An unintentional roof fall occurred at the Galatia North Mine at the #2 Entry 13 West Longwall @ Crosscut 23. The fall was approximately 20' long, 20' wide and 12' thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up.

An unintentional roof fall occurred at the Millennium Portal Mine at the 1 North Headgate No. 2 entry at 5617 north. The fall was approx. 90' long, 18' wide and 12' -16' thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE GALATIA NORTH PORTAL BETWEEN THE 11TH & 12TH CROSSCUT, #2 ENTRY OF THE FLANNIGAN TRAVELWAY. THE FALL WAS APPROX. 25'L X 18'W X 8'THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF WORKERS. THE FALL WILL BE CLEANED UP.

An unintentional roof fall occurred at the Mine No. 6 at the Main South Entry #2 (MMU 010), 305 intersection. The fall was approximately 35' long, approximately 20' wide and approximately 8' thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.

AT THE GALATIA NORTH PORTAL, THE GOVERNOR ROPE ON THE ELEVATOR SLIPPED OFF THE PULLEYS. THE ROPE WAS EXAMINED & NO KINKS OR FRAYED WIRES WERE FOUND. MORE TENSION WAS PLACED ON THE ROPE TO PREVENT SLIPPAGE AGAIN. NO INJURIES WERE INCURRED.

AT THE GALATIA NORTH PORTAL, GUIDE ROPES HAD TO BE REPLACED AFTER THEY WERE DAMAGED BY A PIECE OF EQUIPMENT BEING LOWERED IN THE 8TH WEST AIR SHAFT. REPAIRS WERE COMPLETED BY 9:30PM ON 10/1/05. NO INJURIES WERE INCURRED & WORKERS WERE NOTIFIED TO USE THE GALATIA NORTH ELEVATOR AS THE PRIMARY ESCAPE FACILITY.

THE GALATIA NORTH ELEVATOR GOVERNOR ROPE GOT A KINK IN IT. THE ROPE HAD TO BE CHANGED OUT.

The Galatia North elevator became hung in the shaft due to lilly controllers being out of adjustment.

This is not a fall, it is just a little sloughage from a previous fall that happened on February 27, 2005, in the 5North Headgate between Entries 2, 3 and 4 at Crosscut 53458 North. It is of unknown length/width/thickness. It did not affect ventilation or passage of people.

Employee was getting out of the fork truck when his foot got stuck between the seat and side of the cab, causing him to twist his right knee.

WHILE WORKING ON A PUMP, EMPLOYEE PULLED ON A WRENCH AND POPPED SOMETHING IN HIS LOWER BACK. LOST TIME INJURY EFFECTIVE 1/26/06.

EMPLOYEE WAS WALKING AND TRIPPED ON A WOODEN PALLET AND FELL.

Employee was getting a part from the tool car when he accidentally hit a valve bank, causing it to fall. Whe tried to catch it, he felt a sharp pain in his lower back.

Employee was hanging miner cable in the #2 entry when he strained his left shoulder.

EE WAS PICKING UP A PUMP WHEN HE TURNED THE WRONG WAY AND PULLED A MUSCLE IN HIS GROIN. LOST TIME INJURY. DIAGNOSED WITH HERNIA - SURGICALLY REPAIRED.

EE was on the way out in a battery mantrip along with his crew and a american coal ee. when mantrip became stuck in mud american coal ee got a ram car and pulled the mantrip out of mud he was between mantrip and ram car, the chain was tight he told mantrip driver to pull up about 2 foot so he could release chain. mantrip would not move operator was hit foot pedal.

Employee was shoveling the belt and mid-shift began to feel pain in his left shoulder.

A ROCK FELL OUT FROM BETWEEN THE SHIELDS AND STRUCK EMPLOYEE ON HIS HAND WHILE HIS HAND WAS ON THE PANLINE.

A ROCK FELL OUT FROM THE ROOF AND STRUCK EMPLOYEE IN THE BACK.

Employee was cutting monorail and as soon as the chain was cut, the top fell in, hitting employee on the head, shoulder and left leg.

Employee was in a shuttle car and coming up a hill, his canopy hit a roofbolt, shearing the bolt and the plate hit him on the left wrist. Lost time injury 11/11/05.

An employee was building a stopping, setting block into place when it slipped out of his hands, falling and striking employee on the hand.

The mantrip employee was riding in hit a hole and bottomed out.

EMPLOYEE WAS ADDING STEEL TO DRILL A HOLE WHEN THE BOOM EXTENDED & THE JACK LUNGED FORWARD & CAUGHT EE'S HAND BETWEEN THE STEEL & THE CANOPY.

After servicing the 2105 haul truck, employee was going back up the ladder when the step broke. He pulled his left shoulder trying to hang onto the ladder.

EMPLOYEE WAS WORKING BACK TO BACK WITH A FELLOW EMPLOYEE PUTTING A CRIB TIE IN AN ARCH WHEN THE CRIB TIE FELL OVER THE FELLOW EMPLOYEE'S SHOULDER AND STRUCK EMPLOYEE IN THE CALF.

EMPLOYEE WAS HANDLING CRIBS IN MUD UP OVER HIS KNEES. EE TWISTED TO HAND OFF CRIB BUT WHEN NEXT MAN WASN'T READY, EE HAD TO STOP WHILE IN MOTION TO KEEP FROM HITTING NEXT MAN TO RECEIVE CRIB.

EMPLOYEE WAS SHOVELING AND BENT OVER TO PICK UP A ROCK WHEN HE FELT A SHARP PAIN IN HIS HIS BACK.

EMPLOYEE WAS LOADING BOLTS IN A SCOOP & HURT HIS BACK & FELL ON HIS RIGHT KNEE.

EMPLOYEE JAMMED HIS HEAD INTO THE CANOPY OF THE ROOF BOLTER AND INJURED HIS NECK. LOST TIME INJURY.

Working in belt slope putting rock on belt line slipped and fell and slide on loose rock and strained left hip and left knee.

WHILE WALKING TO THE FACE, EMPLOYEE STEPPED IN A HOLE AND TWISTED HIS KNEE.

Employee was struck on the shoulder, sliding down his back and left leg while helping to build a stopping. Lost time injury.

Employee was unloading a shield when the car lifted on one end. When the shield came out of the car, it caused the car to fall to the ground, which caused the employee to bounce off the seat, hitting the canopy.

Employee was stepping off of equipment when he twisted his knee. Reportable lost time injury October 14, 2005.

Employee was loading the roofbolter when rock fell from between the bolts, striking him on the back. Lost time injury.

THE RIB ROLLED OUT AND STRUCK EMPLOYEE IN THE BACK.

Employee was on his knees shoveling out the feet when screen wire being transported on the panline hit a rock causing the wire to strike employee on the left shoulder. Lost time injury.

Employee was spotting the bolter for the next row when he was struck by rock falling from the roof, mashing his foot. Lost time injury.

EMPLOYEE WAS TRYING TO CUT HIS STEEL ON THE CHOP SAW WHEN HE CUT HIS LEG.

ROCK FELL FROM BETWEEN THE HOG PANELS, HITTING EMPLOYEE ON THE HEAD AND SHOULDER. LOST TIME INJURY, REPORTABLE 7/22/05

EMPLOYEE WAS GETTING OUT OF THE CAB OF THE ENDLOADER WHEN HIS FOOT BECAME CAUGHT, CAUSING HIM TO LOSE HIS BALANCE & FALL.

EE WAS FILLING THE RAM CAR SCRUBBER TANK WITH A WATER HOSE. HANDLE WAS MISSING ON THE VALVE; EE WAS TRYING TO SHUT OFF WITH CHANNEL LOCKS. HOSE WAS DISLODGED AND STRUCK EE ON THE UPPER LIP. MEDICAL TREATMENT INJURY.
