An individual was trespassing on the mine property when they fell off the highwall. This incident became immediately reportable on 6/6/16.

Employee was at work operating a front-end loader. He radioed the leadman he needed to see him at the shop. The leadman met him, employee stated he did not feel well and had bad heartburn. The leadman called the plant manager and he transported the employee to the hospital for evaluation. The hospital later determined he had a blocked artery and installed a stint.

While attaching J-bolts to the middle deck of a screen, sand from the upper deck fell into ee face getting grit into his left eye.

Employee was installing a return training roller when the conveyor belt slipped causing his hand to get caught between the roller frame and the conveyor.

EE parked the Bulk Truck, went to step out of the truck, fell backwards, truck rolled backwards, left front tire ran over left foot.

Struck by falling object

EE was changing the drill hammer. He broke the hammer loose and began replacing the chuck, when the hammer unit fell, catching his left index finger between the hammer unit and the drill deck.

Struck against stationary object

Employee was attempting to remove gravity take-up counterweight. He set the counterweight on the ground with a boom truck. As he was removing the cable the load shifted. When the load shifted it pinned him against the conveyor frame leg.

EE HAD BEEN INSPECTING A PART OF THE PRIMARY CRUSHER- A BEARING FROM THE HYD. TOGGUE- THE BEARING WAS STANDING ON IT'S END- AS THE EE WAS GETTING OFF THE BACK OF THE BOOM TRUCK THE BEARING FEL L HITTING THE END OF HIS LEFT LITTLE FINGER.

EE WAS MAKING REPAIRS TO RECLAIM CONVEYOR WHEN HE TRIPPED OVER WELDING LEADS AND FELL AND STRUCK LEFT HAND AGAINST CONVEYOR FRAME.

EE WAS PERFORMING HIS NORMAL JOB OF HAUL TRUCK OPERATION. AT APPROX. 3:00PM HE WAS SEEN PARKED IN FRONT OF MUCK PILE, SLUMPED OVER THE STEERING WHEEL. A FELLOW EE CHECKED ON HIM AND FOUND NO R ESPONSE AND NO PULSE. HE CONTACTED PLANT OFFICE & STARTED CPR. 911 WAS NOTIFIED & EMT'S ARRIVED.EE WAS TRANSPORTED TO HOSPITAL & WAS LATER PRONOUNCED DEAD.
