Employee was lifting pump off of a cart. Didn't see he was standing on pump cable. When he twisted to walk with the pump, cable pulled pump which caused employee's back to twist.

Roof fall occurred in an outby area. Approximately 30' long x 20' wide x 8-10' high. Did not affect any escapeway or ventilation.
Pulling bolter cable from reel felt pain in shoulder.
The man was changing scoop batteries. The man was removing the scoop cable plug which was tight in the connection. While pulling with force on the plug, it suddenly came loose and struck the man on the lip. Medical Report Only(sutures)
The man was loading belt structure into a scoop bucket when he felt a pain in his lower abdomen.
The man complained of a persistent pain in his knee. He was unaware of any specific incident that caused it.
The roof bolter stood on the head of bolter to insert resin and bolt in a 9 ft high area. He asked another operator to raise wrench up with head to push the bolt into roof. After pushing the bolt up, the assisting operator began lowering the head. The victim's foot which was still on the drill head frame became pinched between the frame and hydraulic fitting of the machine.
The man was sitting on the mantrip. As the mantrip began to move, he noticed the lunch bag on top of the mantrip was rubbing against the roof. He stated he told the operator to stop and reached for the bag. The operator did not hear him and continued to tram. The mantrip struck a rock causing the man's arm to momentarily be pinched between the mantrip and the roof.
The man was carrying a 5 gallon container of oil on the top of his shoulder. When he bent over, the oil can shifted and the man reported feeling a pain in his low back area.

A roof fall occurred in the NW-B section (MMU 001-0) in #1 to #0 crosscut between survey station 7219 and survey station 7185. The fall was approximately 50 feet long x 20 feet wide x 8-10 feet high.
Man reported on 3-4-14 he had numbness in both hands. It was unclear if this was work related. On 5-8-14 man reported off work for carpal tunnel surgery. After further review of information, the incident has been accepted as work related.
The man had finished installing a row of roof bolts. While preparing to advance the roof bolter, a piece of roof scale fell and struck the man on the knee.

The man was carrying a box of roof bolter bits from the mantrip to the bolter when he felt a pain in his lower back.
A man was spraying a fire proofing material on the rib in a scoop charger when the nozzle plugged up. As he was cleaning the nozzle, the injured person was standing beside him. Some spray came out of the nozzle as he was cleaning it and went onto the man's safety glasses and a portion of the material went behind the glasses and into his eyes causing an irritation.
The man was walking when he slipped on a wet ledge and twisted his knee. The man performed his normal duties until 9-26-12 when he went on restricted duty.
The man was lifting a scoop tractor battery cover when it slipped out of his right hand and fell on his left hand.
The man was separating 2 roof drill steels that were stuck together. When the drill steels separated, the steel struck the man in the teeth. The man was switching his seating location in a shuttle to change travel directions. While turning around he tripped and his hand struck the coal rib when he attempted to catch his balance.
The man was cleaning coal off the bottom with the continuous miner just outby the cut that was partially mined. As he was cleaning and positioning to enter the left side of the cut, the right side of the tail contacted him in the hip area.

Due to a break in the sump discharge line, an overflow from the surface sump entered the alternate escapeway and adjacent entries to the left of this escapeway. Workers were withdrawn from the mine.
