Accident type, without injuries

Employee was out on deck of a D11T Dozer when he noticed smoke coming out of the front of the machine. He turned to get fire ext. and when he turned back the fire was coming out of engine compartment. Employee shut it off and got off the machine.

Fall to the walkway or working surface

EE was using a socket to tighten a lid on a Hyd. filter lid and the wrench slipped causing the EE to lose his balance and fall.

Struck by... (Not Elsewhere Classified)

EE was using a porta power to remove a pin and was using his leg to steady it to get started and a metal splinter came off the pump or all thread and went in his skin of leg.
