EE was sitting on same side as scoop operator, approx 50' outby scoop. Scoop operator made a pass to the face, during time EE moved to blind side of scoop. Scoop came around corner, ran over EE's foot.
EE placed his right hand on the drill head for body support while observing roof conditions. His hand slipped, causing EE to strike gloved hand on drill head. Roof bolter not energized. EE continued to work regular duty until 6/13/07, at which time he went on restricted duty.
While advancing a crosscut left off #4 entry of mecharized mining at #00-0 near survey station #1228 a 3"" borehole was cut into. Mining operations ceased until the hole was plugged and a 103(R) order issued by MSHA was terminated. There were no injuries and no property damage. MSHA national hotline was notified.
Injured EE was holding onto drill steel with up pressure on head. He was attempting to drop head to change bit end. Steel broke in 2 pieces. Top part remained in roof & other part in drill head. Upward pressure abruptly raised to roof injuring thumb.

Injured empliyee was attempting to park mantrip in a charging station. Left front tire struck rib line, while attempting to back up from rib-steering wheel turned abruptly in oposite direction striking employee on ungloved hand.
Injured employee was assisting witness named on report unsecuring portable rockduster. Tongue hitch was being blocked. Witness lost control of hitch, dropping same on injured's finger.
EE states he slipped on sloped, wet bottom while carrying 50lb bag of rock dust, landing on right hip. No accident report filed, sought treatment on 3/30/06. Advised mine management 3/30/06.
Employee claims he felt hot flash in lumbar area while handling continous miner cable. Area where accident occured was four feet high slightly sloped to left, bottom was wet. Employee worked entire shift, did not report injury until arriving at residence two hours later

EMPLOYEE WAS STRUCK ON HIS FACE BY DRILL STEEL THAT WAS STUCK IN DRILL HOLE. STEEL SUDDENLY DISLODGED, STRIKING EE & CAUSING A LACERATION TO HIS LEFT CHEEK, REQUIRING 7 SUTURES FOR CLOSURE.
INJURED PERSON WAS INSTALLING A RESIN GROUTED ROOF BOLT WHEN HE INADVERTENTLY PINCHED TWO FINGERS OF HIS LEFT HAND BETWEEN THE ROOF BOLT BEARING PLATE AND THE ROOF. HE SUSTAINED OPEN FRACTURES OF THE MIDDLE AND RING FINGERS ON HIS LEFT HAND WHICH WILL REQUIRE HIM TO LOSE TIME FROM WORK.
Injured person moved a pump from one working face area to another. That afternoon he reported having back pain from moving the pump. He later that same day also reported pain in left shoulder. He sustained a strain of low back and left shoulder.
Employee had accumulations of rock in knee pads. On attempting to kneel down he states sharp pain felt in right knee. Area progressively became swollen, warm to touch over the following week. Sought treatment on 9-27-05.
EMPLOYEE WAS WALKING IN AN ENTRY IN A BENT FORWARD POSITION. HE IS 6'3"" TALL AND THE ENTRY IS 5'6"" HIGH. HE FELT A SHARP PAIN IN LOWER BACK WHILE WALKING.
EMPLOYEE WAS ATTEMPTING TO REMOVE SUCTION HOSE FROM ROOF BOLTER DRILL HEAD. HOSE DISCONNECTED ABRUPTLY CAUSING EE'S LEFT HAND (UNGLOVED) TO STRIKE THE FRAME OF THE MACHINE.
Continous Miner was tramming in reverse direction. Injured employee reached to move cable/h20 line from rear of machine. Finger was caught between miner and line post.
