Struck against stationary object

was washing out some rock under cone crusher. stepped on a larger rock which caused my foot to twist and snap

Fall from machine

Was getting off dragline at the end of the day, stepped in some grease on the track and slipped and fell on the ground. Fell on the ground but my head hit on the left side.

Got out of vehicle, stepped on rock, twisting ankle and fell on right shoulder

EE WAS OPERATING EQUIPMENT AND CLAIMS TO NOT HAVE SEEN LARGE HOLE IN FRONT. WHEN EE HIT THE EDGE OF THE HOLE, LOADER PITCHED FORWARD AND EE HIT NOSE ON STEERING WHEEL. NOSE NOT BROKEN, CHECKED OUT BY DOCTOR AND GIVEN MEDICATIONS, RELEASED BACK TO WORK
