Employee was assisting with a cable repair. Another EE dropped a knife in a pile of rockdust. Injured employee reached into the pile of dust to get the knife a received a cut on the RT middle finger that required 4 stitches. The area is low and wet.

Employee was removing valve bank from MBC. While holding valve bank ee felt pop in RT bicep. Employee felt a tingling sensation and pain when trying to straighten arm.
Employee was holding the offside rub rail on miner while rub rail was being installed. Rub rail sprung back jarring EE shoulder. EE continued to work full duty until date of surgery while diagnostic procedures were being completed and diagnosis was confirmed. Surgery completed 12/14/2023.
Contractor was lowering spacer blocks behind a bearing on the 6-3 nose roller. The spacer block slipped and contractor's finger slipped into 1 of the bolt holes on the spacer block causing a laceration to the left ring finger requiring sutures.
THE BOLTER OPERATOR HAD BENT A BOLT DURING INSTALLATION. THEY CUT ABOUT HALFWAY THROUGH THE BOLT AND BEGAN PUSHING IT TO BREAK IT OFF. THE EMPLOYEE GRABBED THE BOLT FROM THE OTHER SIDE AND BEGAN PULLING TO ASSIST THE OPERATOR. THE BOLT BROKE CAUSING THE EE TO FALL BACKWARDS AND THE PLATE ON THE BOLT STRUCK THEIR ARM BETWEEN THE WRIST AND FOREARM CAUSING A LACERATION.
EMPLOYEE WAS USING B-BOND CEMENT TO SEAL KNEE WALLS. EE RECEIVED IRRITATION/BURN ON BOTH FOREARMS AND KNEES FROM CONTACT WITH THE CEMENT DESPITE THE USE OF PROPER PPE AND WASHING. EE HAD BEEN TRAINED ON THE USE AND HAZARDS OF B BOND.
Injured miner was crossing over top of the haulage unit. When EE grabbed the the metal structure to pull self up onto the structure, EE dislocated right shoulder.
Injured miner lifted a resin box and placed it on the roof bolter. As EE lifted the box, EE felt a sharp pain in EE's lower back and the pain radiated up EE's spine toward EE's neck.
EMPLOYEE WAS APPLYING WET CEMENT TO A WALL. CEMENT GOT INSIDE OF EMPLOYEE'S MUD GLOVES CAUSING A CHEMICAL BURN/IRRITATION.
EMPLOYEE WAS MOVING A BRAIDED STEEL WATERLINE ON THE HAULAGE UNIT. A STRAND OF THE METAL BRAIDING WAS PROTRUDING FROM THE HOSE . WHEN THE EMPLOYEE GRABBED THE HOSE THE STRAND OF WIRE PUNCTURED THE EMPLOYEE'S RIGHT LITTLE FINGER. EMPLOYEE WAS TAKEN TO HOSPITAL ON 7/26 AND GIVEN INTRAVENOUS ANTIBIOTICS DUE TO FINGER BECOMING RED AND SWOLLEN.
Employee was in the process of emptying a trash can in the shop. EE strained stomach as EE attempted to lift the trash can.
Employee was roof bolting. While retrieving EE's drill steels from the roof, the last 2 steels stuck together. When the employee pulled on them to get them apart, one of the steels came back and struck the employee in the nose, causing a small laceration and fracturing EE's nose.
A miner was injured while in the process of changing the canopy post on a twin boom Roof Bolter in MMU 006-0. As the canopy was being lifted off the operators' side, the canopy fell and caused pinched tip of the injured miners' finger.
A miner was injured when ee placed hand on the chuck of the roof bolter head to check for suction. EE right ring ringer was injured when a drill steel fell and contacted the tip of finger.
Dirt/debris entered the injured miners left eye on 05/26/2017. Medical treatment was requested on 06/05/2017 because of a continuing eye irritation. The miner was prescribed eye drops on 06/06/2017.
A miner was injured when employee pinched the tip of employee's left little finger while working on the Fletcher roof bolter.
A group of miners was changing the conveyor belt on number 4 belt line. A miner was injured when EE was struck on the left forearm by a belt grab. The belt grab broke left ulna bone.
A miner was injured while loading 6 inch plastic water line on a scoop. When the 6 inch plastic waterline was placed on the scoop, the end closest to the injured miner bounced up and contacted the miner's safety glasses. The safety glasses caused a small laceration under left eyebrow. The hospital placed three sutures in the affected area to close the laceration.
Changing inserts on continuous miner head. Metal on metal contact caused a metal sliver to become dislodged and penetrated the injured miner's wrist. Doctor removed the metal from EE's wrist on 4/14/2016 and EE received several stitches to close the wound.
The injured miner pinched his finger between messenger cable and a conveyer roller as he was installing a new belt.
Employee was changing scoop batteries when he lifted the lids on the batteries he placed on charge it slipped and struck him on the left hand causing a laceration to his middle finger resulting in 6 stitches
Employee was lifting pump off of a cart. Didn't see he was standing on pump cable. When he twisted to walk with the pump, cable pulled pump which caused employee's back to twist.

The man was changing scoop batteries. The man was removing the scoop cable plug which was tight in the connection. While pulling with force on the plug, it suddenly came loose and struck the man on the lip. Medical Report Only(sutures)
The man was loading belt structure into a scoop bucket when he felt a pain in his lower abdomen.
The man was sitting on the mantrip. As the mantrip began to move, he noticed the lunch bag on top of the mantrip was rubbing against the roof. He stated he told the operator to stop and reached for the bag. The operator did not hear him and continued to tram. The mantrip struck a rock causing the man's arm to momentarily be pinched between the mantrip and the roof.
The man had repaired the shuttle car cable. He was sitting in the shuttle car winding up the cable slack. He leaned out of car to see how much slack was left when the cable came up quickly and struck him on the head.
The man was carrying a 5 gallon container of oil on the top of his shoulder. When he bent over, the oil can shifted and the man reported feeling a pain in his low back area.
Two men were knocking down a block wall. While one person was picking up block, the other man hit a row of block hanging from the roof causing the blocks to fall and strike his co-worker.
The man was crawling in the section when he reported low back pain.

The employee was pulling a miner cable along with another employee. While attempting to throw the cable along the rib, the other employee gave the cable a strong pull causing it to come up and struck the person in the face and knocking off his safety glasses.

Some spray came out of the nozzle as he was cleaning it and went onto the man's safety glasses and a portion of the material went behind the glasses and into his eyes causing an irritation.
