Fall to the walkway or working surface

Employee was attempting to repair a support on a float barge when EE's foot slipped and EE fell over onto hip causing a bruised hipbone.

Struck by flying object

1/8"" shard of steel, from mine owned drill, hit miner in left pectoral.

Fall from scaffolds, walkways, platforms

Employee was standing on front platform helping a mechanic bolt a light bracket on the lift cylinder of a D11 bulldozer after they had replaced Hyd. hoses. The employee lost balance and handhold causing EE to fall. Employee received a cut on the head that required stitches.

Fall onto or against objects

Employee was getting a fuel hose to fuel a loader and slipped on ice and snow pack, employee stuck hand out to catch self. Employee's hand landed up against the ladder bending index finger back and fracturing it.
