Employee was working in shop cutting 3"" angle iron piece. Employee was sliding the unused piece back onto rack when the piece of steel fell from the roller onto employee's foot. Employee received two fractured bones in foot.

Employee was changing cutting edge on loader, with assistance of a co-worker, he was removing the bolts. The employee had a hammer on bottom side of cutting edge holding the bolt up, the co-worker had an impact wrench on top to remove nut. When the co-worker pushed down on wrench, the hammer slipped. When hammer moved, employee's finger came in contact with the spinning bolt.
