Fall from machine

LUBE MAN WAS GREASING CRUSHER WHEN HE SLIPPED OFF THE DECKING AND FELL OVER THE HANDRAIL LANDING ON HIS BACK APPROXIMATELY 4 FEET BELOW ON THE GROUND

Employee was climbing down off of the platform of a Cat 5130 Excavator attempting to put his foot on the first rung of the ladder when his footing slipped off the rung and the employee fell to the ground landing on his feet.

Over-exertion in lifting objects

Employee was removing some debris from the haulage road and sprained his lower back while lifting. **Date of Acc. 6/8/06, first day missed - 6/22/06 **

Struck by falling object

Employee was working around Auger by highwall when a football sized rock fell from the highwall and struck the Auger, ricochetting off the Auger and striking the employee in the shoulder area.

Fall from machine

Emploee was climbing up ladder on rock truck and missed the hand rail falling approximately 5 feet to the ground landing on his left ankle.

Over-exertion (Not Elsewhere Classified)

Two employees were unloading a pump out of the bed of a utility truck when one employee slipped and the other employee took all of the weight in an awkward position and twisted his back.

Struck against a moving object

Employee was traveling up a grade when he ran out of fuel stalling the engine. Without maxi brakes that lock up when depletion of air the truck could not start, it started to coast backwards and struck a birm and rolled onto its side. Employee was wearing his seat belt.

Struck by flying object

Employee was performing maintenance, prying on a gear in a crusher box when a piece of steel from the toothed gear chipped off and struck him in the mouth area.

Fall onto or against objects

Employee was walking up steps to check the anti-freeze in the equipment when he slipped on a step and rammed his side into the hand rail.

Employee was helping with maintenance in the wash plant and was cutting tabs off with a utility knife when the knife slipped lacerating himself requiring stitches.

Employee was attempting to loosen a bolt holding a fuel filter and the wrench slipped and struck the employee in the facial area causing stitches in the lip and mouth area and chipping a tooth.

Struck against stationary object

EMPLOYEE WAS ATTEMPTING TO PULL A FLEXABLE STEP DOWN THAT WAS BENT UP UNDER THE BUMPER AND WHILE PULLING, THE METAL BASE LACERATED HIS HAND.
