An unintentional roof fall occured on the Main West Travelway, crosscut 25, entry 3. The fall measured approximately 25 feet in length, 18 feet in width, and 11 feet in height.
An unintentional roof fall occurred on the 5 West Long Wall, Crosscut 56, in the intersection of number 2 entry. The fall measured approximately 18 feet wide, by 18 feet long, by 16 feet high.
An unintentional roof fall occurred in the 6 seam slope belt between footage markers 1390 and 1420. The roof fall was approximately 10' wide x 20' long x 5' tall. The operator's site specific roof fall clean-up plan in the slope was submitted and approved by MSHA. The fall reference number is 30F.
An unintentional roof fall occurred on the 5 West Long Wall at crosscut number 60, entry number 2. The fall measured 22 feet in length, 16-18 feet in width and 20-25 feet in height. The area was supported with 12 and 16 foot cable bolts, T-3 channel, and wire mesh.
An unintentional roof fall (30E) occurred in the 10WHG footage marker (approximately) 7295, xcut 51, between #2 and #3 entries. The dimensions of the roof fall are 10'T x 18'W x 16'L. The fall will not be cleaned up.
An unintentional roof fall occurred in the 10WHG footage marker (approximately) 7285, xcut 51, #2 entry. The dimensions of the roof fall are 18'W x 10'T x 30'L. The fall will be cleaned up and a roof control addendum has been submitted and is currently under MSHA review.
An unintentional roof fall occurred in the Main North Return xcut 37 footage marker 5592 entry #2. The dimensions of the roof fall is approximately 17.5'W x 12'T x 40'L. The fall will not be cleaned up and a plan was submitted and approved to re-route the lifeline to #1 entry. Roof fall #30 - C.
At approximately 2:55 a.m. an incident occurred involving a dozer operator who inadvertently entered the number 4 feeder located at the raw coal pile. The employee safely exited the dozer uninjured. Job skills and miner's proficiency may be a factor.
An unintentional roof fall occurred in the 10th West Head Gate in the #2 entry between survey footage marker 5934'W to 5956'W. The fall was approximately 15'W x 18'L x 15'T. Employee states his hand was outside of the cab of the scoop, and when turning 3 of his fingers were caught between the cab of the scoop and the rear section of the scoop. The injury occurred when the contractor was struck on the left hand by a crib block that had fallen from an adjacent set of cribs that were being erected. The contractor received a fracture to the left hand. Cause of the injury was working below areas where objects could fall from above.
Employee was pulling an existing 90 foot utility pole with a digger derrick truck. The employee was rotating his boom to the drivers side of the truck when the weight started causing the passenger side outriggers to come off the ground. He then boomed down & winched off lowering the pole but it flipped the truck throwing the employee off.
