Accident type, without injuries

Employee was loading rock trucks and as far as we can tell a Hyd. Line broke and shot oil over onto the turbo. A fire started which could not be put out in the 30 minute span.

Fall from machine

Employee was changing oil filters on his loader and slipped and fell injuring his arm. He fell 38 inches to the ground.

Struck by falling object

Employee was attempting to put a belly pan on a dozer and a weld broke letting the pan to drop hitting him in the arm and hand.

Fall from machine

EE was in a hurry to get fire extinguisher from in back of his cab and slipped and fell from the cat walk. EE landed on his foot breaking bones in his ankle.

Over-exertion in lifting objects

Employee picked up a drive shaft out of a D-9, he lifted it up about 2 feet and said he hurt his neck and shoulder.

Struck against a moving object

Employee was driving up out of pit and the sun blinded him and he run his water pumping truck up a bank and laid it over on its side. He worked the rest of shift and the next day and then took 2 days off.

Employee was climbing up ladder of Rock truck, he had mud on his feet and slipped and twisted his back.

Employee slipped on ice in the parking lot and fell.

Employee was climbing down the ladder of a 988F loader and slipped on the last step. Employee did not fall but twisted his shoulder when he slipped.

Employee was starting up stairs into parts trailer and grabbed the hand rail. He felt something pop in his shoulder.

Fall from machine

Employee was stepping off a D-6 push arm and slipped on ice causing him to fall about 2 feet to the ground. Employee suffered a hairline fracture in his wrist.

Over-exertion in lifting objects

Employee was shoveling stemming, carrying tubes and climbing in and out of truck. No specific act, just started to hurt. Continued working until he saw a Dr on 2/4/2013 at which time he was informed he had a hernia and was given surgery to correct it.

Caught in, under or between a moving and a stationary object

Employee was getting into his dozer and slipped causing the door to slam shut on the very tip of his middle finger breaking it.

Struck against stationary object

Injured employee was attaching a tow cable to his vehicle and in the process slid his hand down cable receiving a cut which required 6 stitches to close.

Struck by flying object

Employee was driving a pin into the track segment and a piece of metal came off and hit another employees arm causing a cut that required 5 stitches.

Caught in, under or between a moving and a stationary object

Employee was climbing back into the pickup truck after checking water pump and slipped getting in. When he slipped he shut the door on the tip of his middle finger and broke it. It was raining and muddy.

Employee was climbing into a 16G Cat grader when the wind blew the door around and hit him in the side of his head causing a cut requiring 4 staples.
