Roof Fall occurred in the #1 entry of the 4WTG of the bleeder entry. The fall location is in the intersection at 5120W to 5140W and the dimension is 10'T x 20'L x 18'W. A plan as been submitted to MSHA for a new EP at the fall area pending approval. The fall will be reference as 5B
Roof Fall occurred in the #1 entry of the 10WLW of the belt entry. The fall location starts outby at 5553' to 5586' and dimension is 12'T x 33'L x 18'W. The roof fall will be cleaned and re-supported according to the MSHA approved plan submitted and approved on 17JUL14.
An unintentional roof fall occurred in the 10WLW travel road between xcuts 36 and 35 at footage marker 4975 in the #2 entry. The dimensions of the roof fall is approximately 50'L x 10'T x 18'W. The roof fall was cleaned up. Roof fall is referenced as #31A.
An unplanned roof fall occurred on the Main West, Crosscut 25, Entry 4. The fall measured approximately 20 feet wide, 20 feet long, and 10 feet high. Compliance with rules and regulations was not a factor. The fall will be cleaned up in compliance with the mines roof fall cleanup plan.
Employee states he felt pain in groin area when he picked up a stack of bow tie plates while loading roof bolter. Lost time injury.
Employee was in x-cut loading supplies from the side of roof bolter. When the other roof bolter operator tried to raise the ATRS jack he inadvertently made contact with other employee toes of right foot with stab jack. Lost Time as of 12/26/2014

Employee was walking off of the longwall face and stepped in a hole injuring lower back. Lost time as of 1/28/2015.

Employee was placing T3 Channel on battery scoop. T3 Channel slipped and struck employee's left hand causing fracture to middle left finger. Lost time injury.

Employee was removing rock from the walkway and his right hand was struck by a piece of rock causing a hand fracture. Restricted Duty.
EE was operating a continuous miner on MU 3 section finishing the clean up in 4 right. According to unit EE's the injured EE was under supported top when found. The injured EE was concentrating on the clean up prior to the accident. Apparently a rock from the mine roof located at 4C fell out bounced off the floor then flipped over striking EE.
Employee was walking from parking lot to Prep Plant, slipped causing slight fracture to right foot. Restricted Duty.

Injured employee was working at the bottom of the airshaft when he was struck on the head by a piece of grout. The employee suffered a fractured skull and a laceration that required 5 staples. Employee was operating a Kubota on the travel road. The front tire caught a notch in the rib due to the employee glancing over in a x-cut while driving and the Kubota turned over on its side. The employee struck head on the roll over protection bar. Restricted Duty.

Employee was stacking rails on a rib corner and the rail slipped causing laceration to his right wrist. Had sutures in right wrist. Medical Treatment
