Employee reported on 7/14/2021 that EE's back was hurting and had been bothering EE since the week of 6/28/2021, when EE was operating various pieces of equipment. Employee stated EE's back hurt from climbing in and out of equipment. Employee received medical treatment on 7/14/2021 was released to full duty and again on 7/27/2021 when EE was given work restrictions.

While an IMTR employee was unloading a drill steel from a delivery truck, the forks of the lull hung up on a cross member, when it released it pushed the drill steel onto the Fred Weber Inc. truck driver's foot.
The employee was shoveling and experienced groin pain. It was initially diagnosed as a personal health condition. A medical specialist later determined that the condition may have been caused by heavy lifting or straining.
Employee was working under the crusher chipping welding slag and fines fell from above and went behind safety glasses into his right eye.

Employee was working on a Pantera 900 drill and he was removing a cover off of the side of the boom, when he bent to set it down he felt a sharp pain in his low back.
Employee was loading scrap steel into loader bucket when he threw a large piece into bucket causing it to bounce out and strike him on the left side of his head. His hearing aid broke inside his ear canal. Employee went to the ER to remove the foreign objects from his ear canal.
