Employee went to the company clinic on 8/28/09 and was placed on restricted duty. While exiting the haul truck, the employee stepped off the last rung of the ladder, she planted her foot down and felt her R-knee twist and pop, resulting in an acute sprain to R-knee.

The employee went to the company clinic on 11/19/07 and was placed on restricted duty. The employee states he used a sledge hammer to remove a wedge that was stuck on the crusher. As he attempted to hit the wedge, he missed the target and hit his R-thumb.

Struck against a moving object

The employee went to the company doctor on 6/14/2007 and was placed on restricted duty. While driving through the plant yard and attempting to make a call on a company radio, the employee lost awareness and visibility and drove into a stock pile resulting in a contusion/superficial abrasion to R-lower leg.
