Caught in, under or between a moving and a stationary object

EMPLOYEES WERE CHANGING BOLTS ON THE ""DUTCHMAN"" ON AN ELECTRIC SHOVEL. THE DOOR OF THE DIPPER WAS BLOCKED OPEN WITH A METAL BAR THAT WAS WELDED IN PLACE. FOR REASONS UNDER INVESTIGATION, THE BLOCKING BECAME DISPLACED AND THE DOOR CLOSED, PINNING TWO EMPLOYEES BETWEEN THE DOOR AND THE DIPPER.

Struck by... (Not Elsewhere Classified)

While rotating the sag emill with a 1 1/2 inch chocker loop over a 1 1/2 inch shall liner bolt, the bolt broke releasing tension on the cable. the cable broke free from the hoist hook destroyed the safety catchs and became air borne. this victim was passing by the work area and was struck by the falling cable.

Struck by falling object

Employee was changing GTE on rubber tired dozer, EE failed to follow procedures and failed to secure it before loosening. GTE fell and struck ee on left leg and ankle.

Over-exertion (Not Elsewhere Classified)

Hole for drill steel in deck plate of new drill was not equipped with cover for use with mast down. Ee was examining drill steel/bit when he stepped into hole. Ee suffered a twisted ankle and fractured fibula.

Struck against stationary object

EE WAS SITTING IN THE CAB OF HAULAGE TRUCK #241. SHE LEANED DOWN TO RETRIEVE HER LUNCH PAIL AND STRUCK HER HEAD ON THE DISPATCH MODULE WHEN SHE STRAIGHTENED UP.

Over-exertion (Not Elsewhere Classified)

Ee was cutting a plate with a track torch when the plate fell and the ee twisted the ankle while getting out of the way.

Flash burns (electric)

WHILE TROUBLE SHOOTING A PUMP CONTROLLER, INDIVIDUAL GOT TOO CLOSE TO A LIVE 4160 LINE & ARCING OCCURRED, APPROX.4-6"". INJURY RESULTED IN 2ND DEGREE BURN. EE WAS RELEASED TO DUTY BUT WITH SOME RESTRICTIONS, NO EXCESSIVE USE OF THE LT. HAND.

Over-exertion in lifting objects

EMPLOYEE WAS SHOVELING BY HAND AND BACK STARTED TO HURT. PAIN MOVED UP BACK INTO HIS NECK. HE WAS LAID OFF 12-20-04. WENT TO DOCTOR ON 12-21-04 AND NOTIFIED ME ON 12-21-04.

Fall from machine

4100 crane disassembled...Rt pendant lines removed from spreader bar...removing last Lt. line when spreader bar rotated causing emp to lose balance & fall 8"" to ground. employee okay, showed supervisor lt foot & ankle...was able to rotate foot w/out pain. refused medical treatment. Drove to Lander, WY Hospital visit & x-ray diagnosed fractured heel.

Struck against stationary object

EE WAS ATTEMPTING TO LOOSEN A BOLT ON A PUMP FLANGE. THE BOLT LOOSENED SUDDENLY AND THE EE'S HAND SLIPPED, STRIKING THE END OF A 1/4"" BOLT PROTRUDING FROM THE PUMP MOUNTING BRACKET. THE EE STRUCK THE BACK OF HIS RIGHT HAND, BRUISING IT AND CAUSING A SMALL PUNCTURE WOUND.

Struck by falling object

An employee was installing angle iron when it slipped and fell on the right two fingers.
