Employee was climbing up on his rock truck and slipped and fell.

Employee was walking across the parking lot and slipped on ice spraining his ankle.

Employee was back dragging a drill bench when he backed over an embankment and roll his dozer over onto its side.

Employee was under a bull dozer changing a hydraulic hose. He was trying to bend it up in place and it slipped out of his hand and hit him in the forehead causing a cut - 4 stitches.

Employee was using a sledge to knock a spacer out and the spacer flew and hit him in the cheek. 4 Stitches required

Employee was winding up a grease hose and the trigger caught on the grease rack and shot grease into his eye. No damage to his eye but doctor prescribed an eye drop for a safety precaution.
