Struck against stationary object

While working to replace the liner on a chute. Employee was adjusting the chute when a sharp edge on the chute made contact with his right forearm, cutting him.

While changing a screen on rock shaker, miner rolled from one side to the other while lying down on his back. While rolling miner twisted and felt an extreme pain in his lower back.

Miner opened hood of water truck, as he stepped down onto his right foot he rolled his ankle. All of his weight went to the side of his ankle, resulting in an ankle sprain.

While working on a loader EE left a Portable power cylinder unattended. He bent over to fill hydraulic fluid on the same unit. The portable power cylinder came loose and fell about 2.5 feet striking him on the back of his head and neck. EE did have his hard hat on. Blunt head trauma and scalp wound.

Employee was zipp tying a loose hose to water truck. While cutting off the excess hose he cut towards his body instead of away from his body and he cut the palm of his left hand.

Walking out of tunnel with a hammer in one hand and a rod in the other hand, he slipped on loose rocks and fell down.

DISMANTLE HYDRAULIC HOSE FROM U107 LOADER. OPEN VALVE TO LET PRESSURE OUT. ALSO RELEASED CAP TO LET MORE PRESSURE OUT. OPENED TOO MUCH AND CAP FLEW UP AND HIT HIM IN THE HEAD.

EE & ANOTHER EE WERE DISMOUNTING A FRAME THE FRAME WAS ON A BENCH 3 FT TALL. PART OF THE FRAMEM BROKELOOSE & FELL OFF THE BENCH ONTO EE'S RIGHT FOOT. HE DID NOT SEE IF COMMING AT THE TIME SO H E DID NOT MOVE OUT OF THE WAY OF FALLING PART
