Laceration to left leg requiring stitches. While attempting to disconnect bucket from skid steer, the bucket slipped off the bucket moment and contacted left leg resulting in a laceration.

Showing all 29Injured employee assisted in maintenance of log washer. Employee was standing on walkway holding journal of log washer. As EE was doing this another employee was in the tub of the log washer removing the bolt. As the bolt was removed the journal became unstable, pinching three fingers of the employees left hand.
Employee was changing a roller on a field conveyor. Employee was holding one side when it suddenly fell pinching the middle finger of right hand between the roller and conveyor frame. This resulted in a contusion to right hand and also injured the fingernail of the middle finger of the right hand .
EE was pulling off a Roller from the conveyor to replace, as EE grabbed it to remove as EE and helper pulled it EE's finger was caught in roller, it crushed and cut middle right finger
Employee states EE and another employee were carrying a pipe. Each employee was positioned on an end of the pipe. one employee suddenly dropped EE's end resulting in EE's right shoulder being strained.
Employee was installing paddles on the log washer and felt back pain. EE previously reported that on 10/13/16 EE had felt similar pain when EE was washing the shaker platform while pulling on a 2 1/2 inch water hose. EE requested to seek medical treatment on 10/31/16.
Employee states he was removing a cutting edge from a bucket. He was using an impact wrench to remove a bolt that secured the cutting edge on the bucket. As he was removing this, the nut began to spin, and he grabbed it to secure it from moving. When he did this it caused a laceration of the index finger of the right hand.
Employee was training employees on how to train a conveyor. He states he was using a hammer to tap the rollers for adjustment. He completed this and walked to his truck and as he began to open his truck door he felt back pain.
Employee was involved in moving a 55 gallon drum from a Bobcat onto a pallet. As he was standing the 55 gallon drum up his middle finger on his left hand was caught between that drum and another drum that was on the pallet.
Employee states he was pulling on a suction hose of a water pump that was stuck in the mud and injured his lower back.
Employee states he was installing flashing on a conveyor belt to prevent spillage. As he was pulling on the flashing to properly place it his right hand went across a piece of jagged metal resulting in a laceration to his index finger on his right hand.

Employee states he was removing a turbo from a Caterpillar generator. He states he had a wrench in his left hand and reached around the exhaust with his right hand. He was trying to put the wrench in his right hand and as he was doing this a metal gasket that joins the exhaust to manifold cut his right forearm.
Employee states he was securing crane to lowboy trailer. As he was throwing chain through the hook of the crane to secure felt a pain in his right shoulder.
Employee states he had positioned a 6X10 sheet of metal in a vertical position in a hopper to weld. He had placed (3) 2x2's in a horizontal position with one end against the sheet of metal and the other against the wall. As he was climbing out of the hopper on a ladder sheet of metal came unsecured and pinned him against the ladder.
Employee states he was climbing down the steps of the trackhoe and felt his left shoulder pop.

Employee was putting an oxygen bottle on the service truck and dropped the bottle on his right foot.The employee received a contusion to his right foot.
EE WAS TURNING PULLEY ON SAND PUMP TO START INPELLER ONTO THE PUMP AND CAUGHT HIS FINGERS OF THE RIGHT HAND BETWEEN THE PULLEY AND BELTS.
DIGGING OUT TRACKS ON DOZER HE FELT BURNING SENSATION IN GROIN AREA.
WAS ENDING SHIFT AND GETTING DOWN OFF SCRAPER. STRAINED HIS BACK AS HE WAS GETTING OFF MACHINE.
