Employee was assisting in repair of conveyor idler. Another employee tipped idler from horizontal to vertical position. It over traveled on to employees right hand and smashed hand.

Miner was installing wheel hub on truck. Miner pinched small finger on left hand while attempting to align valve stem. Required 3 stitches on small finger of left hand.

Employee was standing on ground washing out material buildup when employee turned hitting head on steel structure with forehead. Causing a laceration above nose resulting in three stitches.

Finger caught/pinched between V Belt and pulley groove while manually rotating pulley.

Employee was attempting to roll rock and adjacent rock dislodged and pinched right 4th finger.

While adjusting jaw crusher, toggle plate came loose and moved pinching/crushing tip of right little finger.

Mechanic was repairing hoist cylinder. Attempting to hook cable hoist sheave, slipped away from grasp and hit right foot causing contusion. Went for x-ray (negative).

While waiting for truck to load from overhead bin, employee was standing behind truck when a small rock fell and bounced off tailgate, hit front tooth breaking tooth.

Employee was helping load shot. Carrying buckets of stone to use as stemming. He felt pull to groin area. Thought he was going to be Ok but could not get out of bed next morning.

While changing conveyor roller, employee #1 dropped roller, striking employee #2 in back of head and neck causing laceration requiring stitches.

Miner went to turn flat rock on conveyor and got Right Hand (glove only) wedged under rock, when miner went to pull hand free, he bruised/ strained wrist bone on that hand. Conditions were dry that day.

Changing leaf spring on truck, his finger was pinched between spring and frame of truck. Laceration to right little finger requiring one suture.

On 4-11-07 employee pulled on bin gate that was slightly frozen straining right shoulder. Treated with physical therapy with no lost time until worker's compensation decision that surgery was needed. Surgery on 08-16-2007.

While lifting a wedged screen plate, the screen came loose causing a shift and smashing left index finger between screen and screen base resulting in a laceration and fracture of the tip of left index finger.

Lifting 2" water pump to put in backhoe bucket, strained to lower back.

EE WAS LIFTING PIECE OF STEEL & DROPPED IT, CUTTING HIS RIGHT RING FINGER. EE RECEIVED 6 STITCHES.

EE WAS CLIMBING LADDER TO HIS WORK AREA AND STRUCK HIS ARM ON A PIECE OF STEEL CUTTING IT.

EMPLOYEE WAS CLEANING SCREEN AND HE NOTICED AS HE CONTINUED OT WORK, HIS BACK STIFFENING UP.

EE WAS INSTALLING BELT AND WHEN EXITING CONVEYOR HE SNAGGED THE EDGE OF THE STEEL FRAME CUTTING HIS HAND.
