Accident type, without injuries

An unintentional roof fall occurred at the Galatia Mine New Era Portal at the 1610' West mark between #2 and #3 Entries, in the 10W Tailgate. The fall was approximately 25 feet long, approximately 18 feet wide and approximately 8 feet thick. The fall area will be re-supported according to the MSHA approved Re-support Plan. Fall #29B

An unintentional roof fall occurred at the New Future Mine 4W Headgate Entry #3, 5627W, Crosscut 41. The fall was approximately 25 feet in length, approximately 25 feet in width and approximately 20 feet thick. The fall did not affect ventilation. The fall will be cleaned up. Fall #29E

An unintentional roof fall occurred at the New Future Mine 4W Headgate Entry #1, 6400 W, Crosscut 46. The fall was approximately 20 feet in length, approximately 18 feet in width and of undetermined thickness. The fall did not affect ventilation or passage of workers. The fall will be cleaned up. An unintentional roof fall occurred at the Galatia Mine New Era Portal in the 5 Seam (which is inactive and mined out) at 675 West / 1425 North, #4 Entry. The fall was approximately 30 feet long, approximately 15 feet wide and approximately 6 feet thick. The fall area will be resupported according to the MSHA approved Resupport Plan. Fall #29A

An unintentional roof fall occurred at the 4W Headgate Entry #2, 4840 W. The fall was approx. 27' L. x approx 18' W. and of undetermined thickness. The fall did not affect ventilation. The fall will be cleaned up. Fall#29C

Accident type, without injuries

The crane was being moved into position to make a lift on a containment tub when the cranes boom came into contact with some overhead power lines. Employee stayed in the crane until it was confirmed that the power was off the lines, he was not injured and was checked out at a local medical facility to confirm there were no injuries to him.

The fall was approximately 42 feet in length, approximately 18 feet in width and approximately 10 - 12 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up. An unintentional roof fall occurred at the New Future Mine 1W Headgate Entry #1, Crosscut 16 @ 2077W. The fall was approximately 30 feet in length,17.5 - 23 feet in width and approximately 12 - 14 feet thick. The fall will not be cleaned up. Fall #29A

Employee was helping to turn water off at water tank when his foot slipped off the concrete pad, causing him to stumble and fall.

EE was using a hose to wash a ramcar when he lost his footing and fell on his right shoulder. Reportable lost time injury 10/30/12 due to surgery.

Over-exertion in pulling or pushing objects

Employee was pulling on screen wire. When the wire caught on the bolter, employee jerked at the wire and stained his lower back. Lost time injury.

Employee stated that while lifting high voltage cable and strained his lower back. Reportable lost time injury 9/28/2012 due to surgery.

Employee was picking up scrap steel when it slipped from his hand pinning his hand between the steel and a wooden pallet cutting his left middle finger.

Struck by falling object

Employee was pulling the roof bolter through the curtain when the ATR caught on the curtain. Employee got off the bolter to remove the curtain when a rock on the rib fell and his employee.Lost time injury

Employee was using a wrench to tighten bolts. When the wrench slipped, he fell backwards. Medical treatment injury.

Employee was using a hammer on a belt core to remove belt when a piece of metal flew off and struck his left arm. Lost time injury

Employee states he was loading a CAT idler on a vehicle when his right middle finger became caught between the idler and the fire suppression tank. Restricted Duty Injury.

Employee slipped when dismounting a ladder. Reportable lost time injury 12/07/12 due to surgery

Employee states he was operating the continuous miner when he was struck in the chest by a rock. Medical treatment injury. Revised: As of 10/22/2012 became reportable lost time injury due to surgery.

Employee stated he slipped and fell while walking in wet conditions. Reportable lost time injury as of December 7, 2012 due to surgery.

EE was breaking hub bolts loose on a belt drive with the breaker bar over his right shoulder. As he lifted the bar up, he felt a pop in his right shoulder.Lost time as of November 8, 2012 due to surgery.

Employee states he was grading roads and inadvertently bumped his hard hat on a roof bolt. Reportable lost time injury 10/5/12 due to surgery.

EE states he was riding in a mantrip hit a bump, causing him to strike his head on the roof of the mantrip and bounce back down into the seat, causing pain in his lower back.Reportable lost time injury 10/30/12 due to surgery

EE slipped and fell while loading planks for overcast construction and injured his left wrist. Restricted duty as of 8/10/12.

Employee states he was injured when he was carrying a pump, tripped and fell on his left knee. Employee became a lost time injury due to surgery on 8/28/12

EE stated that while bolting, he extended the boom and it caught on the track and threw him upward. When he used his right arm to brace himself he felt pain in his right shoulder and elbow.Reportable lost time injury as of 8/15/12 due to surgery.

Employee was helping put a steel beam on top of a longwall shield when a rock fell on the beam, causing the beam to fall and strike employee on the right arm. Lost time injury.

EE was removing bolts from the bumper of a ram car and strained his abdomen.Reportable lost time injury on 7/2/12

Employee was checking voltage on an electrical circuit when he received flash burns. Lost time injury.

Employee stated that while lifting and stacking T-channel, he felt a sharp pain in his lower back. Reportable lost time injury 08/13/2012 due to doctor's slip from personal physician

EE was checking water levels in the scoop battery when the lid fell onto his left wrist/arm. Lost time injury

Employee was using a shovel working with concrete when the shovel hung up on a bolt in the drive pad, causing him to slip and twist his left knee. reportable lost time injury as of 5/17/12 due to surgery.

Employee was assisting in aligning the head roller at the 5 ROM on the surface. The bearing housing broke, causing the head roller to shift, striking employee's left foot. Lost time injury.

Employee finished chocking the wheel of a trailer on the cage when he tripped on a door stop and fell on his left shoulder and right knee. Reportable lost time injury on 5/24/12 due to surgery.

Employee was taking down monorail when a rail fell, striking employee's right foot. Original x-rays taken at the time of the injury were negative for fractures. Re-examination and additional x-rays on July 5 indicated a fracture of ankle. Reportable medical treatment injury 7/6/12.
