AT THE GALATIA NORTH PORTAL, GUIDE ROPES HAD TO BE REPLACED AFTER THEY WERE DAMAGED BY A PIECE OF EQUIPMENT BEING LOWERED IN THE 8TH WEST AIR SHAFT. REPAIRS WERE COMPLETED BY 9:30PM ON 10/1/05. NO INJURIES WERE INCURRED & WORKERS WERE NOTIFIED TO USE THE GALATIA NORTH ELEVATOR AS THE PRIMARY ESCAPE FACILITY.

EE was on the way out in a battery mantrip along with his crew and a american coal ee. when mantrip became stuck in mud american coal ee got a ram car and pulled the mantrip out of mud he was between mantrip and ram car, the chain was tight he told mantrip driver to pull up about 2 foot so he could release chain. mantrip would not move operator was hit foot pedal.
Employee was shoveling the belt and mid-shift began to feel pain in his left shoulder.

EMPLOYEE WAS SHOVELING AND BENT OVER TO PICK UP A ROCK WHEN HE FELT A SHARP PAIN IN HIS HIS BACK.
EMPLOYEE WAS LOADING BOLTS IN A SCOOP & HURT HIS BACK & FELL ON HIS RIGHT KNEE.
EMPLOYEE JAMMED HIS HEAD INTO THE CANOPY OF THE ROOF BOLTER AND INJURED HIS NECK. LOST TIME INJURY.
