Over-exertion (Not Elsewhere Classified)

Employee pulled hamstring while walking around truck during pre-op resulting in one day of lost time.

Fall to the walkway or working surface

Employee slipped and fell in mud putting wheel chocks down, felt left knee pop

Fall onto or against objects

Tire guy climbed into wheel, slipped and smacked knee on the wheel. Initially diagnosed as a knee contusion but MRI revealed a meniscus tear.

Over-exertion in pulling or pushing objects

Employee was turning a valve and shoulder popped. Employee treated conservatively with physical therapy for 6 months at full duty/no restrictions but then decided employee wanted to opt for surgery for possible better results.

Employee was turning steering wheel of haul truck and aggravated a pre-existing shoulder injury.

Fall to the walkway or working surface

While loosening a bolt on a drill, the wrench slipped causing EE to fall backwards. EE tried to catch self and landed on left wrist.

Struck against a moving object

Employee was operating CAT 793C haul truck and while backing under a Letourneau 1850 loader, experienced rough loading event. Employee was taken to ER and sustained a muscle strain.

While walking across tie line, employee slipped and fell in a tire track that had been packed down with snow/ice. Employee fell on right side (hip, knee, shoulder) of body and sprained knee.

Fall from machine

Employee jumped off the ladder of the truck and rolled ankle and then climbed into the cab to look at the ankle and slammed toe in the door also.

Struck against a moving object

Employee was a passenger in a man van that hit a pot hole causing employee to strain their neck.

Employee was operating shovel and digging in a hard spot and the shovel started jumping around causing employee's back to hurt.

Large rock loaded into haul truck that caused truck to rock side to side, up and down causing a back strain and one day of light duty.

While putting a belt on a pump sheave, middle finger got caught between the belt and sheave.

Removing high tension spring, pulled hook through finger; required a couple stitches. Back to work same day.

Employee was pulling steel from the feeder and lost footing, causing employee to fall into a wall and employee hit shoulder.

While traveling on haul road, haul truck operator hit a pothole jarring lower back. Initially a report only, the employee reported on 5-23-19 and did not require medical care. Employee went on days off and when returned to work on 5/28/19, went to ER for strained muscle. Began 3 days off on 5/24/19, followed up with dr, restricted duty.

climbing down ladder from the engine compartment and the handrail broke, tech fell onto the shop floor

While operator of HTK280 was being loaded at SHV005, the operator felt the haul truck go up and down and felt pain in the lower back.

Employee was removing plug from haul truck when the wrench slipped and lacerated employee's finger resulting in 4 sutures.

Employee was driving pin on a track hoe and a metal chip went through glove and lacerated finger requiring 2 sutures.

Caught in, under or between a moving and a stationary object

While lowering a 336F final drive to the ground with a crane the technician reached out to grab it and it tipped pinching the tip/pad of the left ring finger off.

Fall to the walkway or working surface

Slipped on ice and fell on left hand/wrist in February 2019 while on tie down. At follow up appointment on 4/25/19 a fracture was discovered on MRI.

Employee had slipped on some ice knocking off the safety glasses. Employee picked up safety glasses and didn't clean them off. Debris got in the right eye causing irritation 5 hours later employee couldn't see out of the right eye.

Inhalation of radiations, caustics, toxic and noxious substances

This report is being filed to terminate citation #9438261. Employee claims exposure to mixed chemicals caused an illness/reaction that caused a lung issue that was treated by medication.

Bodily reaction, (Not Elsewhere Classified)

Employee was simply walking on even ground and dislocated their knee.
