HT639 dumped a load of rock for floor repair at shovel 192 in the gold quarry pit. Operator of haul truck did not realize that the bed was still in the raised position and drove between the cable trees for SH192 contacting the span of cable in-between the trees. This resulted in the HT operator remaining in the cab for more than 30min as a precaution while the power was isolated.

Due to recent, inclement weather, the upper limit switch on shaft #3 was frozen over. This condition was found during hoist checks and had to be thawed out before putting the hoist back into service.
The #3 hoist was reported as not coming back online after a planned power outage. Due to a miscommunication between personnel it was never inoperable. However, it was reported as out of service and called into the MSHA hotline.
TRM004 was dropping a fuel pod in the 3980-fuel bay when the operator smelled smoke. The operator shut the Multimec off to investigate and noticed flame coming from the engine compartment. The operator then activated the fire suppression system and exited the cab.
There was no accident, illness, or damage. During a shaft inspection it was discovered that there was ice forming on the cage guides. Decision was made to idle the chippy and cage until all ice was removed.
While Fuel truck 8852 was staging at the Range Front Decline ore pad to fuel loader 704, the technician noticed smoke coming from hood. Tech shut truck down, exited the vehicle and found it had an active fire. The operator got a safe distance away from the fuel truck and called Mayday. The loader operator picked up the Fuel tech to get them further away from the truck.
While attempting to start drilling a hole with Drill 39, operator felt machine start to sink and lean to one side. Operator exited the equipment safety and made contact with other personnel in the area to report incident.
While backfilling a stope from the 6245-118 top cut, a Sandvik LH621i loader traveled beyond the initial location of the berm with a full bucket of cemented rock fill (CRF). The loader slid down the angle of repose of the CRF fill and came to rest part-way down the slope of CRF. The loader operator was able to exit the cab of the loader with no injuries.
performing hoist pre-operation checks a communication fault occurred brakes unable to release. NGM and OEM technicians found communication issue with a motor current signal wire not being processed by the PLC. Wire was disconnected and reconnected. HMI/PLC was able to update and signal properly. After discovered and repaired hoist checks were performed and passed without incident.
IT was caused by the NGR relay on the 480V MCC tripped. Tripped do to an unknown nuisance ground fault. Thyssen electricians and NGM Electrical Engineers are inspecting and re-checking the relay settings.
Power was lost to the hoist for approx. 2 hours. No personnel were on the conveyance at the time of power loss. 91 people were underground at the time of the incident with two means of escape remaining available. It was found the SE330 neutral ground resistor monitor experienced a watch dog trip which reset the processor.
The #3 hoist had been locked out to conduct planned work on the drip trays under the service hoist sheaves on the headframe. At 10 pm on 10/2/22 when they went to start the hoist back up the drives failed to start. The hoist was backup and operational at 0729 on 10/3/22.
A basket relieved itself, Leaving 40-50 tons of material 30' long 5' high and 15' wide, on the main travel way of the Rodeo 4210 level. (15'x15' heading)
While transporting personnel to their work areas via the #3 shaft, the personnel conveyance experienced a power outage lasting longer than 30 minutes. At the time of this outage there were 20 miners on the conveyance.
The 4703 hoist tripped out about 30 feet above the 1715 level, after leaving the 1715 during the 5 pm man trip with 56 miners on board. The hoist was reset and tried to go up again, the hoist traveled about five feet and tripped again. Electricians got it to reset around 6:30 pm and then lowered the cage back to the 1715 station. The miners were offloaded and evacuated out the #3
The hoist man was performing the checks on the 4703 hoist for an MSHA inspector when the cage traveled beyond the lower level limits. The shoes came out of the guides and the cage would not come back up past that point. The shoes would not line up with the guides and allow the cage to travel.
Jammer operator scraped the right rib up to the spring line to remove the bagged out wire from the previous bolt up. The jammer operator called for the loader to come clean up what was scraped out. While the loader operator was cleaning the material that was scraped down the back failed while EE was dumping material from the cleanup into the muck bay.
A fire occurred at the Goldstrike tire laydown which involved numerous used tires. A set of damaged tires that were removed from a piece of equipment were placed in the used tire laydown. The tires are believed to have been damaged and hot enough to self-ignite causing the fire.
Number 2 hoist was tripped for low voltage coming in from NV Energy side which resulted in voltage card tripping
On 7/26/22 at approximately 9:10pm Hoist # 4703 tripped out (due to an under voltage trip) and could not, initially, be reset by the hoist person. The hoist person was eventually able to reset the hoist. The hoist was returned to service at 9:59pm (7/26/22) No damage to equipment occurred during this incident.
During normal operations, 3 haul trucks and a mucker were in the 4041 heading off the Barrel ramp. Mucker arrived in the 4041 heading and was then accompanied by 3 haul trucks. After the 1st haul truck was loaded it was noticed that they had lost radio communication, and it was at that point when a haul truck operator discovered the ground fall.
Hoist operator at shift change was running hoist checks. A spool valve test port failed causing the hydraulic system to go down.
Temporary power loss to Turquoise Ridge caused by an issue with incoming NV Energy line 147 to the site. 4701 and 4703 hoists lost power during this event. No personnel on the cage at the time. Temporary power was established via generators and all miners were hoisted to the surface until power was restored and hoist checks completed.
While completing hoist limit checks, the hoist operator was going into the upper limit checks when the hoist faulted out. The operator was unable to reset the fault and electrical department was notified
The drift in which the failure occurred is sub-parallel to a modelled major structure. This structure is located in the right rib which correlates to the zone of damage. This structural feature likely contributed to the failure occurrence. (2) Inspected rock bolts (inflatable bolts) had pitting and thinning of the steel wall indicating likely conduit for water inflow.
A communication cable was worn out and caused the problems with the hoist. The damaged cable was found and replaced. No injuries happened during this event.
TKI Employee heard a loud noise coming from the process area. Operator identified that there was a fire in the plant. The fire was located at the top of the R401 vessel. Operator immediately turned-on plant ventilation, while co-worker pressed the E-Stop button. Emergency response was dispatched. Plant operators evacuated to muster area. Fire was extinguished with no injuries.
Hoist 4703 stopped working after showing multiple faults and was out of service for a time. The source of the faults was investigated and repaired. Hoist returned to service at approximately 11pm
On 1/26/22 at approx 1445 hrs, a fall of ground was witnessed by a miner working in the area. Miners were already actively prepping rehab in the area and reported this new ground fall to supervision. The area is already under ""K"" order by MSHA and previously to the find, a backhoe was removed from the area according to the plan.
Hoistman was conducting pre op hoist checks when the hoist tripped on left brake wear switch. hoistman notifies supervisor and troc, who called safety. The decision was made to evacuate the mine on #1 hoist. The fans were taken down and #1 hoist was prepared to be used when we were notified that #2 hoist was ready to be reset, the hoistman reset the hoist, notified troc at 18:48
The reset switch had been activated during down time. On start up the hoist would not power up. The reset switch was found activated and was deactivated and hoist powered up and was exercised for two trips up and down the shaft. No interruptions or faults.
A Sandvik haul truck operator noticed flames. EE stopped, activated fire suppression, and called MAYDAY. The mine was evacuated, and 9 employees sought refuge in 3 different mine arc refuge chambers. Both MSHA and the State of Nevada were notified. All those in refuge were safely brought to surface. The haul truck has been brought to surface and is still under a K Order.
EE involved in a powered haulage incident at stope between 4010 and 4070 levels resulting in fatal injuries. Investigation ongoing. There are seven additional NGM investigators.
Ee was working from a man basket that was boomed out to remove hose. The basket bounced, and the ee's right ring finger was pinched between the basket and pipe hard line, causing a crush injury and amputation of the tip of the right ring finger. Orthopedic specialist in SLC did a revision amputation to the first knuckle. 60 days permanent disability.
Truck driver was dismounting truck and missed last step falling to ground with right leg folded underneath. This caused a dislocation fracture of the proximal fibula separating from the tibia.
Ee working underground when a mud wedge fell 18 feet from the face. Ee struck by rock. Transport to a medical center from site by air med. Ee diagnosed with concussion and contusions to back. Released to work full duty per the C4 effective 10/29/22. Ee scheduled to follow-up with occupational medicine at medical center 11/3 for clarification of RTW status.
EE was getting ready to unload bolting supplies from a mucker bucket. The bolts and wire started to fall, EE tried to move out of the way, but bolts fell on left foot. Seen at hospital, multiple fractures to left foot. EE referred for orthopedic surgery consult.
Ee operating rubber tired dozer on waste dump. Reports backing over large rock with left front tire, causing dozer to bounce hard, jostling EE in cab. Reported that day, completed shift, work two more shifts. Requested doctor visit on 7/21, diagnosed with strain. Referred to physical therapy at ee's request.
Ee reports that ee was lifting and moving boxes when ee felt a pop in neck. Ee reports not feeling pain until ee was getting into the forklift. Ee then noticed ee couldn't turn neck to the right. Requested to be seen at the ER where ee was diagnosed with a cervical strain. Follow-up resulted in restrictions that can't be accommodated.
Ee was pulling a welding machine, lost grip on the handle, and felt shoulder pop. Pain did not resolve, MRI done 9/29/22, compared to MRI done on 10/12/18, and new tear determined to be work related on 11/3/22. This claim is now reportable for restricted duty effective 11/4/22. Surgery is required.
EE was walking around sump pump in muddy area. EE slipped in the mud and twisted right knee, dislocating the patella. EE was sedated at the ER to reduce the dislocation. EE was seen at Golden Health the following day, put off work, and sent to physical therapy.
When pulling a column pipe to replace a pump. The column pipe failed and fell inside the casing. This resulted in tensioning of the electrical pump cable that was being spooled. The individual was clear of event and went to assist a co-worker and was contacted by the cable coming off the spool. This resulted in abrasion to right side of torso, fractured rib and bruised lung.
While preparing to start maintenance task and receiving instructions stepped in slurry which got in boots. Employee did not remove boots until later in the day and discovered burns to bi-lateral feet/ankles. Employee off work 2 weeks with elevation of lower extremities and monitoring of chemical burns.
Ee reports rolling a large rock out of the road in the open pit. Ee rolled the rock and felt a pop on left inner thigh. Has had ongoing discomfort. CT scan on 6/9/22 showed a hernia, making this reportable. Ee being scheduled for surgery.
Two process ees were working to clear a plugged drain with an air lance. The plug cleared and sulfuric acid sprayed both ees. EE was struck in face and neck. Originally released to full duty, on follow-up, ee put off work. Two small areas on face still open.
Ee was changing a drill bit. The bit was stuck in the basket, ee lifted drill and basket several times to get the drill bit out. Seen at clinic, diagnosed with strained right rotator cuff, released with restrictions. Follow-up visit resulted in ee put off work due to pain complaints.
EE using restroom at work, bit by unknown insect on neck and wrist. Experienced allergic reaction, used 3 EpiPens. Seen at clinic, released to full duty. Next day, went to ER for chest tightness, diagnosed with chest pain related to original bug bite/anaphylaxis, and put off for 3 days.
Ee rigging a mill liner and it slipped. EE reports pushing self away from the liner when EE slipped and collided with the wall. Seen at Golden Valley, diagnosed with a rib contusion, and put off work pending follow-up on 1/10/22.
Ee was putting chock blocks down on EE's vehicle and EE slipped on ice, falling to the ground. Seen at Golden Health, diagnosed with a dislocated shoulder which was manually reduced. Released to return to work with restrictions site can accommodate.
Ee reports driving truck through the dump, hit a pothole, and the air seat ""went out"". Ee reports shooting pain in right hip, numbness to outside of right leg to calf, and tingling in right foot. Seen at clinic, diagnosis lumbar disc disease with radiculopathy. Released to modified duty that site can accommodate. Maintenance confirmed seat did not malfunction.
Ee working underground when a mud wedge fell 18 feet from the face. Ee struck by rock. Transport to a medical center from site by air med. Ee diagnosed with multiple traumatic injuries. Admitted to medical center for treatment.
Ee removing a door that had glycol behind it. End cap door on the Geho had pressure behind it, and the pressure moved the door off the studs, causing the door to fall on the ee's foot. Seen at ER, diagnosed with fracture. On follow-up with ortho, ee released with restrictions site can accommodate.
Ee was air arcing metal and dropped a hot arc rod on EE's left leg. Seen at clinic, diagnosed with a puncture wound, released to full duty. EE developed an infection, was seen at Renown hospital on 9/12 and admitted for care. Surgical wound cleaning done and ee certified off work.
Ee was adjusting a tote in the back of truck, turned to grab tie down, and felt right knee pop. Seen at clinic, diagnosed with right patellar dislocation, released to full duty. Did not get better, had MRI on 9/23, results received 9/26. Soft tissue tear requires surgery. Reportable as of 9/26 diagnosis.
The employee was using a core press, the ram was bent under pressure. When the employee retracted the ram, the ram launched the tube upward striking the employee in the hand resulting in a laceration & forehead (eyebrow).
EE reported stepping off platform step of blue room, tweaking their left knee. Claim initially denied as work-related. EE appealed denial and following medical investigation, claim accepted by workers' compensation on 10/27/22. EE lost time following surgery.
Ee was getting on the bus at the Twin Creeks core shed after work. Ee reports a trip/fall. Ee injured shoulder, chin, shin, and lower back. Seen at a medical facility, diagnosed with shoulder strain, left shin abrasion, left lower back strain. Referred for MRI of right shoulder and orthopedic consult. Released to modified duty.
Ee was checking power cables in a box at the lab. Ee was pulling and backing up, and EE's arm slipped and struck right elbow on the frame of the box. Ee diagnosed with an avulsion fracture and ruptured triceps tendon. Surgery scheduled for 6/10. Ee on restricted duty.
In the process of pulling a column pipe to replace a pump. The column pipe failed and fell inside the casing. This resulted in tensioning of the electrical pump cable that was being spooled. This tension caused the spool to move and is when it contacted the individual next to the spooler. Resulted in facture to tailbone and bone in back, right sprained ankle and skin abrasions.

Ee was lifting a tire/wheel assembly onto the back of a flatbed truck when ee felt a pop and immediate pain in the left shoulder. Seen at a medical facility, xrays were negative for fracture, ee was referred out for an MRI and released to restricted duty.
EE reports driving haul truck ""through rough ruts"", bed shifted, jostling them in their seat, causing back pain. Reported and requested doctor visit. Released to restricted duty that site can accommodate.
EE was bolting underground. As EE was backing away from the bolter, EE slipped and fell to the ground causing an abrasion to left hand and ""jolting"" EE's left shoulder. EE reported pain in the left shoulder. EE was seen at the ER and Golden Health. X-rays negative for fracture, EE referred out for an ortho consult 3/18. Restricted duty.
Ee was troubleshooting a PLC at the autoclave. Ee was kneeling and when ee stood up, knee locked up and ee felt pain. No injury by accident and claim was denied by workers' compensation. Ee released to restricted duty.
Injured employee placed a pipe wrench on a drill rod sub and when activated, the wrench struck the employee in the hardhat above right eye knocking employee to the ground.
EE was lifting a 12/24 jump pack on to a charging shelf. EE felt a pop in right shoulder. EE reported immediately but did not seek treatment. On 2/25 EE sought treatment. Denied by work comp pending investigation. MRI done and torn rotator cuff identified. Determined compensable by workers' comp on 3/9/21.
Ee was walking from the 3893 load center back to the truck. Ee stepped on a rock, it rolled, ee lost balance and caught self. This move caused ee's back to twist and ee felt immediate pain. Ee requested a consultation on 1/25, seen at Golden Health, diagnosed lumbosacral strain, released to restricted duty that site can accommodate.
EE was in sample prep cleaning out a reject chute. EE stepped back, leg slipped, and they felt a pop in knee. Seen at medical on 1/17/22, diagnosed with knee pain and instability, released to restricted duty, no prolonged standing and no stairs.
EE was flying a bin down from charging SAG mill, coming down the second flight of stairs to the bottom floor, the EE's left ankle gave out causing EE to fall on the last step to a small landing. EE was holding hand rail with one hand and had crane remote in the other. Diagnosed with inversion sprain, released to light duty.
Ee working on tails line flanges. Pipe suspended in air shifted after last bolt removed, ee moved to get away from pipe, twisted to EE's left and went down slight embankment. Heard pop in right knee, fell down. Reported incident, sought treatment the next day. Diagnosed with internal derangement of right knee, put on restrictions pending follow-up. Referred to orthopedist.
Ee was pulling column pipe when it lifted up and the pipe landed on EE's foot. EE was seen at Golden Health, diagnosed with a chip fracture to the left 1st toe, and released to full duty with a boot. Site can accommodate the boot.
A crew were conducting sonic sampling. The injured miner was holding a bag to collect soil sample while second minor was striking the core barrel to release sample. The injured miner's hand slipped into the path of a 2lb sledge hammer and was struck. The injury was diagnosed as a laceration to right, middle finger. Follow up appointment, 1/9/2023, reveled non-displaced fracture.
Ee was filling propane tanks. Ee accidentally hit the exhaust valve, and liquid propane came out, striking ee on the right hand and wrist. Seen at ER, diagnosed with 2nd degree burns of hand and wrist. Released to modified duty at next scheduled shift.
Ee walking to turnstile on the way in to work, slipped on ice, fell to ground. Seen at clinic, diagnosed low back strain, released with restrictions. Previously scheduled PTO resulted in no restricted duty days worked. Seen again on 11/13 c/o right wrist pain. Seen again on 11/14, xray negative, referred for MRI, released to full duty.
EE was lifting an impeller that weighed approximately 80 lbs. Within minutes after lifting the impeller, the EE felt pain in lower back. Seen at clinic, diagnosed with a lumbar strain, and released with restrictions the site can accommodate.
EE was squatting to do a job. When EE got up, EE's foot caught on a concrete block, and EE tripped, falling backwards onto their left side on a concrete block. Reported that day. Asked to see dr on 11/7, diagnosed with a fractured rib. Released to restricted duty.
Ee was working on a drill. EE was getting ready to nip in, and hit the spool button rather than the unspool button, catching EE's glove in the spool, pulling EE's hand in. Seen at the ER, diagnosed with a laceration to the left thumb. No fracture, sutures required. Ee released with restrictions site can accommodate.
Injured employee was repositioning a horizontal 20' HQ drill rod to stack neatly on the rod rack when it slid into its slot on the rack, EE's right ring finger was smashed between the drill rod EE was handling and another drill rod already on the rod rack.
Ee was removing/demobing paste pipe. While removing the pipe, it fell, striking the ee on the right foot. Seen at ER, diagnosed with crush injury, referred for MRI. MRI and subsequent x-ray identified two small fractures, ee released to restricted duty that site can accommodated. Referred for orthopedic consult.
EE hooking up grease hose to loader, and coupler was not going on. Cleaned coupler and tried again. used left arm to put pressure on hose and coupler, and felt ""something"" in left elbow. Felt pain following. Seen at medical, diagnosed with a sprain.
Employee was stepping off a cement mixer, took a few steps backward and tripped over a piece of equipment and sprained left wrist.
IP was pulling 7 5/8 casing on the 11th piece walking backwards and about 3/4 of the way down the truck IP lost footing causing EE to fall on the side rail and hitting the IP forearm on the railing resulting in a break to the ulna bone in forearm.
Individual experienced pain in the abdomen at end of shift on 6/7/2002. Next day pain was still present. On 6/8/2022 reported to their supervisor about the pain and thinks an injury occurred towards the end of shift when handling sonic core samples (30 inch, approximately 54-60 lbs). This resulted in an a diagnoses of inguinal hernia. Pending follow-up with specialist.
Ee reports walking to loader and felt calf ""tighten"". Ee then began going up stairs to loader and felt a pop and pain in calf. Seen at clinic, diagnosed with a strain/tear, and released to restrictions site can accommodate. No injury by accident. Appears to be idiopathic in nature.
Ee was dismounting a dozer when ee stepped onto uneven ground, twisting an ankle. Reported and requested to be seen at a health clinic. Diagnosed with right ankle strain and released to restricted duty that site can accommodate.
EE was team lifting a small trailer. EE ran out of room to maneuver and moved their body, extending arms. EE felt a strain in the low back. EE reported the incident and sought treatment at the clinic. Diagnosed with a low back strain and released with restrictions site can accommodate.
Ee getting out of haul truck, door stuck, ee tripped, falling and striking EE's left hip and thigh on a metal bar. Seen at ER, no fractures, instructed to follow-up. Follow-up resulted in diagnosis of contusion to left hip and thigh, ee released to restricted duty.
Ee was using a sledge hammer to remove a back tire from a lube truck. EE swung the hammer, connected with a wheel stud, and crushed EE's right index finger. Seen at Humboldt General Hospital, diagnosed with right index finger distal phalanx fracture and laceration. Sutures, splinted, released to modified duty.
A 30' x 18"" x 6"" beam was being moved out of the batch plant building. The beam rolled and pinned the miner in a pinch point against a concrete block causing bruising to upper right thigh.
Ee was lifting a 10 pound reel of wire from a pick up truck. Ee felt a sharp pain in right shoulder so ee put the reel down to stretch the shoulder. Pain increased and is prominent when ee lifts right shoulder. X-rays negative for fracture and diagnosed with biceps tendonitis. Released with restrictions.
Ee working underground, lifting and twisting at the same time. Ee felt pain in left shoulder and down arm. Seen at ER, diagnosed with pain and put on restrictions for two days. Follow-up on 3/7 continued on restricted duty.
Air hose hit employees hand while winterizing Bean Pump. Sent to get Xray that evening and no fracture noted. Took employee for second opinion on 2/28/22 and found EE has a minor fracture to tip of left hand with sprain. Employee started rostered days off on 2/23/22 and advised of fracture on 2/28/22.
EE was stepping up onto a concrete pad and EE slipped, spraining left ankle. EE was seen at the ER, diagnosed with an ankle sprain, and released to modified duty pending follow-up. Site can accommodate.
Ee reports that co-worker who was providing training grabbed ee's hard hat from behind and twisted ee's neck from left to right forcefully. Ee reports increased pain and stiffness in neck. Seen at a health clinic and released with restrictions site can accommodate.
Ee was using a post pounder to set ground rod. The post pounder slipped off the top of the rod, smashing finger between the rod tip and post pounder handle. Seen at ER, diagnosed with a fracture to the finger tip, and referred to hand specialist. Ee released to modified duty, no use of the injured finger, pending orthopedic consult. Site can accommodate.
Ee stepped off the bus at Phoenix mine site parking lot. EE's ankle turned and EE felt immediate pain. Seen at Golden Valley, xrays being reviewed. Ee released to restricted duty and referred for an orthopedic consult.
EE was helping extract a sheared off lead anode from a EW cell. EE was pulling anode from below floor level with a hook when they felt their left shoulder pop and then had pain and numbness in fingertips. Seen at medical and diagnosed with a shoulder strain. Released to restricted duty that site can accommodate.
Ee was cleaning out carbon screen discharge chute. Another pipe ""y'd"" into drop pipe off chute. Ee pushed on the pipe while ee's hand was on the handrail. The line came apart and landed on ee's thumb. Xrays negative for fracture, laceration required stitches. Ee released to restricted duty.
On 1/8/2022, EE was wrapped up in a rotating hose after trying to straighten the hose out without de-energizing the equipment or placing controls into neutral. EE was treated for a dislocated elbow. EE received prescriptions from the facility. EE was released on light duty restriction until 3/24/2022. Final disposition information to be provided on 10/5/2022.
Lube tech was changing a filter on a CAT 777D haul truck and caught their hand between the ratchet handle and a hydraulic hose causing an avulsion to the first knuckle joint of their right hand requiring 9 stitches to close.
Ee reports that ee spotted incorrectly into lime silo, then rubbed eye with lime on it from the handrail, resulting in exposure to lime in eye. Seen at an eye clinic, diagnosed with a corneal abrasion to the eye, prescribed meds for the eye and released with restriction that site can accommodate.
Blocked transfer line impeded (frozen product) the flow of material. In an attempt to push the product through to the receiving tank, the driver attached an airline. The pressure caused the material to mist from the air connection exposing the driver to the sodium hydroxide. The result was a chemical burn of left hand; full thickness burn of left forearm.
Ee was positioning a tire when the wheel lifted up, pinching EE's right middle finger between the tire stand and the wheel. Seen at ER, x-rays negative for fracture, sutures required to close the laceration. Ee released to full duty.
While installing a light fixture the tin lid that covers the roof mount slipped hitting the electrician in the chin which resulted in a minor cut.
Ee had a hand on the face of a sheave to guide drive belts on to the motor. The belt grabbed ee's glove, pulling hand into the belt, catching the right index and middle finger. Seen at a hospital, diagnosed with a small tuft fracture and fingernail avulsion, and released to full duty.
Worker stepped onto uneven surface, straining knee - minor meniscus tear.
Operating 5600 gallon water tanker. Loading water through rear valve. Did not insert R-Pin to lock valve open. Transfer pump created pressure against open valve. Operator inadvertantly contacted valve with right pinky finger. The valve slammed shut and pinched tip of right pinky finger. Finger was lacerated.
Ee was jostled in the cab of EE's haul truck when shovel operator dropped a load. Seen on 8/31 for initial visit, diagnosed with a lumbar and thoracic strain, released to full duty. On 9/12/22, ee referred for physical therapy, making this reportable for treatment.
Ee was in loader, ee stretched, and ee's hand contacted the fan which did not have a cover on it. Ee was seen at the ER, diagnosed with a laceration that required stitches, and released with the direction to keep wound clean and dry. Returned to work full duty.
Ee was working to unplug sump pump. There was a hole in a boot (PPE not appropriate) and slurry got in the boot, causing a chemical burn. Ee was seen at hospital, diagnosed with a chemical burn to the foot, and released to light duty. Ee was offered duty within restrictions, but refused and took PTO. Ee did not qualify for disability as modified duty was offered.
Ee installing insulation between a torque converter and engine on loader. Lost grip on insulation and left index finger slid across the hard rubber edge of the firewall, cutting finger. Seen at NNRH, sutures required to close wound. Full duty, must wear gloves and keep wound clean and dry.
Ee was passing between a vehicle and the rib underground when EE struck a piece of wire protruding from the rib with EE's forehead. EE received a laceration to EE's left eyebrow/eyelid. Seen at ER, diagnosed with a laceration, stitches required, and ee released to full duty.
Employee was working the cart, liner shifted causing employee to fall back - grabbing the cart and crushing the tip of their left index finger.
EE was getting lime at North Area lime silo. When driving away from the silo, they noticed a burning sensation in eyes and started to cough. EE reported the issue and was seen at health. Diagnosed with chemical conjunctivitis to left eye, scheduled to follow up 4/29. On follow-up EE was released to full duty effective 4/29. Prescription for eyes given.
Ee was grinding a weld out when the grinding wheel EE was using blew apart, sending fragments that struck EE's thigh and face. Seen at Golden Health, sutures required to close wound on thigh. Ee released to return to work full duty 04/04/22, EE's next scheduled shift. No lost time.
Employee was loading rounds and was struck by material from the face.
EE was moving air line to lock it out, and the air line came on and hit them on the knee. Reported it on date of injury, but sought treatment on 3/25. Diagnosed with abrasion/contusion/infection to right knee. Antibiotics prescribed to treat infection. Released to full duty.
EE being loaded at shovel on 191. Hoist on shovel went out, the shovel struck the bed of the truck forcefully, leaving the EE in an awkward position for a short period of time. Initial impact jostled EE in cab. Seen at health center, released with restrictions.
Ee stepped off truck onto rock, turning ankle. Reports popping/slight pain. Woke up next morning with pain/swelling/bruising to rt ankle. Text reported to supervisor. Seen at clinic, diagnosed with sprain, released to restrictions for 5 days. Site accommodated. No restricted days worked, ee suspended pending investigation.
EE was walking to the skid steer to move some heel plates. EE was on uneven ground and reports that EE either stepped in a hole or on a rock and ""felt ankle crack"". EE was seen at the ER, diagnosed with a sprain, given crutches and an ace wrap. Referred out for an ortho consult. Restricted duty.
While hooking up to off-load cement, the product hose slipped off the fitting, came back and hit driver in the mouth causing a contusion inside EE's mouth on EE's lower lip. It wasn't deep enough for stitches but EE was prescribed an antibiotic.
Ee was replacing a hoist cylinder with another ee, and directing the ee to raise the cylinder to put in the top pin. Injured ee heard a big bang and then was struck in the face with what the ee believes to be a taylor fork from the cylinder. Seen at ER, CT scan negative, received stitches to laceration in mouth, released to full duty 2/25/22.
EE was trying to step down from the tailgate to the bumper of a pickup when they slipped and fell to the ground. Twisted a knee. Experiencing ongoing discomfort and requested to be seen. Seen on 4/4, diagnosed with an MCL sprain, referred to physical therapy, and released to full duty.
Ee was cleaning cab of haul truck. Ee sat back down in truck after cleaning and closed door on thumb. Seen at a health clinic, diagnosed with a tuft fracture and fingernail avulsion of the left thumb. Referred to hand surgeon for consult. Released to full duty effective 2/16/22.
DURING CLEANUP FOR THE DAY WHILE CUTTING RUBBER ROD FOR FLOORING, CONTRACTOR CUT SELF WITH UTILITY KNIFE ON THE WRIST. A BANDAGE WAS APPLIED BY THE EMPLOYEE AND EE WAS NOT CONCERNED, FINISHED CLEANING FOR THE DAY AND LEFT SITE. ON THE WAY HOME DECIDED TO HAVE IT CHECKED OUT BY URGENT CARE, RECEIVED 8 STICHES.
Individual was in the process of installing a new starter motor located in an enclosed outdoor air compressor housing. During this process, the individual strained their back.
Ee tripped getting off piece of equipment. Ee fell forward and caught self with left arm. Reported on date of injury and requested to see doctor when pain did not resolve. Seen at a medical center, xrays showed midclavicular fracture. Referred to orthopedic surgeon for consultation. Released to full duty.
Ee was holding a piece of pipe in place for a mechanic when a piece of carbon fell into ee's eye. Ee continued to work and did not report the incident. The next day, 1/23, ee felt pain in the eye and reported the incident. Seen at the ER, given an exam and prescribed eyedrops for inflammation. Released to full duty.
Ee slipped on berm and fell, striking hand on rock. Seen at ER, diagnosed with laceration to the left hand. Sutures needed to close the wound. Ee released to full duty.
Ee reports irritation in EE's nose and eyes due to dust in the cab of EE's truck. EE was seen at Golden Health and prescribed eye drops for conjunctivitis. Ee released to full duty at next scheduled shift.
Ee was tramming shovel/excavator and struck fiber optic cable. Ee jumped off equipment without confirming it was safe to do so. When ee got home and saw ee's family, ee experienced stress and anxiety when ee realized what could have happened. Ee was seen for counseling and filed a claim for benefits. Diagnosed with anxiety/stress reaction from the incident. Claim accepted on 12/8.
Ee doing preventative maintenance on shovel 18. EE reported exposure to heat. Seen at ER, diagnosed with heat exposure/dehydration, and released with restrictions for two shifts.
EE reports starting to feel sick around 2pm. AC unit in their truck blowing warm air since 11am. EE downed the truck at 2pm due to no AC. Seen by EMTs, and then at ER, diagnosed with heat exposure and dehydration, no treatment required, given one day off.
Ee was sitting parked in EE's haul truck in queue area. Ee driving haul truck 618 backed into EE, striking EE's truck with the dovetail. No physical injury. Ee reported the next day that EE was having flashbacks to the incident. Seen at clinic, diagnosed with post-traumatic anxiety, and released to one day of restrictions.
EE reports having used a hammer to drive panels into place, and feeling a burning sensation in biceps and shoulder. They reported the pain on the date they first felt it, 8/17. EE requested to see a provider when pain didn't go away. Seen on 8/29, diagnosed with deltoid tendinitis, and released with restrictions site can accommodate.
Ee sustained a Standard Threshold Shift in hearing. Confirmed reportable noise induced hearing loss. Confirmation received from an Au.D. CPS/A, on 7/27/22. Retraining and refitting complete.
Ee was sitting in a pickup on site. A rubber-tired dozer backed into the pickup, jarring the ee. Seen at Golden Health, diagnosed with back pain, and released to restrictions for days off. On follow-up prior to EE's next scheduled shift, 5/26, EE was diagnosed with PTSD, prompting this report for the illness.
Ee reports lifting and pouring pans of dirt. When EE took off EE's tyvek, EE noted swelling in the right forearm. No report of injury, and ee reports EE does not recall hitting, bumping, or otherwise injuring EE's arm. Seen at clinic, no injury identified, xrays negative, ee diagnosed with pain and swelling that doctor believes was industrial.
Ee welded on 2/13, became sick with flu-like symptoms. Felt better after ee went home. Welded again on 2/17, became sick again with flu-like symptoms. Seen at Golden Valley, testing ruled out other diseases, ee diagnosed with metal fume fever. Seen again on 2/22 and released to full duty 2/23.
Received confirmation by Professional Supervisor of a reportable hearing loss in the right ear on 2/13/22. Retraining and refitting completed.
EE developed chronic cough. Sought treatment for cough resulting in diagnosis of pulmonary fibrosis/silicosis. Lung biopsy taken 3/2/22 confirmed. On 4/13/22 confirmed compensable and accepted by workers' compensation.
Ee complains of tingling and burning in wrist, diagnosed with carpal tunnel in the left wrist, confirmed occupational by orthopedist on 1/21/22. Ee released to full duty.
Mine conditions did not contribute to this situation, and no impairment was sustained from this incident.
This was a death by natural causes. Employee collapsed at the mine site at the employee's truck, but passed away at the hospital in Elko, NV. There was no actual accident.
EE lifted an 80 lb penco into back of pick-up when EE felt a pop in EE's right calf. Slid the object further into back of truck, and was unable to walk. Fell on the ground and had a small bruise. Did not request treatment until 4/26. MRI on 4/27 identified tendon rupture resulting in this report.
EE standing behind pickup with tailgate down loading tools. Pickup driven by another EE was driven into them, pushing them forward between the two vehicles. Seen at ER, CT scan and xrays negative for fracture or soft tissue damage. EE released to full duty on next scheduled shift. Referred to PT on 3/29 resulting in MTI.
