Strands were found broken on the regular lay slope hoisting rope. Rope was taken out of service and corrected. This was the secondary escape way and was rerouted underground to another hoist until rope was corrected.

Roof fall occurred on the 5 seam air course. Location is 750W 525S. An action plan was submitted and approved by MSHA to install EP's at approved locations. The fall will be referenced as 33A
A methane ignition occurred on the 12WLW MMU-059 at approximately 7:45 pm on 06/08/2016. The ignition was on the headgate drum while shear was mining to the headgate end. This 7000-1 is being submitted to terminate citation #9103414.
Oil leak was noticed during operation of the end loader. Foreman instructed operator to block the machine & went to get oil. Returned & found EE under the machine looking for the leak. Loader had been raised by tilting the bucket on the ground. Foreman got under machine to help look. Machine came down on top of them shortly after that, just as foreman saw that it was not blocked.
Was moving fan spacer with another employee, put end on trailer & when helper put end down finger was caught between spacer & trailer. Cause of injury was communication with helper. Was shoveling longwall feet & was using left foot to brace self. Foot slipped causing to fall backwards on a piece of steel. Cause of injury wet conditions, body positioning. Employee was shoveling on plant feed belt & picked up big load of mud on shovel, when turned to put on belt strained back. Cause of injury to big of load & body positioning. Employee had unloaded supplies off fork truck. Went back & stepped onto fork truck, got to second step on ladder, turned & foot slipped, toe went into mud on ground & fell forward. Employee was riding out in a man trip when man trip hit a hole in travel way causing employee to be bounced around in compartment. Cause of injury was an undetected hole in travel way. Employee was walking around the roof bolter on the 1st East Headgate slipped braced self with right arm on the coal rib injuring right shoulder. Restricted duty. *Amended to lost time as of 1/19/2017. Employee has not returned to work.

Employee was looking for obstruction between overland belt & chute, while climbing off belt, slipped and fell to ground. Cause of injury no 3 point contact while climbing down.
