The employee requested to go to the company clinic on 10/19/2007 and was placed on restricted duty. While exiting the VMC excavator, the employee stepped on the last step of ladder with R-toes only, causing an alleged sprain to R-great toe.
A WMC Inc. employee was removeing rollers from a conveyor below the C2A conveyor platform while a Vulcan Employee was removing an inspection cover from a motor on the platform above. The Vulcan employee lost control of the cover striking the WMC employee on his hardhat from a drop of approximately 10-15' striking several points of the conveyor structure on the way down.
Employee was washing rock from elevated catwalk with a high pressure water hose. The trailing hose slipped off the catwalk, pulling the hose from employees hands. The hose whipped around and the nozzle stuck him in the lower back on the right side. Injury was reported the day of occurrance, did not seek medical care until 10/5/07
Employee was descending the ladder from his CAT 992G Loader (at the end of his shift). As he was stepping down from the last rung to the ground (using his right leg) he heard a pop in his left knee that was still on the last rung of the ladder (the swinging rung).
The injured was handing a rock to a co-worker while inside the crusher clearing it out. It slipped out of the co-worker's hand and fell about two feet - hitting the injured's left shoulder.

Supervisor/employee was loading screen tubes onto a boom truck with 2 other employees. The supervisor/employee caught his finger between one of the screen tubes and the boom truck.

When he looked down at his arm he saw that the bicep muscle was out of place. Diagnosed as a ruptured biceps tendon and placed on restricted duty 07/24/07.
Injured employee had the deck washed off and was removing a 990 shank and tooth loader from the platform when he lost his grip and dropped shank and tooth loader on his right foot. Resulted in a contusion.

A plant maintenance employee was replacing a screen bearing with the assistance of a co-worker. The two employees were moving the bearing housing when the co-worker lost his hold leaving the injured employee supporting the bulk of the weight causing a strained left shoulder. Employee reported incident to supervisor but did not seek medical attention at the time.
Installing expanded metal 4 x 8 sheet. Employee alleges back strain.
While moving scrap parts in the boneyard, a t-roller fell on his hand, cutting finger on left hand

Bolt dropped unexpectedly, bounced striking left hand and slicing the thumb through the glove.
It was a clear day, but cold. The employee was lifting a 2 3/8 screen wire to replace the existing screen when he twisted his back. When the employee realized that he may have hurt his back, he went to sit down and rest to eat lunch and sat on a small nail. Sitting on the nail made him jump and causing additional pain to the back. No lost time from work.
EE sprained his hip while pulling on the hose from the fuel truck that was stuck.
Employee was attempting to remove gravity take-up counterweight. He set the counterweight on the ground with a boom truck. As he was removing the cable the load shifted. When the load shifted it pinned him against the conveyor frame leg.
Injured employee was washing around the plant using a water hose. He said that the pressure increased and he lost his grip, and the hose struck him in the left knee resulting in a contusion.
