Over-exertion (Not Elsewhere Classified)

Employee alleges that he injured himself while lifting/twisting machine parts. Employee alleges conditions had worsened over a short period of time. The employee stated that there was no specific incident that precipatated the injury.

Struck by flying object

EMPLOYEE WAS CHANGING A BULB OUT ON A DROP LIGHT. HE INSTALLED A 12V BULB INSTEAD OF THE 110V AND THE BULB EXPLODED CAUSING DEBRIES INTO HIS RIGHT EYE

Fall from ladders

EE WAS TYING LADDER TO THE HEADER AT TAILINGS. THE LADDER SLIPPED AND EE FELL WITH THE LADDER.

Over-exertion in lifting objects

WHILE MOVING SOME EMPTY OXYGEN BOTTLES, EE EXPERIENCED PAIN IN THE GROIN AREA. HE WAS DIAGNOSED WITH A HERNIA BUT CONTINUED HIS FULL JOB DUTIES PRIOR TO GOING IN FOR SURGERY ON 5/13/05.

Fall from ladders

EMPLOYEE WAS USING A CHIPPING GUN TO CLEAR ROCK FROM OVER HEAD IN THE SURGE BIN AT THE CRUSHER. MATERIAL FELL ONTO THE EE & HE FELL FROM THE LADDER TWISTING HIS KNEE.

Caught in, under or between a moving and a stationary object

Worker was removing pipe and valve assembly using small hoist. Assembly wasn't secured and was able to flip upside down. Worker's left ring finger was smashed between valve and ground.

Struck by falling object

While mechanics were changing the ripper shank, it got stuck. The operator grabbed the ripper trying to free it. When it came loose, there wasn't enough tension of the lifting line and it fell onto the operator's hand lacerating his finger.

Struck against stationary object

WHILE CLEANING A SLURRY HOSE, IT WAS INADVERTENTLY COUPLED TO AN AIR LINE INSTEAD OF A WATER LINE. WHEN THE VALVE WAS OPENED, THE EE WAS LIFTED OFF OF THE FLOOR & PUSHED AGAINST A WALL. LIGHT DUTY ADVISED BY DR. ON 11/17/05.

OPERATOR WAS ENTERING DRILL CAB WHEN DOOR CAME CLOSED ON LEFT INDEX FINGER

EE STEPPED OFF LADDER OF 793 HAUL TRUCK INTO A HOLE AND STRAINED HIS KNEE. UNEVEN GROUND

Employee moving cable for BE drill, dropping cable trees, right foot was in coiled up ropes, rope was being pulled by truck and took employee with it. Broke lower right leg-cast & crutches for approx. 6 weeks on light duty.

Dozer operator was standing on the ground waiting to be picked up at the end of shift. He doesn't know if he stepped on a rock or in a hole and twisted his ankle.

WHILE USING A CHAIN WRENCH, IT BROKE & EMPLOYEE FELL ONTO HIS RT. SHOULDER.

Struck by falling object

A NEW STEEL BOLT ATTACHING A SLING TO THE CONVERTOR HOUSING BROKE, THE CONVERTOR DROPPED SLIGHTLY AND PINCHED RIGHT KNEE. THE SLING RECOILED FROM RELEASE OF TENSION AND STRUCK LEFT FOREARM

Employee was dismounting a haulage truck on an inclined ladder. He caught his heel on a step and turned his foot under, falling on it.

EE pinched finger between dumpster and forklift waile attempting to open lid on dumpster.

A TRAILING POWER CABLE USED FOR ELECTRICAL POWERED EQUIPMENT WAS BEING PULLED BEHIND A LIGHT VEHICLE. THE CABLE SNAGGED MOMENTARILY WHILE THE EMPLOYEE WAS OBSERVING OPERATION OF THE PICK-UP. THE CABLE CAME FREE & STRUCK THE EE IN BACK OF HIS LEFT KNEE.

Struck against a moving object

EE was driving a mine haulage truck. He was loading at a shovel. The shovel operator struck the edge of the truck bed while swinging to load the truck. The ee complained some time after of a soreness int he area of his shoulders.

EMPLOYEE WAS CLEANING SNOW & DEBRIS FROM THE BOX OFF OF A SERVICE TRUCK. GROUND CONDITIONS WERE SPOTTY & SNOW PACKED.

While walking to a service truck, the employee slipped on some ice and twisted his knee.

While retracting the stabilizer arm on a light plant, it slid quickly pinching the employees finger.

WHILE FUELING EQUIPMENT, OPERATOR VIOLATED POLICY & REMAINED ON THE DECK WHERE DIESEL SPRAYED INTO HIS EYES WHILE FUELING.

WHILE USING A HANDHELD ELECTRIC DRILL THE BIT LOCKED UP IN THE MATERIAL AND THE DRILL JERKED IN THE OPERATORS HAND CAUSING A FRACTURE OF ONE OF THE BONES

Employee tripped on sample catcher. Stepped backwards and left leg went thru hole in deck hitting bit cutting left leg below the knee. (Fourteen stiches).

EMPLOYEE OPENED THE DOOR ON THE FILTER VESSEL BEFORE ALL OF THE PRESSURE WAS RELEASED. THE DOOR OPENED, STRUCK THE VESSEL HOUSING AND RECOILED, STRIKING THE EMPLOYEE'S ARM.

EMPLOYEE WAS DRIVING KEEPER PIN FROM A TOOTH ON 5500 SHOVEL. WHEN HE STRUCK DRIVING PIN W/HAMMER, PIN JUMPED BACK AND STRUCK HIM IN LEG. HE WAS NOT FOLLOWING PROPER PROCEDURES. HE RECIEVED THREE STITCHES

WHILE INSTALLING AN IDLER THE ROLLER WAS STRUCK BY A HAMMER DISLODGING SOME LIME DUST

Fall from ladders

Employee tripped on a lanyard and fell approx. 5' off of a ladder. There was no lost time or restricted duty at this time. Later problems were diagnosed as acute carpal tunnel injury which required surgery.

Light bulb in uncovered dome light exploaded. A fragment lodged on employee's eyeball.

Struck against stationary object

While raising an overflow waterline at the tailings dan, a supporting piece of 2x6 lumber moved and struck the ee int he teeth.

As the employee stepped out of the office onto the steps, he lost his balance and fell onto the floor. He extended his right arm to break his fall and dislocated his shoulder.

Fall onto or against objects

Employee bumped right kneecap on access ladder while boarding a haulage truck.
