Accident type, without injuries

On 12/17/21 at approx 0900 hrs, a fall of ground was witnessed by a miner working in the area. Miners were already actively rehabbing the area and reported this new ground fall to supervision (not beyond ground support). Because the rehabbing of the previously identified ground fall, was already underway this new material that trickled out was not perceived as reportable to MSHA.

The inside flange on the inside tire of the left rear dual broke allowing it to rub on the wheel motor bolts and become hot. Once the water truck was stopped the flange rested on the tire causing it to catch on fire.

The operator of a mucker was loading a haul truck out of a muck bay at the 6245 level. While backing out of the muck bay the operator had the bucket in the air while fully articulated and on a 2-3% grade. The mucker became unstable and slowly tipped against the rib. The operator was not injured but needed a man basket to exit the mucker.

At approx. 0700 am, fixed maint miners were beginning to perform their hoist inspections when the hoist tripped out and stopped approximately 19 ft. below the station on the #1 shaft. After a half an hour of entrapment, MSHA hotline was notified. The miners were brought to surface via the hoist at 9am. No injuries reported.

At approximately 12:30pm on 10.25.21 a washout was reported at the TR underground crusher pad (located on the surface north of the TR Underground admin building). It is believed that this washout of the crusher pad was caused by the excessive rains received in the region. Operations is developing a repair plan that be conducted once the area has dried out.

NGM Loader 3935 was tramming a Z50 truck battery from the mine to the 17 shop. While on the MAD decline, the battery caught fire and visible flames were observed. The loader operator was able to set the battery down in the drift and back away. It is unknown, at this time, whether or not the battery pack is still on fire.

While performing a stack test, Business Partner employees raised the Genie man-lift to approximately 60' and began to have control problem. Inside the bucket, they tried to get the controls to work, unable to do so, the supervisor tried to get them down from the ground controls. They were unable to get the bucket lowered, NGM employees used a crane bucket to get them down safely.

Pre-check of manlift performed before operation; functioned normally. After raising basket to operating height, basket controls and ground controls stopped functioning. Mechanic tried to repair but that didn't work so employees were rescued from manlift using a crane. No injuries, illnesses or damage occurred.

In muck bay #2 located in 4041 Barrel, shotcrete support came off the right rib while mucking the face. The measurement estimated to be 47' in length. The back & left rib were still supported. No injury occurred nor damage to equipment. Due to shotcrete separation, based on some corroded bolts & air slack, the muck bay couldn't be used till a plan was developed to rehab the area

Hoist tripped out on ""under voltage"" alarm and would not reset. Hoist down for 5 hours, while troubleshooting. Broken wire on PLC due to over torque on a termination screw. No miners were on the cage at the time.

Hoisted personnel on Cage #2 at Shaft #1 and struck closed vent doors causing over torque trip of conveyance. Hoist reset and lowered personnel to 1550 station to offload. Lowered conveyance #1 to transport personnel to Shaft #1 collar. Conveyance #1 tripped 175' up from 1550. After 36 minutes troubleshoot, miners were lowered to 1550 and vacated via shaft #2

Operator was moving empty emulsion totes from the UG powder mag to the surface, operator stated they smelled a burning odor from the loader. Operator asked two other employees if they smelled it. They stated yes and inspected under the hood compartment, did not observe any visible signs of a fire. When the two employees went to leave they saw sparks/flames under the loader.

A breaker in a load center failed as designed which caused some smoke to be released. There was no indication of fire at any time and Leeville and Pete Bajo mines were evacuated as a precaution.

Hoist was at a stop when Hoistman heard clicking noises in control room. Hoistman attempted to move the hoist with no response. The hoist began to alarm as several resets were engaged. Electricians tried to troubleshoot. Hoist consultants were able to find and correct problem. At no time was the conveyance occupied.

While preparing to replace patch on acid storage tank, a rapid energy release out of the vent line caused the line to burst, causing unexpected release of acid to spray through the deaerator on top of the tank. During the action of grinding the sparks were going towards the overflow pipe which caused an ignition due to the high amounts of hydrogen.

On 5/22/21 the 4703 hoist cage came in contact with material that had built up on shaft bottom. Cage was inspected and released back to full service after approximately after 2.5 hours of downtime for rope and cage inspection. After inspection, cage was released back to full service and deemed to not have any out of service defects.

An N.A. Degerstrom lube truck had a failure of unknow origins, which started a fire near the rear of the truck. Truck became engulfed in flames and totally destroyed.

A jammer backed over some boxes and signage in a muck bay to pull out of the way a haul truck. It is believed that heat from the engine compartment ignited the boxes and or signage, resulting in a small fire.

A 6' round was shot the morning of 11APR2021. Loader Operator and Truck Driver proceeded into the MXD 2823 to conduct a heading inspection with no issues indicated. The mucker operator loaded the haul truck and returned to see that the right rib and back started sluffing. Barricaded the area, called Supervisor and returned to discover the ground fell beyond primary anchorage.

High winds at site caused a main NV Energy line on-site to arc out at the insulator phase drop; Due to power outage miners hoisting efforts were limited; miners affected were able to mobilize to fresh air, until power was restored to hoist personnel out of mine.

2 business partner employees drilling 20' holes for cable bolts in the back. Once completed on the right side of the drift, it was planned to move to drill on the left side. Bolter operator saw rock raining down & told helper to move to the right of the Bolter. Operator exited cab they heard popping & ran 20' back as the fall of ground occurred. Equipment was partially covered.

A power loss was experienced at approximately 0251 am. At 0538 am, after a let up in a snow storm the source of the power loss was discovered. A power pole was found on fire. Mine rescue was dispatched to assess and extinguish fire. Fire was safely extinguished at 0715 am.

The onboard Air Compressor failed on Blt 7, causing heavy smoke and the fire suppression to be discharged.

A ground fall occurred post blast at Vista Underground-Turquoise Ridge at the 1N3960 level. No ventilation or access was restricted. No injuries sustained.

EE was pulling pipe at 4050 LVL N underground. EE removed the clamp from the pipe EE was removing, and the pipe next to it fell. EE's left pinky finger caught between the handrail and the pipe. Seen at ER, diagnosed with a fracture to the left pinky, released to full duty.

Caught in, under or between a moving and a stationary object

Ee was adjusting forks on a Kubota forklift. Ee's left middle and 4th finger were pinched between the left fork and the frame. Ee was seen at the ER and the left middle finger was amputated at the first knuckle. EE was released to modified duty effective 5/4.

Fall from scaffolds, walkways, platforms

Crew was pulling a conveyor belt off of a system in the crusher area. The belt was being pulled off adjacent to a portable catwalk. The belt made contact with the stairway/handrail on the left side of the catwalk. The catwalk started to tip over. A worker who was standing on the catwalk monitoring the belt jumped off the catwalk. The worker fell to the ground and landed in mud.

Ee reports was getting back on haul truck using the ladder and felt a sharp pain in right low back and hip when pulling self up. After driving for a short period of time, pain worsened to the point where ee could not concentrate secondary to pain. Saw doctor on 11/09, dx with lumbar strain, put off work due to pain complaints/inability to bear weight.

Fall to the walkway or working surface

Ee finished cleaning walkway, rolled up hose. Walked across walkway to get other hose to continue cleanup, slipped in mud on the side of the walkway, both feet got stuck, ee fell to ground hitting head and shoulder. Seen at ER, laceration to forehead closed with stitches, ee taken to surgery to repair fractured shoulder. Put off work until 10/15 follow-up.

Struck by flying object

Ee was changing a tire on an UG haul truck. The section repair that was installed blew out, causing ee concussion injury to head, ears, torso. Ee was seen at ER, CT negative for acute injury, ee released to full duty. Ee complained of ongoing symptoms that were determined occupational on 10/4/21, and put off work, prompting this report.

Struck by falling object

Ee was attempting to move a table by pulling on a vise. The vise was not mounted and was pulled off the table, landing on the ee's left foot. Seen at a med. center, xrays negative for fracture, ee put off work and referred for consultation with a foot surgeon.

Over-exertion (Not Elsewhere Classified)

Ee reports popping and instability in knee dismounting last rung of ladder on equipment. MRI results reviewed on 9/15 show torn meniscus. After medical investigation accepted by workers' comp on 9/16, making this reportable. Ee restricted duty.

Absorption of radiations, caustics, toxic and noxious substances

Ee was opening valve. EE stepped down onto hose causing the hose to unhook from the valve, and lime sprayed out, hitting EE in eyes. EE was transported to NNRH then to SLC for bilateral alkali burns to both eyes. Followed up with eye doctor on 7/19 and 7/21 and was put off work through 7/26 follow-up.

EE was walking at the drill site and EE's knee buckled.

Caught in, under or between a moving and a stationary object

EE had left arm out of door opening between door and door frame of lube truck, holding a wooden block, attempting to use the block and the articulation of the machine to force the door back into position. The lube truck articulated unexpectedly while arm was outside the cab, pinning the arm between the door and frame. Open fracture needed surgery.

Ee reported being jostled in cab of haul truck when rock hit the headache rack. Seen at clinic and diagnosed with thoracic and lumbar strain. On 2/12 investigation complete and claim accepted as occupational making this reportable. Ee released to restricted duty.

Struck by falling object

EE was lowering gate, it got stuck on the rib, and glove caught on a wire. While pulling out the glove, the gate fell, striking the EE on right collarbone. EE was diagnosed with a fractured right collarbone and will be having surgery to repair on 1/22.

Ee was bolting when a sensor on the boom stopped working. Ee moved the boom to a safe location and went to check the sensor. A piece of CRF fell from the back approximately 16 feet, knocking the ee to the ground. Ee contacted help, was seen at a hospital, diagnosed with 3 fractures in L1, L2, L3. Released with restrictions.

Technicians inserting 16"" pipe into 30"" pipe sleeve, when the open end of the pipe caught on the sleeve. Ee grabbed open end of 16"" pipe and pulled upwards to free it. Pipe moved upwards, pinching the ee's right hand between wall of 16"" pipe and the 30"" sleeve. Avulsion to right index finger. Ee sent for surgical consult after ER visit.

Driver walking around the back of truck, slipped and fell on ice/snow, right leg bent back. Current medical consult scheduled for 12/28/21

Over-exertion in pulling or pushing objects

EE was reinstalling a clamp on hose and used a pry bar to close the clamp. EE pulled on the pry bar with both hands and felt a sharp pain in EE's lower right side abdomen. Seen at Golden Valley, diagnosed with a hernia. EE is being scheduled for surgical repair of the hernia.

Struck against a moving object

Ee was operating a 777 haul truck, getting loaded by a loader. Ee reported the loader operator dropped boom of loader on side of truck, more than once, causing the truck to come off the ground, jostling EE in the cab. Reported to supervisor on date of incident. Seen by doctor on 10/29, put off work pending follow-up.

Fall to the walkway or working surface

Ee dismounting shovel using stairs. Wearing large overboots due to muddy conditions, tripped over large rocks in shovel track grooves, landed on left knee. Follow-up with orthopedist on 1/25/22 resulted in referral to physical therapy making this reportable for treatment. Ee working full duty since date of injury.

Over-exertion in lifting objects

Ee reports moving a loader tire and felt pain in their shoulder. Seen at Golden Valley, diagnosed with strained right shoulder, released with restrictions site can accommodate.

Struck by falling object

Ee was scaling off a hanger plate. A loose piece of shotcrete fell and hit the ee in the left eye and on the nose. Ee was seen at the ER, diagnosed with a corneal abrasion, blunt eye trauma, and iridoplegia. Ee was referred to an eye doctor and released to restricted duty on ee's next scheduled shift.

Over-exertion in lifting objects

EE was using torch to cut metal. EE ran out of oxygen and when replacing the bottle, they loaded the bottle into the back of the pickup and heard a pop in the left bicep. EE lost all strength in the arm for a few seconds. Seen at Golden Valley 9/2, diagnosed with left biceps tendon rupture. Referred to ortho consult. LTI

Ee was working on a loader. The hood cover fell and slammed on Ee's thumb. Ee was seen at a clinic diagnosed with an avulsion fracture, and released to full duty.

Employee tripped on rock in pit and Fractured right patella (Kneecap).

Over-exertion (Not Elsewhere Classified)

EE was climbing a ladder to reach some steel balls in the sage mill hopper. On 3rd trip down the ladder, they stumbled over a rubber mat that was left on the ground at the bottom of the ladder. EE twisted right knee. Seen at clinic, diagnosed with a strain, subsequently put off work effective 8/27/21.

Struck by falling object

Ee was lowering a turnstile head when it slipped and slid down EE's arm, causing a laceration. Seen at NNRH and referred to Renown ER. Renown ER referred for ortho consult. Ee released to restricted duty which site could accommodate effective 6/4/21.

Ee was drilling underground. Ee set up the jumbo to start drilling, and went to put the collar pipe when rocks came down the face and hit ee's left leg, specifically the left knee and calf. Seen at a medical clinic and sent to ER for evaluation and observation. Put off work pending follow-up.

Ee was exiting the compartment beind the cab on loader and slipped on the driveline cover. Ee felt right knee pain. Ee caught self from falling and strained left arm/shoulder. MRI done on 5/12 positive for biceps tendon rupture. Referred for surgery.

Over-exertion in lifting objects

EE lifted a bucket of bolts that weighed 138 pounds. When EE turned, EE felt a sudden pain in right side of groin. Seen at Golden Health, diagnosed with a hernia, referred for surgical consult. Site accommodating restrictions pending surgical consult.

Caught in, under or between a moving and a stationary object

EE was working with a partner to move superswellex bolts off a flat bed. They moved one, and three others moved with it, pinching the EE's finger. EE was seen at the ER, diagnosed with a fracture, and referred to ortho. EE is scheduled for surgery on 2/23.

Fall to the walkway or working surface

Ee was pulling cable with a hot stick when the handle broke and the ee fell backwards, landing on the ground. EE was seen at the clinic, x-rays negative for fracture, ee released to restricted duty and referred to an ortho.

Over-exertion in lifting objects

Ee was loading tools from cart to service truck. EE had previously moved two 8-D cat batteries as well. While lifting a tote bag of tools EE felt a sharp pain in EE's left bicep. Seen at ER and referred to orthopedist. EE is released to restricted duty the site can accommodate.

Fall from ladders

EE was standing on 5th rung of ladder replacing a hoist cylinder. EE was moving the cylinder to go between the bones of the truck when the cylinder tilted out toward the ladder, hitting the ladder and causing the EE to fall to the ground. EE seen at medical, diagnosed with a fracture to T3 and referred to ortho. EE released to light duty by ortho.

Ee was sampling leak ports. EE has EE's foot on a berm and EE slipped, twisted EE's body, and felt a pop in EE's right leg/knee. Reported and continued working. EE requested to be seen by a doctor the next day, seen at Golden Health, diagnosed with a lower extremity strain, and released to restricted duty.

Ee lifting track pin to wedge in place to keep track from rotating while setting pressures on pump. Pin started to fall, ee let go and smashed tip of right pinky finger between the pin and track. Seen at hospital, diagnosed with right distal tuft fracture. Splinted, released to restricted duty.

Ee was reaching to put something away on bus when bus started to move. Ee started to fall and EE caught self from falling, straining their shoulder. Seen at Golden Health, diagnosed with a shoulder strain, and released with restrictions that site could accommodate.

Fall onto or against objects

Employee was changing out steel on a bolter when they lost footing and fell back. Employee denied medical attention initially. November they requested to see a doctor, employee was taken to an urgent care and referred to a specialist. Employee was put on RD by the specialist pending surgery.

Struck against a moving object

EE backing up haul truck, shovel bucket struck bed, stopping truck suddenly and jostling the EE in the cab. The EE was seen at health clinic, diagnosed with sprain, and released to full duty. On follow-up on 7/29/21, EE prescribed physical therapy and released with restrictions, making this reportable.

Over-exertion (Not Elsewhere Classified)

Ee reported heat exhaustion, reports descending equipment. EE reports right ankle pain, but does not remember injuring ankle. Claim investigated by workers' comp and determined work related 7/26. Review of ee's schedule on 9/9/21 discovered one restricted duty day worked prompting this report. Original review of time did not show any restricted duty days.

Over-exertion in lifting objects

Ee was lifting a lid to a dumpster while trying to throw a full bag of garbage into the dumpster. Ee felt pop in right shoulder. Did not feel pain until that night at midnight. Reported to supervisor, seen at Golden Health, diagnosed with a sprain, released to light duty that site can accommodate.

Struck against stationary object

Ee was pulling a metal hose. The hose caught on something and the ee's hand slid along the hose and left pointer finger was cut. Seen at a health clinic, sutures required to close the wound and ee released to restricted duty the site could accommodate.

Fall onto or against objects

EE stepped onto step of dozer, slipped, and fell, striking chest on a rail and right shin on a step. EE was seen at Golden Health, diagnosed with a contusion to the right rib and abrasion to the right leg. Released to restricted duty the site can accommodate.

Struck by falling object

Ee was removing a belly pan on haul truck 203. The pan fell onto the ee's right hand causing a laceration at the base of the index finger. Stitches were required to close the wound and the employee was released with restrictions the site can accommodate.

Ee kneeled down to work on a haul truck and kneeled on a sharp rock, causing EE to stand up quickly. EE felt a popping and pain in the right knee. EE was seen at clinic, diagnosed with a strain, and released with restrictions. After investigation complete, the claim was accepted as occupational by WC on 2/26.

Ee was working with another ee to install a transmission guard. Guard caught on grease lines; ee reached in with left hand to clear grease lines out of the way and the guard slid forward, pinching the ee's left thumb between the guard and steel plate. Seen at ER, referred to ortho. Diagnosed with a fracture and returned to light duty that site can accommodate.

Struck by flying object

Ee was using a chisel to remove a broken bolt from the base of a pump when a piece of bolt broke off and went into EE's left eye. Seen onsite by emergency response and requested treatment on 12/30. Diagnosed with a corneal abrasion and referred to optometrist. Released to full duty.

Caught in, under or between a moving and a stationary object

Ee was removing the 24"" shoe from the Tracstar HDPE fusion machine. The shoe was frozen, when the ee tugged on it, it came loose and dropped back down, pinching the ee's left middle finger between the shoe and the shoe frame. Ee was wearing hand protection. Seen at clinic, stitch required to close wound. Released to full duty.

Ee washing equipment with water hose equipped with ball valve. Ee finished task, closed valve, put down the hose in a manner that caused the valve to open. There was enough pressure to cause the hose to jump, striking the ee in the mouth and breaking a tooth. Seen at dentist, tooth is being extracted. Released to full duty.

Descending from CAT 16-H Blade while on the 2nd step with right foot, they attempted to step onto the mullboard, missed with left foot. EE attempted to lower themselves the rest of the way to the ground. Employee has both hips replaced, the stress on right leg caused a spiral fracture of the femur starting just below the shaft of the replacement hip on the right side.

Employee was in a man basket and put hand on the rail. When the basket was raised EE's finger got pinched between the man basket and the fork carriage assembly.

Struck against stationary object

Employee was working on a damaged hose clamp. The tap broke causing employee to lose footing and employee struck the broken tap tool with hand causing a laceration.

Absorption of radiations, caustics, toxic and noxious substances

Ee at lime silo; cleaned mirror off and then rubbed eye with hand. Noticed a burning sensation, seen at ER at NNRH and UofU. Diagnosed with conjunctivitis of left eye, released to full duty. Reportable for prescription meds for eye injury.

Struck by falling object

EE was loading rounds at the face. A small piece of debris came off the face, striking EE's left wrist. EE reported and did not seek treatment. When the pain did not subside, EE saw a doctor on 11/30/21 and was diagnosed with a chip fracture and released with restrictions that do not preclude EE from performing all the essential functions of the role.

Ee was using an impact wrench to remove bolts. A nut on one of the bolts turned and cut the ee's left middle finger. Seen at Golden Health, stitches required to close the cut, ee released to full duty.

Miner lacerated thumb (L) while attempting to cut away a zip tie with a hook knife that was connected to piezometers resulted in sutures.

Caught in, under or between a moving and a stationary object

Ee reports getting backpack from the back seat of the light vehicle in the parking lot. Ee closed right hand ring finger in the door of the truck causing a small laceration. Seen at clinic, laceration closed with 3 sutures, ee released to full duty.

Ee was moving metal arm for mixer bolts into position to install bolts and spacer blocks. When ee went to move arm into position, ee bumped it and the arm slid, pinching ee's finger between the arm and spacer bar. Laceration was repaired with sutures and ee was released to full duty.

Fall onto or against objects

Employee lost consciousness due to a medical incident not related to work. EE fell forward and struck EE's face on a bolt plate causing a laceration to EE's cheek.

Struck by flying object

EE was using a pressure washer. Enough pressure built up in the wand, causing it to ""jump back"" and strike the EE in the face. EE was seen at the ER where a laceration near the right eye was closed with sutures. EE released to full duty.

Manpower employee used a battery operated circular saw to cut cardboard for sample labeling. EE cut left thumb with saw.

Caught in, under or between a moving and a stationary object

Employee was conducting maintenance on a dozer. While utilizing a prybar to turn the engine, the bar slipped and caught left pinky between the engine and the prybar. The employee removed glove and discovered a small abrasion, employee was examined at the hospital and it was identified that the pinky suffered a fracture on the distal end.

Employee loaded hat channel into bed of truck which stuck out approximately 3'. The employee walked into the channel during EE's lunch break while talking on EE's phone

Struck by flying object

Employee was using a grinder to repair a piece of equipment. Later that evening EE felt something in EE's right eye and was taken to the hospital where they removed a small piece of debris.

EE was walking into control room. As the door was closing, EE's finger got pinched, causing a laceration. EE was seen at the ER and stitches were required to close the wound. EE released to full duty.

Absorption of radiations, caustics, toxic and noxious substances

EE was working in CV003 chute. Lime dust fell and got on EE's neck and back and also in and around eyes. EE washed eyes out and reported to WorkCare. Self-care seemed to be working, but on 2/15 requested an appointment with an eye doctor. Was seen on 2/16 and diagnosed with allergic conjunctivitis. Eye drops prescribed and EE released to full duty effective 2/17.

Struck against stationary object

Ee was working on a drive. Ee stood up, took a step back, and struck head on a bracket. Seen at the ER and laceration was closed with staples. Ee released to full duty 1/19/21.

Contact with heat

Ee and coworker working on face fan on scissor deck. Ee started to get headache, lowered deck down to take break. Ee near back of scissor deck and briefly lost consciousness. Seen by medics, cleared to return to work full duty.

Over-exertion (Not Elsewhere Classified)

EE reports ongoing discomfort in elbow when performing certain movements. On 9/22 they noticed it at work when lifting a heavy trash can. EE sought treatment for the pain when it did not resolve. Diagnosed with occupational illness, epicondylitis.

Employee was found on the floor of the Pipeline Shop restroom unresponsive and bleeding from the back of the head where EE had fallen and hit the floor. EMT's were notified and performed CPR while air ambulance was called. Helicopter arrived at 13:28. Scene secure, cleaned, released back to operations by MSHA at 19:25.

No accident occurred on the mine site. EE experienced a cardiac event while on the bus traveling to site. Discovered upon arrival at the administration building, North entrance. EMT's responded, CPR efforts unsuccessful.
