The truck employee was operating was parked under the shovel, a boulder fell out of the shovel into the bed of the truck. Second incident - shovel dropped boulder on tail end of truck causing the bed of the truck to lift up and bounce back down. Employee's neck was jarred both times.

Employee states while watching drill operations on the deck of the drill, he was standing too close to a portable floor fire heated burner and felt stinging on the back of his legs. Employee states that he realized he was on fire. Employee states that he used his hand to try to put out the fire with no luck and finally the other driller took the water hose and put him out.
Employee was loading wood into bucket. Operator of the loader lowered the bucket down onto the foot of the employee.
While pulverizing samples, lifting the pulverizer lid and knocking off excess sample, ee's neck popped at the base between shoulder.
EE was stepping up onto service truck back bumper when he felt a popping noise... couldn't go up or get down. Lt ankle sprain or torn ligament.
Backing under shovel. Shovel spotted bucket too low. Employee hit dutchman, causing bed to go up and come down hard, jarring her.
Climbing on dozer to get in cab. Got to the top lost balance started to fall backward. He then jumped so not to land on his back. Landed on his feet breaking bone in his left foot.
Climbing up ladder on water tank to see if it was lined up with water island pipe, while coming down ladder he tripped on a hose, twisted ankle and fell landing on exhaust and burned arm.
Shovel bucket on 495BE shovel struck head rack on 793 cat haul truck, jarred truck and injured his back. 5/15/2006 was his Friday; 5/19/06 was his Monday back.
Finished working on haul truck 274, doing paper work in cab of service truck. 274 honked horn pulled foward turned right and ran over service truck.
Hooking up chain binder to haul dozer, he pulled chain to hook up. Chain had twist in it. As he pulled, chain spun binder handle and hit him in the mouth, busting front tooth.
WALKING OUT TO SNOWPLOW AND SLIPPED IN THE SNOW.
WHILE WALKING TO PARKING LOT AT SHIFT CHANGE, SLIPPED ON ICE & HIT HIS HEAD. FOLLOW UP WAS ON 1/06/06, DR. GAVE EMPLOYEE A SHOT IN HIS NECK TO HELP WITH THE PAIN.
Getting off haul truck, walking to van at shift change. Slipped on ice and twisted knee.
Employee was working on water truck rear end in a tight spot. When work was done his leg was stuck. He pulled on his leg to free it. When he pulled it popped. An MRI in July showed a torn meniscus in his right knee. Operation on 7/27 made this a medical reportable, light duty.
Attemting to remove the drain plug on the front wheel of 274 haul truck using a brake bar. When plug came loose, slipped and pinched finger between bar and rim. Cutting little finger on left hand. (6 stiches).
Putting metal shelfs together. The person on the other side was pushing his end in place, shelf slipped and corner was bent down when it slipped it hit his right leg cutting it.
Getting out of van at end of shift. Cut arm on arm rest that was missing. Needing 7 stiches.
EE was on grader hand slipped on icy tire. Caused him to loose his balance. Slid down the tire like a slide landed on his feet on the ground and then he tumbled over.
Trying to get a bearing retainer out tapping on it with a hammer (3Pound) a sliver of metal shot off the retainer and went into his hand between the finger and thumb on top side of hand.
Left middle finger was pinched on an hydraulic out-rigger. Tip of finger was cut and fractured. Stitches were required.
