Employee was lifting pump off of a cart. Didn't see he was standing on pump cable. When he twisted to walk with the pump, cable pulled pump which caused employee's back to twist.

The man was changing scoop batteries. The man was removing the scoop cable plug which was tight in the connection. While pulling with force on the plug, it suddenly came loose and struck the man on the lip. Medical Report Only(sutures)

The roof bolter stood on the head of bolter to insert resin and bolt in a 9 ft high area. He asked another operator to raise wrench up with head to push the bolt into roof. After pushing the bolt up, the assisting operator began lowering the head. The victim's foot which was still on the drill head frame became pinched between the frame and hydraulic fitting of the machine.

The man was sitting on the mantrip. As the mantrip began to move, he noticed the lunch bag on top of the mantrip was rubbing against the roof. He stated he told the operator to stop and reached for the bag. The operator did not hear him and continued to tram. The mantrip struck a rock causing the man's arm to momentarily be pinched between the mantrip and the roof.

The man was carrying a 5 gallon container of oil on the top of his shoulder. When he bent over, the oil can shifted and the man reported feeling a pain in his low back area.

A roof fall occurred in the NW-B section (MMU 001-0) in #1 to #0 crosscut between survey station 7219 and survey station 7185. The fall was approximately 50 feet long x 20 feet wide x 8-10 feet high.

Man reported on 3-4-14 he had numbness in both hands. It was unclear if this was work related. On 5-8-14 man reported off work for carpal tunnel surgery. After further review of information, the incident has been accepted as work related.

The man had finished installing a row of roof bolts. While preparing to advance the roof bolter, a piece of roof scale fell and struck the man on the knee.

The man was drilling a bolt hole into the roof near the rib to install a roof bolt. While lowering the drill head, a corner of the drill head struck the man on the upper leg.

The man was cleaning off the fender of a shuttle car when his hand scraped across a piece of metal strap and lacerated his hand.

The man was assisting a mechanic to tighten a scoop tire when the wrench slipped and the man fell onto the bottom twisting his ankle.

A man was cutting a piece of belting to make a splice. The injured person reached down in the area to make a measurement and put his hand in line with where the man was cutting the belt. The knife contacted the injured person's hand and lacerated it. Report filed for Medical Treatment Only.

The man was walking to the personnel carrier when he tripped in a rut and fell on his left shoulder. The man continued in a regular duty status until 8-8-13 when he went on a restricted duty status.

The man slipped and fell while walking in the mine and injured his shoulder when he attempted to stop his fall. The man continued to work his regular duties until 1-29-2013 at which time he went on a restricted duty status.

The man was carrying a box of roof bolter bits from the mantrip to the bolter when he felt a pain in his lower back.

Absorption of radiations, caustics, toxic and noxious substances

A man was spraying a fire proofing material on the rib in a scoop charger when the nozzle plugged up. As he was cleaning the nozzle, the injured person was standing beside him. Some spray came out of the nozzle as he was cleaning it and went onto the man's safety glasses and a portion of the material went behind the glasses and into his eyes causing an irritation.

The man was separating 2 roof drill steels that were stuck together. When the drill steels separated, the steel struck the man in the teeth. A roof fall was discovered in the #1 entry, second intersection outby the 3 Right Mains faces, Spad 3558. The fall was approximately 30 feet long x 20 feet wide x 6-8 feet high.

The man was switching his seating location in a shuttle to change travel directions. While turning around he tripped and his hand struck the coal rib when he attempted to catch his balance.

The man was cleaning coal off the bottom with the continuous miner just outby the cut that was partially mined. As he was cleaning and positioning to enter the left side of the cut, the right side of the tail contacted him in the hip area.

Sperry Well Drilling Co. was drilling a well for a customer and drilled into the mine. Water entered the mine through the 6"" hole. The hole was plugged and grouted.

The man was riding on the personnel carrier with his legs resting on the front fender. While traveling through a check curtain, the man's foot caught on the curtain and caused his leg to hyper extend.

The man was installing a roof bolt in the roof. The bolt bent while he was installing it. When the man began to pull on the bolt, he reported that he felt pain in his back.

While drilling roof, the man drilled the first steel into the roof. He lowered the roof bolter head to insert the second steel into place. The first steel fell out of drill hole and struck the man on the hand.

Heavy rain overcame a diversion ditch and the water ran into the mine's surface pit sump. As a precaution because of the continuing heavy rain, mine personnel were instructed to evacuate the mine at approximately 11:15 am. At approximately 01:17 pm, the heavy rain and water from the diversion ditch overcame the mine pit sump pumps and water entered the primary escapeway.

The man had his hand on the boom arm of the roof bolter and as the boom raised he caught he finger between the boom arm and canopy.

The man reported pain in his right lumbar and right knee. The man reported off on 7-29-10 with a non work related medical problem. On 8-4-10 he completed a work incident report for his knee and back regarding a stated 7-27-10 incident. On 8-16-10 a medical examination indicated that the lumbar and knee pain were work related.

The man was walking through the section when he slipped and fell forward. He attempted to break his fall with his arms and jammed his arms into the bottom. The man continued to work regular duty until 09/07/2010 when he reported off work.
