EE walk to the safety trailer where EE received first aid treatment until the paramedics arrived and took EE to the hospital for treatment of EE's burns. Employee not know power on
Employee was vacuuming material through plastic hose when static built up was discharged into right knee, jolted up to chest and head, and down leg. Unknown if EE hit EE's head. Diagnosed with concussion.
An employee was throwing a breaker when it arc flashed. Employee received minor burns to face and hands.
Employee was on top of the railroad car with his harness intact and roped. He lifted the pole into the air, at that point the power line arced a discharge towards his pole. He stood straight up and fell off the railcar. Received 3 burns, chest, right arm, left leg. The electricity exited his body through his left ankle.
MCC went to ground. Power was turned off and breaker was repaired. When MCC was renergized, the breaker blew and caused a flash burn to hand. New breaker was believed to be faulty.
EMPLOYEE WAS SECURING THE OVERHEAD CRANE SNATCH BLOCK TO A STEEL SLING IN ORDER TO REMOVE EAST MANHOLE COVER. AS HE WAS DOING SO HE FELT AN ELECTRIC SHOCK THROUGH HIS BODY.
EE WAS PULLING OUT FUSE. HE HAD BEEN TOLD POWER WAS OFF. SHOCK & BURN FROM CONTACT WITH 2300 VOLT.
INJURED AND OTHER EE WERE ASSIGNED TO REPLACE 2 BREAKERS IN LCK1 UNIT SUBSTATION. THE BREAKERS WERE K1A AND CH. THE BREAKERS WERE OPENED AND DISCHARGED, THEN RACKED OUT TO THE DISCONNECT POSIT ION. THE BREAKER DOORS WERE THEN OPENED AND THE TRIP SETTINGS WERE TRANSFERRED TOT THE REPLACEMENT BREAKERS. FIRE AND FLASH FROM THE EXPLOSION ESCAPING THE STARTER CUBICLE BURNED BOTHS EE'S.
INJURED AND OTHER EE WERE ASSIGNED TO REPLACE 2 BREAKERS IN LCK1 UNIT SUBSTATION. BREAKERS WERE K1A AND CH. THE BREAKERS WERE OPENED AND DISCHARGED, THEN RACKED OOT TO THE DISCONNECT POSITION. BREAKER DOORS WERE OPENED AND THE TRIP UNIT SETTINGS WERE TRANSFERRED TO REPLACEMENT BREAKERS. FIRE AND FLASH FROM THE EXPLOSION ESCAPING THE STARTER CUBICLE BURNED BOTH EMPLOYEES.
EMPLOYEE WAS PLUGGING A JIG SAW INTO AN EXTENSION CORD TO CUT A PIECE OF PLYWOOD. THERE WAS A BARE SPOT IN THE EXTENSION CORD ALLOWING THE EMPLOYEE TO COME INTO CONTACT WITH 110 VOLT ELECTRICA L CURRENT IN THE CORD. EMPLOYEE APPEARED TO RECEIVE A MINOR SHOCK. EMPLOYEE COMPLAINED OF A TINGLING SENSATION IN ARM.
EMPLYEE HEARD ON PLANT RADIO THAT THE PLANT AIR COMPRESSOR WENT DOWN AFTER STARTING #1 FINISH MILL SYSTEM. HE WENT DIRECTLY TO THE COMPRESSOR STARTER. SHIFT UTILITY MAN TRIED STARTING THE COMPRESSOR. AN ELECTRICAL EXPLOSION OCCURRED IN THE STARTER BREAKER PANEL RESULTING IN FLASH BURNS TO EMPLOYEE'S LEFT SIDE, 1 DEGREE TO LEFT SIDE, 2ND AND 3RD DEGREE TO ARM.
A #6 WIRE HAD A NICK IN IT'S INSULATION AND SHORTED TO METAL IN THE MCC WIRE WAY. RIGHT BEHIND WHERE THE SHORT ACCURED WAS THE 800 AMP MAIN BUSS. WHEE THE #6 WIRE THE #6 WIRE SHORTED IT MUST OF TRACKED OVER TO THE 800 AMP BUSS WHICH THEN SHORTED TO THE METAL IN THE SAME LOCATION AS THE #6 WIRE SHORTED.
