Employee was attempting to open the service truck door. The door latch was stuck and the employee forcefully pulled on the door causing it to spring open. After the door sprung opened the employee noticed some soreness in their wrist.

Employees were hanging a 2" angle iron 107" long above the v-belts on the 7' crusher. They set the angle iron in place for welding and SE's end fell and it pinched EE's hand between the angle iron and the beam, cutting through EE's glove. EE and SE were wearing Radnor A6 cut resistent gloves and the angle iron pierced through the glove between the thumb and forefinger.

EE reports was assisting install a tail (bin) pulley out of a manlift along with a second employee. The second employee was using a come-along to lift the pulley up so they could get it in position when the EE decided to assist by pushing the pulley. It weighs approx. 100-150 lbs. As the EE began to push, EE felt pain in lower back. EE started PT on 11/14/2024.

EE states they were washing out a bucket on 988 K loader. EE was dragging the hose but, the hose got hung up on rocks. They turned and stepped back onto a pallet. Their foot slipped off the pallet and they fell backwards onto their knee.

The injured employee was picking up a 1'x2' ceramic embedded rubber panel and when EE went to set it down to readjust their grip, the panel slipped. EE went to grab for the panel and it pinched EE's left index finger between the panel and the toe board. EE was wearing leather impact and cut resistant gloves at the time of the injury.

While pulling on a piece of replacement rubber belt, the employee's foot was struck by the wooden piece connected to the rubber. The wood hit their foot, bruising it and caused them to step backwards awkwardly. Self-care was advised by care coordinator, but symptoms didn't resolve. On 4/19/24, employee was taken for additional treatment and received work restrictions.

Tip of employee's right pinky finger was partially amputated after being caught between a v-belt and motor sheave connected to the jaw crusher flywheel. The flywheel counterweight was not resting in the down position after lockout. When the employee checked the tension on the v-belt, it caused the flywheel and belt to move and caught EE's finger in the sheave.

On 8/18/22 The employee was collecting the shank and tooth (approx. 60 lbs.) from the loader that had been collected at the belt magnet. As they pulled the shank off the belt, which was shoulder height, their fingers slipped out of the tooth and the shank fell causing the employee to "catch" the weight with their left arm. This caused muscle strain in their left shoulder.

Employee had a first aid case in Feb. 2022. Reported further aggravation of this and a pre-existing condition on May 17 2022. Medical evaluations in June 2022 revealed possible aggravation to pre-existing condition in knee. Surgery required and performed on 7/11/22, releasing miner to work on 7/12/22 with restrictions.

EE was clearing rocks from a plate that had fallen onto the crusher. As they removed a rock to clear the shackle hole, a 8" rock rolled down from the pile and struck the EE on the right ring finger. This caused a fracture to the tip of the ring finger. EE was seen and released to full duty at the ER. EE was given a splint. No other treatment was ordered.

Miner was installing channel to structure. Channel was supported on fork lift forks and secured w/a chain fall. Channel slipped off forks pinching the miners right hand.

Employee was washing with a 1 inch water hose equipped with a ball valve fitting at the end. During the process of cleaning, EE had laid the hose on the catwalk. Pulling the hose, the ball valve caught on a toe plate, turned the water on, causing the hose to whip under pressure, striking the employee on the left eyebrow.

Employee was removing a clevice from a lifting cable after moving a crusher head. When the clevice pin was removed, tension on the cable caused the clevice to spin, striking the employee on the chin. The employee suffered a laceration to chin requiring sutures to close the wound.

Employee was helping to change a return roller on a conveyor, holding the roller can in position, ee was losing grip. To get a better grip on the can the employee took glove off and placed it back on to the roller can. When the roller seated in the frame, the tip of left little finger got pinched between the roller can and the bracket resulting in 6 stitches.

IE was pouring hydraulic fluid into rock breaker at the jaw when some fluid spilled/splashed up and into IE's face. IE was wearing safety glasses but fluid ran down IE's face and into IE's R eye. Another EE was in the area but not a witness to the accident & helped IE flush out eye at eye wash station. IE was taken to local ER and treated & released to go back to work that night.

The lid on a grease drum had a metal tab sticking out on one side. Employee was changing out grease barrel when EE's left elbow contacted the metal tab. The result was a laceration requiring six stitches to close. No lost time and no restricted duty.

During the process of lifting and pulling a 3" poly discharge hose from a position in the man lift to remove water from the south side of the creek in a sump area the employee through he experienced a potential back related problem. He thought that it might get better overnight but was still experiencing a strain the following morning.

Repair of RS Conv 99% complete w/severe storm minutes away. 2 riggers supplied by Edwards, Inc on catwalk loading tools on deck of hyd. manlift supplied by Hanson Agg. 1 rigger thrust a heavy steel pinch bar on deck of manlift & it slid across deck, under entrance w/no kick plate, fell onto conv. carriage, glanced toward EE who threw rt arm up as shield, hit his arm & broke it.

A spacer plate that went behind a jaw die fell off the wall of the jaw, landed on the skirts under the jaw and fell against employee, squeezing him between the wall and plate. Employee suffered soreness and stiffness.

Before attempting to start the pit pump, there was a piece of expanded metal laying on top pump that was picked up by employee and tossed to the side. When he tossed & let go of it, a sharp edge cut his finger.

Employee was checking oil in haul truck, and put a flashlight in his mouth. He bit down on flashlight, which resulted in a small portion of tooth breaking off. Due to the existing decay present in the employee's teeth it was not immediately known whether this incident would require repair. After evaluation by dentist, a minor repair was made which made the incident reportable.

One employee was assisting another employee unload steel plates from a freight truck. After sliding the plates off the truck, they attempted to move one of the plates out of the roadway. At this point the employee felt a pain in his lower back. He was taken for medical evaluation and diagnosed with a lower back strain and put on work restrictions.

EE was putting screen cloth in place and felt a slight pain in his shoulder. EE reported the pain on 09/16 and was taken for medical evaluation. EE returned to duty same day with no restrictions. On 09/21, EE complained of pain again and was again taken for medical evaluation, this time, EE was placed on minor restrictions. EE was released to full duty-no restrictions on 09/28

On July 8, EE was dismounting piece of mobile equipment when he felt a pain in lower back. Nagging pain persisted and on August 8, 2010, EE went to doctor and physical therapy was prescribed.

After placing a cable to be used in maintenance of a crusher, the cable came loose and struck employee on lower left leg. Employee was able to walk away from the area but upon medical evaluation, several days of restricted work activity were prescribed.

On 11/4/08, EE stated that elbow still hurting and asked to see doctor. Diagnosis was "tennis elbow." Given single dose of prescription med. Released to regular duty that day but was prescribed physical therapy; thus this report

At the beginning of the shift the employee was fueling his haul truck. After fueling, he got back in his truck and started to the pit. After wiping his face and eyes with his hand, he felt something in his eye. After reporting this to his supervisor, he washed his eyes out. This effort did not improve the situation, so he was taken to the company doctor for treatment.

Two EE's were removing a spring hander from the grizzly feeder. When they moved the hanger, it slid and caught injured's finger between a handrail and the hanger, cutting it between the pinky finger and the ring finger on his left hand.

Tech was reinstalling rim base componets. He had his left hand on componets adjusting to position properly..side ring slipped from tire positioning hand and caught finger tips of fingers on left hand between bead seat band and side flange. Amputated pinky finger at 1st knuckle, laceration index finger, loss of finger tips to ring and middle finger.
