Was replacing a hydraulic hose when it slipped out of their hand and hit them on the nose.

Employee slipped and fell.

Employee was changing screens on the wet shaker when the rail fell on EE's right hand injuring EE's middle finger. Employee went to clinic for treatment for possible fracture. Employee was found to have a BAC of 0.04.

EE was opening the valve of a water tank when EE strained EE's right shoulder.

EE was lowering a front door plate on a wet screen when it slipped out of EE's hands and hit right shin.

Employee was lifting a steel plate and twisted their back.

Employee was loading a cutting edge up onto the back of a truck and it cut their pinky finger.

Employee was loosening the bolt for a turn buckle of a motor to release the tension to replace the v-belt. When loosening the bolt, the turn buckle fell from its position crushing the tip of employee's left hand pinky finger crushing it between the turn buckle and the platform.

EE lifted the hood to replace a welding rod and a foreign object landed in EE's right eye.

EE hit hand on a piece of angle iron cutting left hand.

Employee hit hand/wrist on a steel plate while working on a cone crusher.

EE was climbing down a ladder after working on a shaker screen when EE felt pain in EE's left bicep.

EE says lower back started to hurt while shoveling

While getting ready to cut a piece of angle iron (10' X 3") the angle iron slipped off the vice and struck left pinky finger.

While removing cutting edges off of Loader WL-65, cut left middle finger on sharp bolt. Received stitches from doctor.

Injured right forearm while shoveling.

While working to replace the liner on a chute. Employee was adjusting the chute when a sharp edge on the chute made contact with his right forearm, cutting him.

While repairing crusher, the chute that feeds the fine crusher fell onto ee hand, smashing his left ring finger. Crushing injury to left ring finger.

While changing a screen on rock shaker, miner rolled from one side to the other while lying down on his back. While rolling miner twisted and felt an extreme pain in his lower back.

Employee was setting down the screen that he was carrying. In doing this he twisted his back and felt a sharp pain. He did not trip or fall.

While working on a loader EE left a Portable power cylinder unattended. He bent over to fill hydraulic fluid on the same unit. The portable power cylinder came loose and fell about 2.5 feet striking him on the back of his head and neck. EE did have his hard hat on. Blunt head trauma and scalp wound.

On 4-27-09 EE says that he hurt his lower back while moving a metal rack, he did not report the injury at that time. On 4-28-09 the pain returned while operating his truck. Doctor's diagnisis: Lower back strain/spasm.

Injured back while shoveling, reported injury on 2/10/09. Doctor diagnosis: Lower back strain.

While shoveling, he strained his left shoulder.

coming down from loader he hit the bent step and hit his lower right leg.

Shoveling material while under conveyor while bent over. Pulled back out while reaching for broom.

Stepped on uneven ground and twisted his ankle.

Working at the rock plant. When a rock came down and hit him on the top of his neck.

Pulling rocks from cone crusher, a piece of metal went into his thumb. Employee was not wearing gloves. *Returned to work on 11/27/06. Off for Thanksgiving 11/23/06 thru 11/26/06. Off on 12/1/06, still off.

EE was lifting tooth into position on loader bucket to be welded. EE said he lifted with his back and not his legs. Hurt his back.

Working in cone crusher area, inspection plate came loose and cut inside of his leg, while on the platform
