Showing 50 of 908While installing lagging on the head pulley, the pulley was being rotated using the drive belt. During rotation, employee's fingers were positioned too far back on the drive belt and motor pulley and two fingers became pinched between the drive belt and motor pulley, the tension prevented immediate release of the fingers. Drive belt was cut.
Employee felt a pop in left knee when mounting water truck. Employee has had a previous knee injury. Not determined until May 6th that this was a new injury and not a reoccurrence.
EE middle left finger fractured when struck between bucket tooth and shank of WA800 loader

Carolina Cat technician was entering through an interior door when the door closed on EE's hand.
Employee lifted improperly and twisted while lifting; metal cover that was lifted weighs 35lbs roughly. Due to positioning, the tech needed to remove the cover to access the filter in the compartment of the 980G. Resulted in back strain.
Employee was struck by foreign object in the eye during maintenance. Foreign object was removed from eye.
Employee smashed finger removing rock from crusher
The employee was removing metal from a conveyor, and while going to dispose of it, the employee lost control of the metal object and pinched their hand between the metal and the handrail. This resulted in a laceration and a broken finger.
Employee sprained EE's left knee, stepping up and over an air slide. EE's knee was treated as a first aid until 9/9/2025 when a visit to an orthopedic doctor resulted in physical therapy being prescribed to help strengthen the knee.
EE was injured while washing out a dust bin when material buildup sloughed off and fell onto EE's left arm pushing it downward onto the steel opening causing a laceration to the bottom of the wrist.
While removing an expanded metal guard bent around a motor shaft, the guard flexed back to a straight position after the bolt was removed striking and cutting the employee's nose. employee received 5 stitches.
During screen maintenance the sharp edge of a metal panel caused an open fracture to the right index finger of the employee.

Trying to dislodge a rock 17' down a bore hole with loading poles. The rock was dislodged and the poles rebounded and hit employee under the right eyebrow. Cutting EE with a 1' cut. EE was wearing safety glasses. EE was taken to the emergency care unit. for stitches.
Employee's hand was pinched between a plate of steel and a block of wood resulting in a laceration approximately 2.5 cm that required stitches.

A Miner was injured while separating the forks on a Skid Steer that resulted in a sprain to Miner's left index finger.
Employee received a laceration from a screwdriver while reaching into a toolbox, The laceration required stitches.
Employee was leaning across the drag line cable electrical box to verify there was no energy prior to performing work The door closed, hitting the employee. Incident date: 1/25/25. Became reportable when employee received restrictions on 2/7/25
Chemical exposure to the eyes and face
EE pulled a locker on top of themselves while attempting to retrieve gloves.
150 ton track press was being used to press track pins out of a 390. Press back plate had to be removed first. 2 cranes were used to support track press about waist high. Tech was positioned at back plate. Back plate was removed by sliding off guide rods manually by tech. When back plate came off guide rods, the press section cantilevered up striking tech's lip causing laceration.
Sprained wrist while installing roof glass on machinery.
EE was washing out in the surge tunnel. While pulling the hose to reposition, it got caught around a pile of dirt and pulled EE's left arm. Injury date occurred on 10/16/2024 but injury did not become reportable until 1/13/2025.
Employee reported pain with shoulder after adjusting a gate at the secondary plant. It was determined on 11/19/2024 by an Orthopedic Doctor that the employee needed physical therapy.
Employee was shoveling and felt a strain in their shoulder that was treated as first-aid but employee was later given restrictions by a doctor on October 23rd.
Employee was lifting a wooden block and injured their bicep. Injury date occurred 9/23/24 but injury did not become reportable until 10/30/24.
EE injured hand while removing guard.
Employee started feeling knee discomfort and unstableness while pulling cords during equipment teardown. Employee was prescribed physical therapy for string band strain. Injury was deemed work-related on 10/01/2024.
Employee lifting screen and strained left bicep.
The employee was changing the spool on a weed eater, and their left thumb made contact with the string cutter, causing a laceration.
Operators machine was down. Mechanic was working on it. Operator rested right hand on stick while waiting. EE slid hand down and caught hand on a metal burr, cutting EE's palm.
EE seen water coming out of sandscrew and pointed at it and was unaware there was a hole in sandscrew and it cut the tip of right index finger almost off. Was taken to hospital and was stitched up and returned back to work.
Employee was changing return rollers when the employee felt lower back pain. Physical Therapy and restrictions were prescribed by the clinic. Injury became reportable on 07/23/2024.
Employee got cement dust in their eye causing small abrasion.
The miner was performing maintenance on a crusher; in the process, the miner came into contact with a sharp edge. This resulted in a laceration and partial amputation on the tip of the left ring finger.
Repairing crusher and got hand caught between spider and mainframe. This cause miner to have a crushing action on their right hand.
EE was closing observation door on crusher and slammed it on hand causing a fracture.
Employee strained EE's back while cleaning a bin out
Loading a fuel tank on a trailer. Ladder fell down injuring right index finger.
EE was moving a large rock, rock slipped, EE moved to grab it and EE's thumb was smashed between the rock and loader frame. EE received a fracture to tip of right thumb and received 7 stitches.
Employee was shoveling material into a bucket and strained lower back
Operator was helping pull a cable that was caught in the tracks and while EE was pulling, EE turned and pulled a muscle in EE's back.
Employee was shoveling debris off a catwalk and felt pain in lower back.
Employee was pulling knife gate out from chute when a finger was pinched causing a laceration.
Contractor was changing V-belts on #4 conveyor. EE was using index finger on their right hand and was walking the belt around the sheave. EE thinks the belt hung on the back stop, because it broke loose and EE's left wrist was run between the belts and sheave 2 times before EE could get it free.
