Struck against stationary object

Employee was sliding under mechanics truck to check an oil leak, hit his head on a piece of metal cutting his head. This cut required two stitches.

Employee was using 2 come-a-longs to align the top plate of the carousel so it could be welded. One of the come-a-longs slipped while employee's hand was under the plate and mashed his right ring finger requiring seven stitches.

EE was pulling electrail cord, cord got hung up and when EE gave the card a jerk, it came free and hit EE above the left eye. Causing a cut requiring 3 stiches.

Mounting water truck using the grab bar and felt something pop in his left elbow. (Did not seek medical attention until later.) Missed 8 days from work.

Employee was raising major bogie using a come-along to install rollers. The come-along slipped, allowed the bogie to fall mashing his left hand between the bogie and the dozer frame.

Operator backed his hauler through the berm, overturned and fell approximately 40 feet. He sustained a cut on his hand requiring stitches.

EMPLOYEE WAS OPERATING 988 LOADER PULLING ROCK OFF OF COAL. WHEN GOING IN FOR A BUCKET OF ROCK, THE LOADER BUCKET HIT SOLID & TWISTED. HE COMPLAINED OF PAIN IN HIS LOWER BACK & NUMBNESS IN HIS LEFT LEG.

ON 10/20, EE WENT TO THE DR., CONTINUED TO WORK THRU 10/26/05 & DEVELOPED A BLOOD-CLOT IN HIS RT. LUNG, HE WAS HOSPITALIZED & DISCHARGED ON 10/28/05. EE NOTIFIED COMPANY THAT THE BLOOD-CLOT WAS RELATED TO INJURY ON 11/01/05.

EMPLOYEE WAS SCALING THE HIGHWALL USING AN EXCAVATOR. A ROCK CAME OVER TOP OF THE BUCKET THROUGH THE WINDSHIELD AND HIT EE ON HIS LEFT KNEE.

EE WAS WORKING ON A DRAG LINK ON A PICKUP TRUCK AND WHILE INSTALLING THE FRONT LEFT TIRE, HE FELT A PULL IN HIS BACK. EE WENT FOR MEDICAL ATTENTION AND RETURNED TO WORK THE SAME DAY. THEN HE W ORKED UNTIL 3-4-02, WHEN DR. REQUESTED HE STAY OFF WORK FOR SEVEN DAYS & THEN CHECK BACK WITH HIM.

ROCK FROM THE HIGHWALL HAD A MUD SCAM BEHIND IT,THE ROCK FELL OUT AND HIT THE BUCKET AND BOOM ON 992-G LOADER. THE OPERATOR WAS WEARING HIS SEAT BELT, BUT HE WAS JOLTED FROM THE FORCE OF THE R OCK HITTING THE LOADER. THIS CAUSED HIM TO HIT HIS KNEE ON THE HYD CONTROL VALVE AND TWISTED HIS HIP.
