Employee dropped a cone crusher liner on foot
Employee was lifting a hydraulic cylinder with another employee and it slipped, crushing the employee's right middle finger.
Employee was pulling the hydraulic hose from the crusher when they slipped and hit hip on the edge of the crusher.
Employee was lengthening a chain to reinstall feeder plate. When the chain was slackened the plate moved causing the chain to tighten and injure employee's thumb.
Employee was working in shop cutting 3" angle iron piece. Employee was sliding the unused piece back onto rack when the piece of steel fell from the roller onto employee's foot. Employee received two fractured bones in foot.
Employee was placing new flashing rubber on conveyor when the clamp bar slid down and pinched finger.
Employee was lifting a plate on crusher with coworkers when it became stuck. The plate then fell onto employee's finger crushing it. Employee received a fractured finger.
The employee was sliding a screen into position on the screen deck of the crusher when his thumb was pinched between the screen and screen deck. He was wearing gloves. The employee was taken to a local medical facility, treated for a fractured thumb and released with restrictions.
EE was fueling up the crusher when a piece of dust flew under his safety glasses.
EE was working on the jaw crusher in Prospect. He was putting a cover from the crusher down when a bolt on the cover hit the index finger on his right hand. The EE pulled his hand back towards him in pain which caused the cover to take the skin off of his finger from the middle of the finger to the tip.
Employee states he was walking under the catwalk to the stairs of Tower #1. Debris fell from above, landed on top of his hard hat, he looked up & more debris fell into his right eye. He was wearing a hard hat and safety glasses at the time, the debris went into the space between the top of his safety glasses and the brim of his hard hat. Determined recordable on 8/18/2009.
Employee was changing cutting edge on loader, with assistance of a co-worker, he was removing the bolts. The employee had a hammer on bottom side of cutting edge holding the bolt up, the co-worker had an impact wrench on top to remove nut. When the co-worker pushed down on wrench, the hammer slipped. When hammer moved, employee's finger came in contact with the spinning bolt.
Employee states he was removing the extension pre-strike off on a paver. He removed the bolts getting ready to remove the extension pre-strike off, when it slid down and catching his middle and ring finder on his right hand against the shop floor.
Employee strained left shoulder and upper arm accessing water truck cab. Restricted duty to no repetitve lifting over 20 lbs, no pushing/pulling over 20 labs force, no reaching over shoulder, prescribed celebrex and vicoden.
Employee was pulling a come-a-long to get a wedged rock out of the jaw of the crusher when he felt a pain in both his shoulders
Employee sustained a lower back strain as he was attempting to lift and slide an oxygen tank into back of the yard truck.

EE INJURED HIS LOWER BACK MUSCLE STRAIN WHILE MOVING AROUND THE JAW CRUSHER REMOVING THE TOGGLE THE EE CRUSHER FOREPERSON HAD TO MVOE AND GET INTO AWKWARD POSITIONS DURING THIS MAINTENANCE WOR K. U
MECHANIC HAD JUST COMPLETED CHANGING A FUEL FILTER ON THE BIN TRUCK. WORK WAS PERFORMED IN THE EQUIPMENT SHOP LOCATED AT THE MINE SITE. WHILE CLOSING THE HOOD, MECHANICS RT. THUMB WAS CAUGHT BETWEEN THE HOOD & THE FRAME RESULTING IN A LACERATION REQUIRING 6 STITCHES. WHILE LOWERING HOOD, MECHANIC WAS ASSISTED BY ANOTHER EE(BIN TRUCK DRIVER) ON THE OPPOSITE SIDE OF VEHICLE.
EMPLOYEE WAS GETTING INTO ROLL AWAY BOX TO REMOVE STONE AND HIT HIS HIP AGAINST A PIECE OF METAL.
EMPLOYEE WIPED EYE WITH DIRTY HAND CAUSING A PIECE OF METAL TO BECOME EMBEDDED IN EYE.
EMPLOYEE WAS LIFTING AN ELECTRICAL CONTROL CABINET TO LOAD IN PICKUP WHEN HE STRAINED HIS WRIST.
WHILE EMPLOYEE ATTEMPTED TO REMOVE AN OXYGEN TANK FOR HIS TORCH SET OUT OF THE BACK OF HIS PICK-UP TRUCK, HE FELT A PULL IN HIS BACK AND THEN THE PAIN SLOWLY GOT WORSE AS THE DAY WENT ON.
WHILE CHECKING ON A BLOWN HYDRAULIC LINE UNDERNEATH A LOADER, A DROP OF THE HOT OIL LANDED IN THE EMPLOYEE'S RIGHT EYE. THE HOT OIL BURNED HIS EYE BEFORE HE WAS ABLE TO START FLUSHING IT OUT.
EMPLOYEE WAS WORKING DOING HEAVY MANUAL LABOR WHILE REBUILDING CONE CRUSHER, LIFTING & CARRYING ITEMS.
WHILE EXITING OFF THE GYRO LADDER, EE ROLLED HISANKLE CAUSING A FRACTURE TO THE BONE IN THE SIDEOF HIS FOOT.
EE CLAIMS THAT AFTER RETURN FROM WINTER LAY-OFF THAT HE HERNIATED A DISC IN HIS BACK WHILE PUTTING A TOGGLE SEAT IN THE JAW.
WHILE ATTEMPTING TO REMOVE A STEEL PLATE FROM CRUSHER THE WEDGE SLIPPED OUT FROM BEHIND THE STEEL PLATE CAUSING THE PLATE TO MOVE INWARD WHERETHE EE FINGER WAS AT THE TIME
AFTER EE FINISHED CLEANING UP AROUND CRUSHER, WENT HOME THAT EVENING 5/1, HE NOTICED THAT HIS RIGHT EYE WAS IRRITATED. EE DID NOT REPORT THIS UNTIL 5/4 AND HAD MEDICAL (FIRST AID) TREATMENT ON 5/4/00.
