Over-exertion in lifting objects

Employee was lifting screen into place and felt a pain in his left arm.

Struck by falling object

After placing a cable to be used in maintenance of a crusher, the cable came loose and struck employee on lower left leg. Employee was able to walk away from the area but upon medical evaluation, several days of restricted work activity were prescribed.

Contract driver entered scale house complaining of chest pains and feeling bad. EMS was called. Driver was taken to hospital. He was kept for observation and released on May 11, 2009. Driver mentioned history of high blood pressure and not taking medication. He stated that the symptoms on May 8 were blood pressure related as indicated by his doctor and the hospital.

Employee had orthoscopic knee surgery on May 24, 2007 to remove floaters from under kneecap. Related to a first aid incident in November 18, 2006, which later became reportable due to fluid drained from knee and reported to MSHA on 2/2/2007. Employee has returned to light duty.

Fall onto or against objects

On November 18, 2006, IE had tripped while walking to his truck, striking knee on a rock, requiring first aid treatment. IE began suffering swelling to knee, and had fluid drained from knee by doctor on January 26, 2007. He returned to full duty on Monday, January 29, 2007.

Employee was manually clearing rock from the cavity of a 7' cone crusher when he suffered a muscle strain in his lower back.

Caught in, under or between a moving and a stationary object

Employee was adjusting the finger gates on a discharge chute from the vertical to horizontal postition. He had lowered two of the gates and was working on the third, which was stuck. The gate finally broke loose, catching the employee's finger between the third gate handle and second gate handle. He suffered a minor laceration of the finger which required sutures.

While moving a piece of screen cloth, employee cut hand on edge of cloth, which required 4 stiches to close.

Struck by flying object

While digging out a crusher, employee got sand in right eye. Eye was flushed on site, and employee sent to physician for examination. He was given a prescription eye medication (antibiotic) and returned to work with no restrictions the same day.

Fall onto or against objects

EE slipped while exiting cab of service truck, striking heel of left foot on outside step on truck cab. Experienced no pain at the time, but following day experienced some discomfort in ankle. Next day EE saw physican who treated his pain with anti-inflammatory meds. & advised him to restrict use for sev. days. 4/6/05: Physician diag. gout & determined gout was cause of pain.

EE WAS REMOVING 4 FT X 4 FT WIRE SCREEN MESH PANEL FROM A PALLET AND STACKING IN A RACK FOR STORAGE. HE STRAINED HIS BACK.
