Employee was cleaning catwalk and turned sending intense pain to EE's foot/ankle area making it hard to move and walk. Employee has had foot discomfort on left foot for a couple weeks and never told anyone. Employee was seen in ER, was given an injection for pain, and taken out of work for 1 day. EE was diagnosed with tendonitis.

Employee was shoveling off the primary belt at the crusher and when EE went through the railing to get onto the belt, EE felt a burning sensation in EE's back. This injury was originally a first aid incident, but on 6/17/25 the employee was taken out of work.

Miner was assisting in changing a sheave on a gearbox with the shaft pointed vertically. The miner held the sheave from rotating using their hands while another miner attempted to free the bushing. The sheave unexpectedly broke loose from the busing and shaft and fell down to the gearbox, pinching the miner's finger between the sheave and the gearbox housing. Left, middle finger.

Employee got EE's finger caught between a gate lever and handrail. Employee lacerated EE's finger and received 8 stitches. Employee was returned to regular duty.

Employee arrived to work at normal time (6am) and performed normal duties (sitting in crusher shack operating jaw). Around 10 am employee starting having knee pain. Employee stated that EE does not know what caused the pain. Employee went to urgent care and was given prescription medication and taken out of work for 2 days.

Injured worker was team lifting a 60lb sheave when the sheave slipped out of coworker's hands and dropped on IW's right hand. When the sheave was dropped it pinched IW's right hand between the sheave and some steel guarding in the area that resulted in IW receiving 4 stitches to the right ring finger. Injured worker was placed on light duty for 2 weeks.

Employee was assisting with a screen change out during wet, rainy conditions during the shift. Employee went home, showered, went to bed, and woke up around 3am with pain in left eye and went to the ER. ER doc removed a small foreign body from EE's eye. Employee followed up with eye doc who returned EE to work.

While tramming on ledge 2 after finishing boulder drilling, the drill got stuck. EE tried to tram and the drill went up on a rock giving a tipping feeling. EE moved back off the rock and got out of the cab to move it out of the way of the track. EE lifted and rolled the rock. It moved unexpectedly and pinched EE's left index finger cutting it between the first two joints.

While installing a chute to a steel frame, Employee improperly placed hand in a pinch point area causing a laceration to index and fiddle finger on left hand, requiring sutures. Employee returned to work with no restrictions, No Lost days.

Employee was removing a scraper from a belt when their left thumb became pinched between the scraper and the plate that held it in place. The weight of the scraper pinched their thumb. Left thumb is fractured. Employee has temporary limited use of their left hand.

Employee was performing clean up duty at 18 plant using a shovel. It was sand, rock, clay material it was wet. EE put the shovel in the ground and prying back on it to loosen it was when EE felt the pain in EE's left shoulder.

While throwing a rock into the jaw to help get a big rock out of the crusher employee's hand was smashed between the rock and the I beam in the front of the opening. This caused the skin to open on left thumb.

Employee was moving a 10 foot piece of 2X4 box beam and felt something snap in EE's left arm. Employee was evaluated in the ER and referred to an orthopedic for further evaluation. Orthopedic diagnosed injury as ruptured tendon and scheduled surgery for 8/28/2018.

An employee was removing chute door. When they removed last bolt, door fell and rolled over the foot of another employee that was observing the work. Employee worked the rest of the day then stated the foot hurt. Employee was sent to clinic and diagnosed with a bruised great toe. Employee was placed on light duty, and was released to full duty on 10/24/2017.

Me and ee worked together to move a drum of oil into the shed we did this by tipping the drum over on edge to roll it into the shed .EE called me the next day to say that ee thought ee pulled a muscle . At that point I took ee to the medical facility to have it checked out they said that it was a back muscle sprain and released back to full duty the day 05/08/2017

Employee was assisting in reinstalling the top spring plate on a Deister screen. While doing so, employee's hand slipped off the plate as the plate was seated and got left pinky finger pinched between the plate gusset and screen gusset. Employee received 7 stitches to close the injury.

Employee was lifting an oxygen cylinder. Upon lifting, the employee felt a pop in back and began making complaints of back pain. The employee decided to wait until 9/2/16 to ask to be taken to the doctor. The physician placed the ee on light duty with a back strain as the diagnosis at this time. EE is still on light duty and has a follow up appointment on 9/15/16

The miner was working on reversing the toggle plates in an Austin-Western jaw crusher. Miner was in the rock box removing a cheek plate when the stationary toggle slid approximately ten inches, and during its travel, it came in contact with the miner's hard hat. As a result, the miner received sprained muscles in back and neck.

Loader operator returned to his loader after lunch and turned on the air conditioner. When the air conditioner fan turned on, it blew out some dust and small debris into his face. Employee flushed out his eye and returned to work. The next day his eye was still irritated so he went to the doctor and was treated for a scratched cornea and prescribed antibiotic eyedrops.

The employee was helping R&I screens in the sand shaker. It was then that the sand debris was dislodged and became airborne. The sand debris got lodged in the left eye. The employee then rubbed the left which caused a corneal abrasion. He was wearing eye protection.

On 6/27/14 while removing gears from feeder shaft, employee pinched right wrist. Employee continued work without issues until 7/2/14. While washing screen decks on 7/2/14 with water hose, employee felt "pop" in right wrist. Employee was seen by doctor on 7/3/14 for first time.
