the EE's hand from the centralizer. Seen at the ER, diagnosed with a fracture and laceration, and referred for an orthopedic consult. Released to full duty.
EE was working in their office when they felt a bite/sting on right ankle. Continued working and went home for the day. Next morning their ankle was red, swollen, and hot. Seen at the clinic, diagnosed with an insect bite with resulting cellulitis. Given an antibiotic and released to full duty.
EE was using comealong to pull a belt up when the hook came off and hit their lip. The employee did not seek treatment for the injury, but NGM determined that the EE's real tooth was chipped, prompting this report.
Ee was in their office at the Leeville admin building. Ee squatted down to plug in the computer and Ee reports their kneecap dislocated. Ee reported dislocating same kneecap at home 2 days ago. No injury by accident, circumstances are not unique to the work environment. Ee given brace for support. Released to full duty.
Employee began to get dizzy and removed from site. Diesel powered heater was in connex with little ventilation. Employee had carbon monoxide poisoning.
Ee operating haul truck that caught fire, ee evacuated. On 5/28, EE requested to see a doctor. Diagnosed with a physiological stress reaction from the incident that was not instantaneous. Ee put off work to avoid triggers. Reportable for OI code 29.
EE reported that they had pain in left wrist over the last year. Suspected possible carpal tunnel from operating equipment. Requested to be seen at clinic and evaluated for a possible cumulative trauma. diagnosed with tendinitis in their left thumb. Released to full duty.
Ee had arrived at work and completed a tour of the pit in a light vehicle. Was then walking up 5 steps to enter a building, sat down, said wasn't feeling well. EMTs were called, during assessment, ee complained of severe chest pains and lost consciousness. CPR administered. Life saving efforts were unsuccessful.
Employee experienced a personal medical event described by other employees as a seizure and included a brief loss of consciousness (30 seconds). Employee regained consciousness and was coherent and responsive shortly after. Employee taken to hospital for medical evaluation and released to full duty with no restrictions and non-occupational.
Driver was backed in to unload and had what is believed as a heart attack and died.
EE was grabbing a 10-foot HQ rod from the rod sloop, to add to the system. EE tripped, catching their right foot on the sloop frame, causing EE to fall with the rod still in hand. As EE landed, their left hand was caught in between the outer rod sloop frame, and the rod, which caused a laceration on their left ring finger.
The employee was greasing the tracks on an Excavator and felt pain in the low back after climbing into the operator's cabin. The employee reported previous back issues, and this was considered an aggravation of a pre-existing back problem. MRI results received on 8/21/2024 and are indeterminant if the results were pre-existing.
