Injury said that he was trying to exit the scoop & his foot got stuck under the starting box. He twisted his knee trying to get out. He said if the foreman would take it easy on him, he wouldn't go outside and fill out an acc. rpt or go home and seek medical attention. There were two witness besides the foreman to this statement.

The miner was trying to access the feeder. He was assisted in taking the guard off the pick breaker. As both miners were raising the guard, one miner screamed with pain.

Employee was cleaning up debris on the bottom floor. A ceramic lined chute was being removed 3 floors above. A 1/2" X 4" X 6" pc of ceramic fell from the chute into the machinery well, bouncing from side to side. The employee app 30' away from the barricaded area was struck on the rt ring finger.
