Struck by flying object

Employee states: On Friday July 7th approx 8pm while working on a broken cable I was swinging a sledge hammer to knock out the wedge to remove the cable and I felt something explode in my right arm. I thought I pulled a muscle so I continued to work. Then 1 hour later I took off my rain jacket and then I seen all the blood and the hole through my jacket and both t-shirts.

The miner fell getting off the dragline. He tried to push himself away from the tracks and got his right hand caught in hand rail.

Fall down stairs

I was walking on the clay belt catwalk, when I walked up to the clay deck. I had my hand on the rail. I slipped on the step and my armpit got caught on the hand rail and twisted my shoulder & back when I fell.

Absorption of radiations, caustics, toxic and noxious substances

Employee was standing behind the feed belt to the stacker, washing off the concrete. He went to climb up the belt when his arm starting burning. He snatched his arm back to see a snake fall to his feet and run away. Was taken to the ER, treated and released.
