Strands were found broken on the regular lay slope hoisting rope. Rope was taken out of service and corrected. This was the secondary escape way and was rerouted underground to another hoist until rope was corrected.

Roof fall occurred on the 5 seam air course. Location is 750W 525S. An action plan was submitted and approved by MSHA to install EP's at approved locations. The fall will be referenced as 33A

A methane ignition occurred on the 12WLW MMU-059 at approximately 7:45 pm on 06/08/2016. The ignition was on the headgate drum while shear was mining to the headgate end. This 7000-1 is being submitted to terminate citation #9103414.

Was moving fan spacer with another employee, put end on trailer & when helper put end down finger was caught between spacer & trailer. Cause of injury was communication with helper. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing 7/19/16

Was shoveling longwall feet & was using left foot to brace self. Foot slipped causing to fall backwards on a piece of steel. Cause of injury wet conditions, body positioning. Employee was struck by equipment doors upon removal. (As of 11/17/2016 Lost time).

Employee was caught between long wall shield tip and mine rib. Compliance with rules and regulations was not a factor. Lost time as 11/07/2016

Contractor was carrying pipe when EE's legs gave out. EE fell to knees and strained back. The pipe hit shoulder. Lost time as 11/03/2016. EE was a contractor working for David Stanley consultants (Contractor ID YBV)

Employee was shoveling on plant feed belt & picked up big load of mud on shovel, when turned to put on belt strained back. Cause of injury to big of load & body positioning. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/28/2016.

Employee had unloaded supplies off fork truck. Went back & stepped onto fork truck, got to second step on ladder, turned & foot slipped, toe went into mud on ground & fell forward. Cause of injury wet, muddy conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/24/2016.

Temporary mine employee was standing on second ladder rung and ladder twisted causing injury to employee's right ankle. Lost time.

Was down in the #1 reclaim tunnel on the #4 feeder landing changing a nozzle on hose to a wand. Feeder broke free causing surge of material to pin against hand rail of the platform. Cause of injury material falling due to wet conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing 7/8/16

Was helping end loader operator look for leak under machine, machine suddenly dropped pinning between end loader frame & mine floor. Cause of injury machine not blocked. Compliance with company rules/regulations was a factor, miner proficiency may have been a factor. Started missing work 6/8/16.

Temporary mine employee while shoveling slope strained lower back. Lost Time.

Employee was shoveling in the slope. EE noticed back starting hurting. Cause of injury bad body positioning while on incline. Incident didn't involve any aspect of compliance with rules, regulation, & PPE. Miner proficiency may have been a factor. Started missing work 6/06/16. This is a DSC contractor.

Alleged incident was to have occurred while attempting to wash down the slope. Walking backwards and slipped on oil. Cause of this injury was employee awareness. Compliance with rules/reg., PPE, or mining equip. not a factor. Mine proficiency could be a factor. Employee continued to work. Started missing work on 4/22/2016.

Employee was spraying shear drum with water using the road hose when rock flew off drum and smashed hand on pan line. Cause of injury was hand positioning while cutting rock. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may have been a factor.

Employee was picking up a nitrogen bottle to put in the 4 bucket of scoop for transport. Employee twisted & felt a stabbing & burning in stomach. Cause of injury was twisted body with heavy load. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may be a factor. Started missing work 3/12/16

Changing out DA ram cylinder. Very tight fit in relay bar. Laying on stomach with 12 # sledge arms extended forward driving ram to relay bar pocket. Felt a twinge in shoulder. Employee was working on Longwall & tripped. Employee fell with arms stretched outward & landed on right wrist/hand. Cause of injury was gob in front of shield feet. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may be a factor. Started to miss work 2/5/16.

Employee was walking and tripped over miner cable fell and injured left shoulder.( Lost time as of 02/05/2016.)Compliance with rules and regulations was not a factor.

Employee was riding out in a man trip when man trip hit a hole in travel way causing employee to be bounced around in compartment. Cause of injury was an undetected hole in travel way. Incident did not involve any aspect of compliance with rules and regulations & PPE. Mine equipment involved & miner proficiency may be a factor. Started to miss work 2-9-16.

Employee was walking around the roof bolter on the 1st East Headgate slipped braced self with right arm on the coal rib injuring right shoulder. Restricted duty. *Amended to lost time as of 1/19/2017. Employee has not returned to work.

Employee was removing cover off rear of road grader and felt pain in right shoulder. The road grader was parked in the supply/air shaft area. Restricted Duty as of 12/27/2016. *Amended to lost time as of 1/12/2017. Employee has not returned to work.

Employee was handling material and heard something crack behind EE and a rock fell from the roof striking EE on the lower back and left leg causing fracture to leg. Lost time as of 11/28/2016

Employee was looking for obstruction between overland belt & chute, while climbing off belt, slipped and fell to ground. Cause of injury no 3 point contact while climbing down. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 11/22/2016

Employee was walking thru mud and slipped on crib buried in the mud. Compliance with rules and regulations was not a factor. Lost time as of 10/18/2016

Employee was lifting a 13 HP flyte pump when felt a pop in shoulder in left arm. Cause of injury wet & muddy conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/27/2016.

Employee was walking along the first submain North take-up area at the drive. The employee slipped falling on right knee and shoulder. Lost time as of 2/16/2017.

Was spreading rock dust that was on travel way with drag attached to ram car. Was watching drag when a piece of T-Channel on rib hit me. Cause of injury T-channel loose on one end sticking out. Was washing out #3 feeder in #2 reclaim when a large chunk broke free & wedged between feeder belt & catwalk. Catwalk raised & lost balance falling from platform. Was in process of weekly examination of bleeders & was walking in 10WTG when I stepped over loose rock & twisted knee. Cause of injury was wet conditions. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. Started missing work. 6/9/16

Employee's foot slipped off walk bridge and twisted knee. Cause of injury was wet and muddy conditions. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may have been a factor. Started missing work 4/14/2016.

Employee was making pre-shift examination when ee stepped out of vehicle at the 11WHG and stepped on a piece of concrete block & fractured foot and ankle. Cause of injury was stepping on uneven floor. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may have been a factor. Was pulling a charge pump onto landing when I slipped & holding onto the pump so I wouldn't fall I strained my back. Cause of injury was wet conditions. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. Started missing work 6/30/16.

EE was carrying a screw prop on right shoulder beside stage loader. EE tripped on belt rail. EE tried to throw prop off shoulder and ran head into pump & landed on left shoulder. Cause of injury was wet/muddy conditions. Incident did not involve any aspect of compliance with rules, regulations,& PPE. Miner proficiency may be a factor. Started to miss work on 2/11/16.

Employee was taking a piece of metal to grinder. Stepped over a roll of lagging & caught foot on lagging causing ee to trip & fall. Landed on right shoulder & arm. Cause of injury not having a clear walk way. Incident did not involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may be a factor. Started missing work 2/25/16.

Temporary mine employee was carrying a concrete block and dropped it on left foot causing injury. Restricted Duty.

Employee was performing duty as an examiner walking beltline. Employee lost footing straining right ankle. Restricted Duty.

Employee was working on the face of the 1A East Long Wall, pulling shield and right foot was struck by crib block causing contusion. Restricted Duty.

Employee was struck by rock on right hand while standing at the Main North 2 Belt Head. Restricted Duty.

Employee was knocking out block from brattice and bent over to pick up block when a block fell from above striking the second and fourth digit on left hand causing fracture and laceration sutures required. Restricted as of 11/27/16.

Employee bent over handling cable and pulled something in back. Restricted duty as of 11/13/2016. compliance with rules and regulations was not a factor.

Employee was prying on lid and bar slipped and EE fell back on EE's leg. Restricted duty as of 11/14/16.

Employee was assisting mechanic putting chain back together when mechanic struck piece of roller chain and a piece of metal struck employee in the right arm. As of 11/09/2016 restricted duty sutures required.

Employee states while employee was installing roof bolts in the Main North 2 travelway the roof bolter machine was bumped by a ram car causing injury to right hip. Restricted Duty.

Loading roof bolter with supplies. The utility person passed supplies with the other employee back towards EE. EE turn into the roof bolt causing laceration to EE's nose. **Restricted duty** sutures required.

Employee was loading I beams dropping one end of the I beam on left foot. Restricted Duty.

Employee was hanging miner cable when a piece of tie wire made contact with left eye. Restricted Duty.

Employee while walking at the longwall (10WLW) face stepped in mud and twisted right leg. Restricted Duty

Employee had placed hand on dust box door and a other employee shut door on EE's hand causing laceration to left hand. (Medical treatment sutures required) Compliance with rules and regulations was not a factor.

Employee was roof bolting on the 10 East Headgate at face. Roof bolting steel slipped injuring middle left finger. Restricted Duty.

Employee slipped while carrying crib block twisted and injured lower back.

Employee was hanging a piece of tin when a another piece of tin came loose and struck right wrist.(Medical treatment sutures required) Compliance with rules and regulations was not a factor.

Washing guarding at slope tail, coal loosened under guarding around tail roller dropping guard on left hand. Cause of injury, hand placement. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. This is a Contractor for DSC contracting.

Was working in slope swinging a sledgehammer busting concrete. broke through concrete & smashed finger between handle & concrete. Cause of injury not paying attention work area. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. This is a Contractor for REDBIRD construction.

Working on C side & had left hand behind bracket & took impact tool to run bolt in. Bolt went through bracket and smashed left small finger. Cause of injury hand placement. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor.

Walking down slope East side & slipped on wet cement floor & struck knee on metal plate. Cause of injury floor wet & on incline. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor.

Employee was handling belt material and smashed right index finger on structure. (Medical treatment sutures required) Compliance with rules and regulation was not a factor.

During roof bolt installation employee injured the 2nd and 3rd digit on left hand causing laceration. (Medical treatment)Compliance with rules and regulations was not a factor.

Employee was roof bolting and holding the roof bolt steel in the roof with one hand and the steel slipped out of hand and fell onto left index finger causing laceration. (Medical treatment sutures required) Compliance with rules and regulation was not a factor.

Death occurred from natural causes. Person suffered a fatal heart attack.
