While flushing out radiator on end loader in the Clintwood Spoil area, employee was on counter weight in rear of loader with water hose. Flushing out radiator when they slipped and fell approximately 6' from counter weight of loader to the ground. Landing on their back.

EE was tightening the tire chains on MG-11 and felt something pul in his right shoulder.

Employee was hooking up light plant to his pick up, he was liptims upon the ongue of light plant to align it with the hitch on his pickup. The light plant rolled forward and caught his middle finger on his left hand between the light plant and the trailer hitch.

Employee was taking plug out of air tank. Pressure pushed hand sideways away from the tank into the housing, breaking his pinkie finger on his right hand.

Employee was operating a drill and had opened the door. When he closed the door he caught his left index finger and thumb in the door. Left thumb was amputated from the lack of his fingernail (tip)

Employee opened door of hauler to check a fuse behind the driver's seat when the door shut and mashed his left ring finger requiring stitches.

Employee turned over a piece of metal and it fell on his left foot.

Cut leg on sharp edge of drum that had been cut in order to use it as a drain pan.

EE was pulling electrail cord, cord got hung up and when EE gave the card a jerk, it came free and hit EE above the left eye. Causing a cut requiring 3 stiches.

Employee was laying on his side & picked up a crib block, placed it against the idler on a D-10 Dozer. He rolled over onto his back to get up then felt a sharp pain in his neck and shoulders, then running down his spine. Suffered headache and nausea.

Employee shut the drill door on his middle and ring fingers on his right hand.

Employee opened the door to enter the cab of a 980 loader. The door swung open hitting him in the left elbow. He did not seek medical attention until 10-18-07.

Employee was replacing track on D10T, pulling and lifting caused him to pull the webbing from a hernia injury two years ago, not job related.

EE was driving a pin into his excavator track & hit his little finger on the right hand on the track. Mashed his finger & lost his finger nail. Required stitches.

Employee was dismounting dozer, said he twisted his back as he turned after dismounting. Did not miss work until 5/22/07.

Employee was stepping off his grader when he hopped off the step, he heard a snapping sound & started having severe bak pain & muscle spasms.

Employee bent down to open a valve on water tank and felt a twinge in his lower back.

Mounting water truck using the grab bar and felt something pop in his left elbow. (Did not seek medical attention until later.) Missed 8 days from work.

EE was working on drill, got dust/rust in right eye. Rinsed and thought it would be okay. It was worse the next day, sought medical attention.

Employee was unloading parts from parts truck and felt pain in his right leg.

Employee was lifting brake caliper off of truck when he experienced pain in his lower back. This injury was not reported until 11/21/06.

Employee was lifting slate rock from the rock crusher and hurt his back. He first sought medical attention on 9-26-06. Did not miss work until November 1, 2006.

EE was putting bed cable back in hanger when cable twisted and caught his thumb.

Employee was working on a drill when he bumped his elbow on the undercarriage, chipping a bone in his elbow.

Employee was bent over to pull the entrance ladder up and felt a sudden sharp pain in his left lower back area.

EMPLOYEE UNDERNEATH DRILL LOOKING FOR AN OIL LEAK, CAUSED DUST AND GRIT TO ENTER HIS LEFT EYE.

Employee was loading a tank of oxygen on his welding truck, it caught on the bumper of the truck, twisting his back.

Employee was using 1 1/2" cable to pull dozer and when finished, attempted to remove cable from the dozer and in the process the clevis struck ee in upper left corner of left eye causing a laceration requiring 8 stitches.

Employee was preparing to raise access ladder on shovel, he bent over to pull the release pin, felt a pop in his lower back.

Employee was shoveling underneath a conveyer beltline and reported that he felt sharp pain in his back when he twisted to empty his shovel.

EE WAS ATTEMPTING TO MOVE LIGHT BY PULLING ON LIGHT CORD AND THE LIGHT SPRANG TOWARD HIM AND HIT HIM ON THE LEFT TOP SIDE OF HEAD. REQUIRED 5 STAPLES.

EE WAS HELPING TAKE TRACK SEGMENTS OFF OF DOZER. ONE SLIPPED AND HIT HIS RIGHT HAND, INJURING HIS MIDDLE FINGER. REQUIRED SUTURES (LACERATION/FRACTURE).

EE WAS SHOVELING MUD OFF THE EDGE OF HAUL ROAD AND CLEANING OUT A DITCH. ALLEGES HURT BACK

EE HAD BEEN HELPING MECHANIC HOLD HOSES IN PLACE UNDER DOZER AND WHEN EE TRUNED AROUND, FELT BACK POP

Employee was preparing to change undercarriage on D11 dozer. After blocking machine and coming out from under it, he felt a sharp pain in right leg groin area.

Employee and another co-worker were repairing an rh200 shovel and were using a battery powered light. the battery part of the light was knocked to the ground and in doing so jerked the light out of co workers hand and knocked injured ee in the mouth breaking a tooth.

A BOGIE PIN ROLLED OFF A SHELF IN THE PARTS TRAILER & LANDED ON EMPLOYEE'S LEFT HAND, MASHING HIS MIDDLE FINGER.

Employee was installing a hose on a Cat 5230 shovel in the boom section. When he let go of the hose, it popped out of the flange and hit him on his nose and under his right eye. This caused a cut that required (3) stitches.

CUT LITTLE FINGER PICKING UP A FUEL PUMP. THIS REQUIRED FIVE STITCHES.

Employee had just taken a chain out of the side compartment of the boom truck and started to hook the chain on the boom hook, and as he turned he felt a severe pain in his back.

EMPLOYEE PULLED HIMSELF UP ONTO HIS DOZER BY USING THE GRAB RAILS. HIS BACK STARTED HURTING.

EMPLOYEE WAS INSTALLING A VALVE ON A 994 CAT LOADER. THE VALVE SHIFTED PINNING HIS LEFT HAND AGAINST THE FRAME OF THE LOADER. NO LOST TIME.

EE WAS INSTALLING HOIST JACK PIN WEIGHING 40-60 LBS. HE FELT PAIN IN HIS STOMACH.

WHILE STEPPPING OUT OF A FIELD TRUCK, HE CAUGHT HIS FOOT IN A SHIM THAT WAS LYING ON THE GROUND.WHEN HE STARTED PICKING THE SHIM UP TO PUT IT IN ANOTHER LOCATION IT WAS FROZEN TO THE GROUND & WHEN HE PICKED IT UP IT CUT HIS FINGER.

EE WAS CHECKING OUT BOBCAT LOADER, HE OPENED DOOR TO CHEEK ENGINE OIL AND DOOR SWUNG BACK CLOSED CATCHING HIS FINGERS IN THE DOOR. CUTTING HIS LEFT INDEX FINGER REQUIRING 2 STITCHES.

EMPLOYEE WAS STANDING ON BUMPER OF SERVICE TRUCK REACHING FOR A PRY BAR AND FELT SOMETHING HIT HIM IN THE EYE.

EE BENT OVER TO CLEAN MIRROR AND FELT SOMETHING POP IN HIS LOWER BACK.

EE WAS WORKING ON A 789 HAULER TAKING OFF THE REAR SUSPENSION CYLINDERS...THE WIND WAS BLOWING HARD, SOME DIRT OR OTHER OBJECT CAME OFF THE CYLINDER INTO EE'S EYE.

EE WAS PUTTING HYDRAULIC TANK COVER BACK ON A 789 HAULER. THE COVER SLIPPED & PUSHED HIS THUMB BACKWARDS. EE WORKED UNTIL MARCH 29, HE HAD SURGERY ON HIS THUMB ON THIS DATE.

EE WAS REPLACING HARD BAR BUSHINGS ON DOZER. THE MACHINE WAS BEING MOVED UP AND DOWN BY THE HYDRAULICS WHEN A WEDGE THAT WAS BEING USED WAS STRUCK AND DISLODGE, STRIKING THE EE.
