Fall down stairs

After the shift employee had exited the Scale House and fell down stairs while walking to EE's car

Struck by rolling or sliding object

An employee was removing chute door. When they removed last bolt, door fell and rolled over the foot of another employee that was observing the work. Employee worked the rest of the day then stated the foot hurt. Employee was sent to clinic and diagnosed with a bruised great toe. Employee was placed on light duty, and was released to full duty on 10/24/2017.

Employee was using a utility knife to cut a piece of belt material to replace a skirt on hopper feeder #6. The utility knife slipped causing a small laceration to employees left leg. Employee was taken to the clinic and received 4 stitches then returned to full duty on same day. EE received a full discharge from care on 10/25/17.

Fall from machine

While installing a classifier on the wet site of plant, employee was welding on a new sand chute. At break time, the employee was descending the chute and foot slipped causing EE to fall on back. Employee seemed okay but was taken to the clinic to be checked out. Employee was put on light duty 10/16 and returned to full duty 10/18.

Struck by flying object

While welding, acetylene gas built up in the closed cabinet that housed the welding hose reel. The acetylene gas in the closed cabinet ignited and blew the door off the hinges, striking the employee in the left calf, causing a laceration. UPDATE 12/4 - Fractured bone in knees. Surgery on 12/4 to USC LA County hospital

Fall from machine

Dragline Operator slipped while washing windshield and fell less than 4 feet to the ground. Operator was taken to the clinic and diagnosed with a bruise/contusion to the posterior, back. He was placed on light duty for a week.

Flash burns (welding)

Employee was welding during his shift and later started feeling discomfort in his eyes. He reported discomfort on Monday 8/24/2015 and was sent to the clinic. He was returned to regular duty with no follow-up appointments. However he was given a prescription eye drop.

Over-exertion in lifting objects

Employee is the landscaper in charge of trimming bushes and mowing grass. He reported pain in his back from loading brush and shrub clippings into his trailer. He was sent to the clinic where he received non-prescription Motrin and was placed on light duty for 8 days.

Accident type, without injuries

Workers were removing bolts to repair a crack in the housing of shacker below a bearing. After cutting a bolt with oxygen/acetylene torch the hot bolt fell inside the shacker box and down into the discharge chute. The hot material caught in the chute liner and after 30 to 35 minutes caused the chute liner to burn. The fire burned approximately 30 min.

Struck by flying object

The employee was washing down the deck of the dredge with a water hose. As he through the hose down the valve on the nozzle hit the deck which caused the valve to open and the hose to whip around and the nozzle struck the employee's foot and caused a contusion.

A tear was noticed on the belt of a conveyor. The conveyor was locked out. Employee used a pocket knife to cut the belt and to remove the torn piece. While holding the torn piece with his left and cutting with his right, he completed the cut. The momentum of his hand continued causing the knife to cut the top of his left hand. This required stitches.

An employee under his own independent action stuck his hand into the return side of the belt to get a rock that was hitting an idler roller. The idler roller sucked his hand in and he broke his right forearm. The pull cord was activated and the belt stopped. He called for help and was removed from the pinch point.

Employee failed to report the injury to employer until July 26, 2010. Employee was on the third deck of the mill performing clean up operations. He had over filled the wheel barrow causing materials to fall on the deck. When he went to move the wheel barrow he slipped on the spilled material that he had dropped. He did a split and got up and continued to work.

Our employee was climbing down the fixed ladder on the Cat 980H loader he stepped on to uneven ground in Pit 4 by the dust machine and sprained his left ankle.

Using pry-bar to clean materials and smashed his finger between tunnel wall and pry bar. The pry bar slipped.

Struck against a moving object

Employee was on the road in Pit #3. While turning the corner near Conveyors 13 & 14, the truck struck a pile of silt material on the right side of the road near the conveyor. The driver was distracted by passenger and was going too fast for the conditions.

Struck by falling object

Two employees were reattaching a cyclone gate to an existing fence. Other ee advised ee that he was going to remove the come-along from the gate and asked him to hold the gate in position. EE let go of the gate to remove his hard hat to wipe his brow. This allowed the gate to fall and to strike him on the left side of the head.

Employee was cleaning the windows on a CAT D-9 Dozer when he lost his footing and fell on his feet. He felled to maintain three-points of contact while cleaning the window.When he landed on his feet. He fractured his left ankle.

Employee and co-workers were working on rake cables on dredge. Employees failed to properly lock-out equipment. Employee was caught between rake cable and wench, when the wench energized, causing extreme bruising to right forearm.

Employee, EE, was trying to brake a nut loose on a feeder table with a large ratchet. When the nut finally broke loose, the force from the ratchet caused his right pinky finger to get smashed between the bottom of the motor feeder table.

Employee, was working on the pit line, tearing down a head-pulley. Foreman, Other EE and Another EE were working with Him at the time. HE placed a pipe wrench on the bearing to break it loose, when the belt was being pulled. The wrench slipped from EE's hand hitting him in the forehead, causing him to get eight stiches.

Employee,He was checking the pit line, when he slipped on the concrete walkway. At the time of the incident, it was raining rather heavy and the ground was slippery. He fell on his back when he slipped.
