While trying to secure a 30"" SDR-17 pipe into a fusing machine, the pipe came out and over the ""jaws"" of the machine, striking the worker on the right side.

EE was lifting chip tray, as he lifted the tray he felt his arm pop. The weight of the ship was 10-15 pounds,

After a ""dog"" was welded into place the employee put a wedge in, put his foot on the ""dog"". Hit the wedge in until liner was as far as possible, got down to check the gap, looked good stood up and was turning to tell another employee to go ahead and stitch it in. When the weld broke and the ""dog"" struck the employee in the face.

Employee was getting logded under p&h shovel. Operator of shovel swung around to put a bucket in back of haul truck. Shovel operator over swang and made on tact with truck bed. When contact was made it jolted the driver. Hurting truck drivers lower back and neck.

Employee was getting back on dozer after getting fuel. He slipped & fell twisting right knee. Took employee to Doctor. He was put on crutches & brace. He wore for 1 week. He went back to Dr. after MRI. Some scar tissue in his knee & was up to employee if he wanted it operated on. He chose not to have the operation & go back on crew.

Employee was working in the crusher on a down day. Employee was working on a mount on an air canon. The mount on the air canon broke. Employee grabbed the canon so it would not hit the ground. As he grabbed it, it pinched his hand between the canon and the ground. Per the enclosed letter, ee had surgery 12/22/08 making this reportable.

EE was pushing on a 36"" pipe wrench to tighten a joint of drill pipe.

The Crew were tripping the drill pipe out, employee grabbed the pipe (that was hooked to the winch line) and turned and swung it to the helper on the pipe truck when he states he felt a pop in his left shoulder.

Employee states he was attempting to cut through a rubber sealing ring that is used on the casing advancer, using a pocket knife, when the knife slipped and cut his left index finger. He was not wearing gloves.

Employee was operating a nail gun with one hand and holding a board with the other hand. Nail went through the board and punctured his finger.

Working on P&H Shovel light guards, slipped on track and twisted knee, took EE to doctor. Over time knee started to hurt. April 28th had MRI. A small tear of the miniscus in right knee was found. Knee was operated on teh 16th. The 17th EE started 7 days off. EE is scheduled back to work on night shift on the 23rd.

3 maint. techs were rebuilding floor sump pump. EE tech holding wear plate, 2 EEs thread impeller in place, wear plate slipped pinching EE right index finger in between the wear plate and the pump shaft causing a laceration 6 stitches.
