Operator was in the process of unplugging 604 sump when they saw flames coming from the roof of boiler #2. Fire stayed contained to the metal roof and did not damage the surrounding area.

A bolter operator was bolting the nose pillar in the FED 2627. The operator noticed the back move and while attempting to back out, the ground failed causing 50 tons of material to fall to the ground past the 8' primary anchorage bolts. Small material contacted the boom of the bolter causing no damage.
UG supervisor was performing a routine inspection and discovered a ground fall on the 6190 North-150. It was the 2nd round in, and it was unbolted, it was working and broke back into the last wrap of supported ground. It was starting to work past the ground support and it was decided it needed reported at 13:30. MSHA was called at 13:39, and had the area barricaded off.
Employee was loading trucks with an LHD and started smelling smoke. EE noticed smoke in the air, and hit estop on LHD. When EE got out, EE believed EE saw flame under loader and deployed fire suppression. EE called a Mayday, and mine was evacuated.
Business partner shifter noticed bagging of ground in the SRM 3285 and had new 8' swellex installed but they weren't anchoring. They made a plan to remove the bag and install 12' s their next shift. When the operator entered the heading on EE's next shift the bag was on the ground having pulled past the 8' swellex
#2 service hoist had a system issue due to a DHS fault triggered during routine maintenance by the operator, causing the HMI to lock up after a system reboot. Electrician contacted Tiley, hoist control system experts, and collaborative troubleshooting revealed an older operating program. A controlled system rebooted with an updated IP to address successfully restore operation
The operator of haul truck 619 plug dumped material on the 5110 elevation and rolled forward to allow the material to clear the bed. The bed contacted the overhead powerline (25KV) and fiber optic cable. Miner was trapped in the cab of the haul truck for more than 30 minutes. No injuries.
While troubleshooting the approach switches on the 4704 production hoist, it was found that there was damage to the wiring harness. As the wires were moved to see the severity of the damage, the main breaker in the E house tripped causing power outage to the production hoist.
#3 hoist tripped out with a drive fault at 12:55 am. All of the connections used to power the service hoist VFD were checked, and the hoist 4705 had a fault. As a result, the breaker was repaired and is being tested until end of shift on 9/21/2023. There were no personnel on the cage at the time of the event.
A faulty connection was identified on the Main Control Relay (MCR). During this time, we also found CR2 contactor getting stuck. The unit was replaced with a new one. There were no personnel on the cage at the time of the event.
During a storm event, TRUG experienced a power bump that ultimately caused the #2 hoist to go down
During a power outage our computer system recorded an under-voltage fault and the Genset did not auto switch as intended causing the system to fail.
During a power bump the hoist computers lost power, and when the power was restored, the software was having trouble displaying the correct version of the Tiley HMI program. Resulting in the number hoist being down for 44 minutes.
While performing 24 hour functionality test, it was found that the safety latches were not fully engaging the shaft guides.
On Hoist #3 not all of the dogs were lining/coming into contact with the guides, which caused the hoist to become down until necessary repairs are made. Having Hoist #2 and Hoist #3 both down, we evacuated the mine. Hoist #3 went down on the nightshift of 7/07/2023.

#2 and # 3 Hoist down at 12:01 A.M. due to a power bump. Power restored at 12:10 A.M. #3 hoist immediately reset. #2 Hoist was not able to reset until 12:47 A.M.
While performing an inspection of the mine, the supervisor came across a fall of ground.
While performing an inspection in an old heading, it was found that a fall of ground had occurred.
During a thunderstorm, Turquoise Ridge experienced a power bump. When electricians reset the power, the hoist computer screen did not come back up causing the computer to lose its settings. The setting needed to be inputted manually.
Hoist number 2 went down for an under voltage card issue. Hoist #1 was still out of commission waiting on an encoder to be replaced. At 5:00 P.M. we evacuated the mine due to two of our 3 conveyances being out of service leaving us only 3rd shaft. All miners were out of the mine and accounted for at 5:51 P.M. The hoist came back up at 6:40 P.M.
Encoder malfunctioned causing hoist to not function at 100 percent capacity. Hoist was downed until encoder can be replaced.
The 3rd shaft hoist went down due to a blown fuse in the control circuit for the PT. Heat in the room combined with the tight tolerance of the PT fuses was the root cause. The fuse was repaired and the AC unit is also being repaired to prevent reoccurrence. The hoist was back up at approximately at 10:30 P.M. MSHA was notified within time.
While being loaded with muck from the 3944-119, haul truck 101 was parked in the 3950-215 cab first. As loader 94 was placing a bucket of material into the truck's bed, a fall of ground occurred 15.5x16x11 releasing material onto the bed/articulation of the haul truck. The operator could exit the cab, evaluate the condition of the back and find a safe path out of the 215 drift.
The elevator attendant in Mill 6 was operating the elevator and had dropped a crew off on the 7th floor and was coming back down when due to loss of power the elevator stopped between the 5th and 4th floor. This elevator is well ventilated, we had clear communications, and no time did the operator feel distressed. No injuries or property damage associated to this event.
While routine maintenance was being performed - with no one in the conveyance - the brakes locked up bringing the hoist to a stop. Operators could not immediately reset the brakes and they began troubleshooting. The brakes were able to be released and the conveyance was put back into service.
While traveling to the dump, the operator of HT304 noticed smoke coming from the truck. Operator hit the emergency stop and the fire suppression. Operator exited the cab and went down the ladder to the ground safely. At this point the truck was actively burning.
While performing daily hoist checks the hoist drive was found to have 2 blown fuses, which shorted out the SCR. This caused an outage lasting longer than 30 minutes. Electricians were notified and they replaced the fuses and put the hoist back into service.
An unplanned non injury fire occurred at the the Goldstrike radio communications shop building.
A utility crew was assigned to complete rehab work at the 1600 haulage corridor, which entailed removing utility lines. A team of three employees were utilizing a scissor deck with a pipe handler to remove 10"" steel pipe. They had removed one pipe section and were starting to disconnect the second pipe when they sustained injuries.
EE was closing the window on equipment. They caught their right pinky finger in the window, and amputated the tip of the pinky. Seen at ER, consulted with hand specialist at UofU, and referred to them for treatment. Seen on 5/10 and kept off work pending follow-up on 5/31. Pinky amputated to first distal knuckle. 50 lost days.
EE using 1.5 ton crane moving motor. The motor was on the manual crane approximately 4"" from the cat walk, and the hoist system failed, and the motor fell, striking the EE on the head/shoulder/back. Seen at ER, diagnosed with compression fracture to spine, laceration/soft tissue injury to right leg. EE put off work, non-weight bearing.
Ee stood up from seat of bus and felt pain in EE's back upon arriving at work. No incident or accident, workers' comp denied ee's claim for compensation. NGM is reporting based on MSHA requiring any ""injury reported that occurs on site"" regardless of compensability or outcome of investigation. We have no evidence that an accident or incident occurred.
EE reports being loaded at loader 46, and being ""shook hard"" when the loader was leveling out material. Reports neck jolted sideways and feeling immediate pain and migraine. Video review does not support verbal report. EE reporting severe pain at follow-up appointment and put off work pending follow-up.
Driller wanted to discuss with the blaster what kind of burn cut was drilled in the heading and what was needed to load the round. They examined the round on the face, and when they turned around to head back to the drill, material fell on them, striking them both from behind.
EE was carrying 137lb flange to weld location, kicked rock with toe of boot and started falling. EE released left hand. Flange landed on right hand. Open laceration to R 3rd finger, vascular compromise, requiring 14 stitches, and broken bones at first knuckle requiring surgery to repair.
EE reports that on 8/19 they were lifting and moving pumps weighing approximately 70 lbs. when they felt a pop in left shoulder. Reports having chronic pain in same shoulder, had steroid injection in same shoulder on 7/6/23. Was supposed to have MRI a month ago but did not. Diagnosed with left shoulder pain, released to restrictions.
EE was working with another EE on a Geho pump. Removing diaphragm door, took last nut off, door popped off, and hot slurry struck the EE in the face. Seen at ER, trasnported by air to Medical Center for burn consult and x-rays of arm/shoulder. No fracture, EE treated and released for burns. Released to full duty for burns effective 8/2.
Ee was working with another ee on a Geho pump. Removing diaphragm door, took last nut off, door popped off, and hot slurry struck the ee in the face. Seen at ER, transported by air to a Medical Center in Vegas for burn consult. Treated and released, able to RTW full duty 8/2 per a Dr. at burn center.
EE changing wear iron on loader. In the process of hammering the wear iron off the loader, a piece of shrapnel dislodged and struck the EE in the left leg. Seen at ER, referred to surgeon. Shrapnel surgically removed, EE put off work pending post-op appointment.
Ee tripped on welding screen and fell on elbow. Diagnosed with a fractured olecranon process and referred to orthopedic surgery.
EE was dismounting equipment when ladder rung broke. They fell about 18 inches and reports twisting their left knee and ankle and hearing a pop. Diagnosed initially with contusion. On 4/7 MRI confirmed strain, no tear, EE put on restrictions site cannot accommodate. Confirmed reportable on 4/7.
An employee was troubleshooting a bolter for a hydraulic leak. EE reached EE's hand into the floorboard where the hoses were located when EE sustained an injury caused by a high-pressure hose line.
Ee was getting fuel and coolant put in machine. Walking to hand fueler the radio, slipped and fell on back and shoulder. Seen at ER, diagnosed with lumbar fractures, referred for a spine consult. Ee put off work.
EE was walking to shift change bus at the 5 points queue, Carlin South Area. EE slipped on ice and fell. Seen at ER, diagnosed with pain and contusion to the low back, and released to work. Able to return to full duty as truck driver. Subsequent follow-up on 2/28 resulted in referral to physical therapy, making this reportable for treatment.
EE reported loader striking side of truck repeatedly on three separate loads. They reported pain which got progressively worse. Workers' compensation investigated claim and accepted as occupational on 3/23/23 making this reportable for lost time.
Ice on ground. Slipped when walking. Distal fibula fracture closed, commuted. Extent of impairment unknown. Referred to podiatrist, awaiting worker's comp information to be seen by podiatrist.
Ee reported that EE was filling and lifting bags of floc and heard a pop in EE's back. Statements indicated this may be related to non-occupational shoveling. Seen at clinic where EE reported pain. X-rays negative. Ee released to light duty site can accommodate. After investigation, accepted by workers' comp 2/2/23.
Two employees were using a telehandler to assist in moving a tractor that had broke down. The tractor could not be shifted into gear. One employee was operating the tractor and the other employee was between the telehandler and the tractor hooking up a chain. The tractor unexpectedly moved backwards striking and pinching the employee between two pieces of equipment.
While walking around truck checking to confirm tire chains were in place driver slipped on ice/snow. While falling driver put right arm down to break the fall and caused injury to right shoulder.
Ee was working with another ee fusing pipe. EE reports moving a piece of pipe with arms extended forward, trying to position the pipe for fusing when EE felt a pop in EE's left arm. Seen at ER, diagnosed with a left bicep injury. Ortho referral resulted in surgery, done on 12/11.
Ee performing walk-around of haul truck, tripped on small rock, fell, landing on knee and hand. Reported and continued working. On 11/13 sought treatment, diagnosed with pain, referred for MRI. Released with restrictions site can accommodate.
Ee reached around another ee to help hold discharge gate, and ee felt a pop and burning sensation in right shoulder. Sought treatment on 11/20, initial diagnosis was pain and arthropathy. MRI on 12/4, on 12/5 diagnosed with a tear in bicep. Put on restrictions, making this reportable.
Ee was walking a drill pattern tying in a control row. Ee fell into a drill hole, and right leg was stuck in the hole up to ee's thigh. Ee was seen on 8/19, diagnosed with a bruise and released to full duty. MRI on 9/18 showed a complex meniscus tear and ee referred to orthopedic surgery, making this reportable.
Ee was participating in confined space training at Gold Quarry. When being pulled out of manhole, the tripod fell over, pinning EE's left arm. Seen at clinic, diagnosed with left shoulder sprain and chest wall strain, and released to full duty. Follow-up on 8/29 resulted in referral to physical therapy making this reportable for treatment.
Ee wringing out mop and mop bucket broke. Left hand went in mop water, and right hand grabbed edge of bucket to prevent self from falling. Reported and called Work Care. Pain did not resolve, sought treatment on 7/25 and released to restricted. Follow-up on 8/1 continued restricted. Site accommodating.
Ee was placing a metal skid plate onto a loader bucket. The skid plate attached to the clevis when the clevis rotated. Skid plate smashed ee's right thumb. Seen at clinic, diagnosed with a fracture and referred to orthopedic surgery. Currently released to restricted duty.
EE went to empty used oil tank, go the evac hose, and slipped off the second step to the cab. They fell approximately 3 feet from the step, to the ground, landing on their butt. Taken to hospital, x-rays show 2 fractures to vertebrae. EE put in brace, referred for orthopedic consult.
EE was mounting a haul truck wheel. Stood tire and wheel up to sit the lock ring, turned to walk away to continue working, and felt the rim fall on them. Initial report is that the rim ""popped"" out. Investigation is ongoing. EE transported to hospital, diagnosed with multiple fractures, and transferred for surgery. Surgery took place approximately 10 am on 5/24.
Ee was doing walk-around on ee's grader. Ee reports slipping on a rock, sticking leg out to break the fall, and hyper-extending left knee. Seen at clinic, diagnosed with a strain to the left knee. Released with restrictions that were continued on follow-up. Site can accommodate.
EE was putting down chocks for the van. EE was going to help put cones out, the road was icy, and they slipped and fell, landing on their right arm and shoulder. MRI on 3/1 confirmed rotator cuff tear and distal clavicle fracture.

Ee reports stepping off lube truck and EE's ankle twisted causing a sprain. Seen at ER then at Golden Health. X-rays negative for fracture. Ee diagnosed with ankle sprain released to full duty for next scheduled shift.
EE was walking in the process area when EE's boot caught on the grating and EE fell, striking their right knee and left hand. Seen at Golden Valley. Lacerations to right knee and left hand were sutured and EE was released to restrictions effective 11/21 that site can accommodate.
EE reports cleaning mud off concrete with a snow shovel, shovel got stuck in crack and the handle struck them in the right side. Diagnosis chest wall contusion. X-ray confirmed no fractures. On follow-up on 8/28, EE released with restrictions, making this reportable.
EE was moving a crate. When they went to set it on the flatbed truck, the crate slipped and it fell on their finger. Seen at clinic, diagnosed with a tuft fracture and referred to orthopedic. Released with restrictions site can accommodate.
EE was walking around the pickup to the drivers side. EE stepped on a rock, twisted foot and fell back against the truck, striking their right elbow. Seen at Golden Health, diagnosed with a contusion and nerve injury, released with restrictions site can accommodate.
EE was on a drill pattern pulling stakes when they stepped in a drill hole, twisting an ankle. EE was seen at clinic, diagnosed with a chip fracture and a sprained ankle, and released to restricted duty that site can accommodate.
Work Shift started on 6/01/2023. Incident occurred on 06/02/2023. Employee was using an angle grinder to buff rubber on floor of a flotation cell. The angle grinder kicked back resulting in a laceration to right knee.
EE was unloading cement at the rear of EE's trailer, the loader operator on site was filling the pug mill screen and sent rocks off the back side where EE was standing. EE ran to avoid being struck by rock tripping over a discarded wheel chock rolling EE's foot and fracturing EE's big toe.
Ee was lifting boxes of crucibles from a pallet to stacks for weigh-up. Following this task, they reported feeling pain in low back. Seen at clinic, xrays negative for fracture, diagnosed with lumbar strain and sacral/low back pain. Released with restrictions that site can accommodate.
Ee was moving a barrel and it shifted, landing on left foot. Ee was seen at clinic where xrays were negative for fracture. Diagnosed with left ankle strain and released with restrictions site can accommodate.
Ee picked up a 90 lb drill bit, using forearms as a fulcrum, out of the back of driller's truck. Ee was seen at the ER, diagnosed with a fracture, and referred to the orthopedist. Released to restricted duty that site can accommodate effective 4/24/23.
Individual was in the process of moving the hammer assembly from the truck bed to the rod rack. When doing this individual fell same level to the ground and the hammer inter change landed on their leg. Change of status 05/11/2023.
Ee was walking down a small set of stairs, felt sharp pain in upper back. Prior to walking down stairs, EE reports having collected sample and assisting the 5 inch bull feeder. Leaving reclaim and walking down stairs when felt pain. Investigated and accepted by workers' comp on 3/27, reportable for restricted duty.
Ee was opening cab door on equipment. EE had to push hard on the button, and EE's thumb slipped, jamming against the door with force. Ee reports feeling pop in EE's thumb. Seen at clinic, diagnosed with a flexor tendon injury to right thumb, released to restricted duty.
Ee was working on retention screen rebuild. EE removed a beam and felt a pinch on EE's right hand. EE went to the lunch room to check EE's finger and noticed a large cut. Seen at clinic, laceration was closed with 10 stitches. Released to restricted duty site can accommodate.
Ee was replacing the ""E"" cyclone when the wire/cable sling broke, striking the employee's right hand. Seen at a medical center, xrays negative for fracture, diagnosed with a contusion to the right hand, and released with restrictions the site can accommodate.
Ee was using a box cutter to open a box of crucibles. The guard broke, and the blade cut ee's wrist. Ee was seen at the ER, stitches were required, and ee was released to modified duty site can accommodate.
Ee was on site walking from the bus to the break room. Ee slipped on ice and fell, striking knee and shoulder. Ee was seen at the ER, diagnosed with a sprain to the right knee, and released to modified duty that site can accommodate.
EE slipped while boarding TD-32 equipment. EE caught self with left arm. Seen at clinic, diagnosed with neck, upper back, left shoulder strain. Released to modified duty that site can accommodate.
Employee was unloading wire mesh out of the mucker bucket. EE lifted the corner of the wire mesh when EE slipped on another piece of mesh panel, causing EE to fall and catch their hand on the mucker bucket causing injury.
Ee was torquing nuts on a valve. Ee was adjusting the open-end wrench and when ee began to torque, the wrench moved and pinched ee's pinky finger between the torque and the open-end wrench. Seen at clinic, diagnosed with distal fingertip fracture, released to full duty, referred to hand specialist.
Ee was using a torque wrench on a bolt. The wrench slipped striking ee's finger. Seen at a health clinic on 10/31 and 11/1, diagnosed with a small fracture to the fingertip, and released to full duty.
Ee was working with another person putting a drive line back on a carrier bearing. The weight of the drive line shifted, and the ee's hand was caught between the drive line and the come-along. Seen at clinic, laceration closed with stitches. Seen again on 10/9, released to full duty. No restrictions, no lost time.
Attempting to break top sub off tooling using chain wrench while drill was rotating. Wrench struck employee in face resulting in laceration requiring stitches.
Driller helper was in the process of adding a quill rod back on the drill string. EE missed placing the rod in the rod roller stop causing the quill rod to fall on the right dorsal area of EE's hand striking the box end of the rods protruding out from the rotation unit.
Ip was trouble shooting a leak on the drill when pulling hand out hand made contact with the burr on the pipe causing a laceration to the right hand
Employee was working in the engine bay and had to get to the back of the truck on the torque convertor. Went to close rear hood. Stepped on the left intake tube to reach the latch and lower hood. Upon stepping on intake tube lost foot placing due to boots being muddy/wet from the rain. Left knee landed on a clamp that was pointing up. Causing a laceration to the left knee.
EE was changing a filter with a partner. They noticed that the lid was not flush and the filter was not seated correctly. Working to seat it, the lid came loose and pinched the EE's hand between the lid and the filter housing. Seen at the Hospital, diagnosed with a fracture to the right middle finger, and referred to a hand specialist. Released to full duty.
EE was filling the accelerator tank on a spray truck when the hose split before the base of the valve, causing it to spray out and go into the injured employee's right eye.
EE was turning the upper master key on a dozer to shut down the equipment. They struck their pinky finger on a piece of metal, dislocating a finger. Seen at health clinic, diagnosed with a chip fracture to the right pinky finger and released to full duty. EE has history of dislocating this finger.
EE was walking through an air door to get to EE's haul truck when EE was hit by a gust of air and fell over. EE struck their head. EE was seen at the ER, x-rays were negative, and a small puncture wound on EE's head was closed with two staples. Released to full duty.
The employee was working with a driller installing a table plate when the employee decided to do the same with other table plates on the deck, during that process a table plate fell over pinching EE's left index finger tip.
Ee being loaded by hydraulic shovel and reports bucket ""slammed"" into bed of truck jostling ee in the cab. Reports pain in back, seen at the clinic on 7/7, diagnosed with cervical, thoracic, lumbar strains, released to restricted duty. Medical and factual investigation completed 7/26, claim accepted by workers' comp 7/27, resulting in this report.
EE was walking with a blast box, tripped over a rock, and struck hand on a wire. Wire went through glove, puncturing right hand. Seen at the ER, received a tetanus shot, and on follow-up at the clinic was diagnosed with an infection. Released to full duty.
Moving a piece of steel, lost footing and corner of steel plate contacted miner's right leg causing a small laceration.
EE on bus on the way to work, bus hit pothole causing EE to go forward in seat, striking face on the seat in front of them. EE seen at ER, sutures to lip, and two broken teeth. Referred to dentist. EE released to full duty.
K order in place investigation on going. 6/2/23 - K Order still in place pending equipment retrieval plan approval. Preliminary investigation showing mechanical failure. Waiting for equipment to be able to make final determination.
While changing a fuel transfer pump, technician went to clean off oil pump hardline with Brake Kleen, sprayed above EE's head but the nozzle was facing EE instead of away. Brake Kleen hit EE in the eyebrow and ran down under EE's safety glasses into EE's eye. Immediately flushed eye out at the eye wash station, then flushed again with saline solution.
EE was installing a starter in a haul truck. Starter slipped, pinching the EE's finger. Seen at ER, laceration to tip of middle finger, fingernail removed. Sutures and wound care provided, EE released to full duty 3/30.
The crew was prepping for a top head change and decided to try to pry the spindle tube free when the spindle tube became unstuck it dropped IP stuck EE's hand in a pinch point to grab the fallen spindle tube causing a cut to EE's ring finger on EE's right hand.
Driller was in the process of loading a 5 foot piece of 3.25 inch Matrix rod from the rod stack to the jaw of the rod handler, when right middle finger was pinched between the rod and handler jaw. Driller immediately pulled hand from between the rod and handler jaw and noted the injury to the right middle finger tip.
Ee driving tractor underground, reports being unable to downshift and slow tractor. Ee reports ribbing the tractor and jamming right wrist into steering wheel. X-rays negative for fracture. Ee released to light duty site can accommodate.
Ee was walking through RIL containment area and slipped on resin/ice that was covered with water. Fell and twisted right knee and lower back. Saw doctor on 1/31, diagnosed with strain lower back and tendonitis in right knee. Follow-up on 2/7 resulted in referral to physical therapy making this reportable for treatment.
Ee was using a box cutter to cut a belt. Ee was pushing down on box cutter with both hands, and hand slipped, cutting ee's finger. Seen at ER, laceration to finger given one suture. Released to full duty.
Ee was cleaning metal shavings from the lathe machine. While pulling the shavings by hand, some got struck and cut through ee's leather gloves and ee received a laceration to the tip of the right thumb. Seen at ER, sutures required, and ee released to full duty.
EE sustained a Standard Threshold Shift in hearing. Confirmed reportable level hearing loss. Occupational noise could not be ruled out as a factor in the noise induced hearing loss. Retraining and refitting complete.
Ee was working to unclog a discharge line. While spraying water up the end of the line, pressure built up, the clog dislodged, and the ee was sprayed with liquid. Ee reported incident occurred at 11:30. Ee saw a provider at 4:00 pm, diagnosed with contact dermatitis from the exposure to the liquid. Released to full duty.
Ee sustained a Standard Threshold Shift in hearing. Confirmed reportable noise induced hearing loss; workplace noise cannot be ruled out as a factor in the hearing change. Confirmation received from AuD. CPS/A, on 07/26/2023. Retraining and refitting complete.
Ee reports pain that began in June or July of 2022. No specific incident. Off work effective 3/30/23, went on STD. Following MRI, diagnosed with herniated disc T9-10. Specialist told ee that this appears to be caused by cumulative trauma at work. Filed WC claim 5/19, WC claim accepted 5/25/23.
Ee working in slurry 1/8-2/9/23, wearing leather boots. Over time, EE developed a rash on EE's feet. Seen at clinic, diagnosed with contact dermatitis. EE was released with restrictions site can accommodate.
Ee backfilling. Haul truck bed flipped, EE stopped, called shifter. No injuries. Began to have feelings of anxiety, lost confidence, questioned self. Reported this to NGM 3/9/23. Filed claim for workers' comp, occupational illness, acute stress reaction, accepted on 3/17/23.

This employee was a response team member who performed CPR on the coworker who was fatally injured.
A utility crew was assigned to complete rehab work at the 1600 haulage corridor, which entailed removing utility lines. A team of three employees were utilizing a scissor deck with a pipe handler to remove 10"" steel pipe. They had removed one pipe section and were starting to disconnect the second pipe when they sustained injuries. This ee was the scissor deck operator.

Ee found on floor of mill locker room, blood on floor. Ee reports passing out. Ee was not coherent, unaware of where ee was, or who ee's coworkers were who found ee. Transported to ER, diagnosed with a non-occupational condition. This incident was immediately reported as a courtesy. No evidence of work-related event/condition/injury.
EE was helping utility crew hang vent bag. EE grabbed a cable to hang the vent bag, turned around, and ran into a piece of protruding wire. Seen at clinic and referred to eye doctor. Diagnosed with a laceration to the eye and released to full duty effective 6/16.
An operator was bolting in the CTR 2970 at approximately 9:20 a.m. The operator was about to readjust bolter to begin installing more ground support. A small rock began to fall. The ground support began to fail past the long support anchorage. The operator got out of the cab safely but the boom of the bolter was covered in material.
