Struck by falling object

On 10/12/2019 EE was taken to the hospital where EE was diagnosed with two fractures in the left hand. While reinstalling the 980H bucket pin, that the EE had placed on the back of the bucket, the pin fell on left hand while EE was lowering the bucket, resulting in two fractures to left hand. *On 5/28/2020 the EE underwent surgery to remove fracture plate, resulting in 1 day lost.

Over-exertion in lifting objects

On 5/6/13 the employee requested med treatment, was taken to the Company clinic & was placed on restricted duty which was accommodated. The employee alleged that while lifting & pushing a water hose into the dust suppression tank, he felt pain in his low back. It is unknown if this alleged event occurred on mine property & it is currently being investigated by the WC TPA.

Struck against stationary object

The employee went to the company clinic on 8/23/2010 and was placed on restricted duty. Employee was using two wrenches to loosen a hydraulic coupling; one wrench slipped off the fitting and employee's L-elbow hit the frame of the haul truck.

The employee was taken to the company clinic on 7/18/08 and was placed on restricted duty. The employee was using a cutting torch and as he re-positioned himself, he turned the torch towards his R-wrist resulting in a 1st - 2nd degree burn.

Conditions contributing to the injury are unknown. On 2/19/08, the employee stated it is unknown how injury occurred but he has experienced right knee pain for approximately four months. The employee was taken to the company clinic on 2/19/08, was diagnosed with tendonitis and was placed on restricted duty.

Employee went to the company clinic on 12/18/07 and was placed on restricted duty. The employee states that while he used a hammer and chisel to adjust skirting on a conveyor, he missed the target and struck his left thumb resulting in a contusion and laceration requiring sutures.

The employee went to see the company doctor on 3/16/2007 and was placed on restricted duty. The employee allegedly strained his back while lifting a steel plate.

EE WENT TO SEE COMPANY DR. ON 3/19/04, STITCHES WERE APPLIED AND HE WAS PLACED ON RESTRICTED DUTY. WHILE REMOVING NUTS AND BOLTS FROM A LOADER BUCKET, EE CUT HIS LEFT INDEX FINGER CAUSING A LACERATION.

EE WENT TO SEE COMPANY DOCTOR ON SEP 8TH 2003 AND WAS PLACED ON RESTEICTED DUTY. EE WAS CHANGING A BEARING AND A METAL ADAPTER WHICH WAS PART OF PORTA-POWER UNIT FLEW OFF AND STRUCK EE ON LEFT WRIST AND LEFT LEG RESULTING IN CONTUSIONS.

EMPLOYEE WENT TO SEE COMPANY PHYSICIAN ON 12-6-02 AND WAS PLACED ON RESTRICTED DUTY. WHILE MOVING A CHAIN SPROCKET FROM FORKLIFT, SPROCKET SLIPPED AND FRACTURED L INDEX FINGER.

EE WENT TO SEE CO DR ON 8/9/02 AND WAS PLACED ON RESTRICTED DUTY. EE CLAIMS THAT OPERATING MOBILE EQUIPMENT OVER THE YEARS, REAGGRAVATED AN OLD BACK INJURY. ALLEGED.

EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 3-18-02, STITCHES WERE APPLIED AND WAS RETURNED TO FULL DUTY. WHEN CHANGING TEETH ON A LOADER, EE HIT TOOTH WITH A HAMMER AND A PIECE OF METAL BROKE OFF AND HIT EE IN THE LIP/ CHIN AREA.
