Struck by... (Not Elsewhere Classified)

Employee was swinging sledge hammer, missed and hit finger. Finger required stitches and end of finger fractured (3rd finger left hand).

Fall onto or against objects

EE was inside the cage for cone crusher. The floor is wet and full of rock and mud. He slipped and fell against oil tank and hit neck and left shoulder. I finished the day but on Saturday, I could not move my head. On Monday, I was in pain and went to the emergency room.

Fall to the walkway or working surface

I was in the process of routine maintenence. I was began greasing the bearings on the conveyor tail pulley's when I slipped in a puddle of mud. I lost my balance and landed on my right foot. I heard a pop sound and a sharp pain shot thru my ankle. As I tried to stand up the pain became unbearable. I fell back to the ground and asked for help. 2 men came & put me into control room.

Struck against stationary object

Employee was on catwalk and hit his left knee. Knee became swollen and hurting.

Employee was swingin a sledge hammer at the primary plant. He started getting light-headed & pains in his chest. He went over to the belt, sat down, and then passed out. One of other employees went to check on him and called 911.
