Employee was lifting pump off of a cart. Didn't see he was standing on pump cable. When he twisted to walk with the pump, cable pulled pump which caused employee's back to twist.

Roof fall occurred in an outby area. Approximately 30' long x 20' wide x 8-10' high. Did not affect any escapeway or ventilation.

Pulling bolter cable from reel felt pain in shoulder.

The man was changing scoop batteries. The man was removing the scoop cable plug which was tight in the connection. While pulling with force on the plug, it suddenly came loose and struck the man on the lip. Medical Report Only(sutures)

The man was loading belt structure into a scoop bucket when he felt a pain in his lower abdomen.

The man complained of a persistent pain in his knee. He was unaware of any specific incident that caused it.

The roof bolter stood on the head of bolter to insert resin and bolt in a 9 ft high area. He asked another operator to raise wrench up with head to push the bolt into roof. After pushing the bolt up, the assisting operator began lowering the head. The victim's foot which was still on the drill head frame became pinched between the frame and hydraulic fitting of the machine.

The man was sitting on the mantrip. As the mantrip began to move, he noticed the lunch bag on top of the mantrip was rubbing against the roof. He stated he told the operator to stop and reached for the bag. The operator did not hear him and continued to tram. The mantrip struck a rock causing the man's arm to momentarily be pinched between the mantrip and the roof.

The man was carrying a 5 gallon container of oil on the top of his shoulder. When he bent over, the oil can shifted and the man reported feeling a pain in his low back area.

The man was tightening a bolt on a shuttle car. As he twisted the wrench, his hand rubbed against a piece of metal lacerating it. The man first went for treatment on 5/22/2014. Report Filed for Medical Treatment Only (stitches)

The man had changed the blade on his utility knife the previous shift and forgot to retract the blade. As he was beginning his next shift he reached for his knife and slid his finger across the blade cutting his finger. Report Filed For Medical Treatment Only (sutures)

A roof fall occurred in the NW-B section (MMU 001-0) in #1 to #0 crosscut between survey station 7219 and survey station 7185. The fall was approximately 50 feet long x 20 feet wide x 8-10 feet high.

Man reported on 3-4-14 he had numbness in both hands. It was unclear if this was work related. On 5-8-14 man reported off work for carpal tunnel surgery. After further review of information, the incident has been accepted as work related.

The man had finished installing a row of roof bolts. While preparing to advance the roof bolter, a piece of roof scale fell and struck the man on the knee.

The man was drilling a roof bolt hole. While removing the drill steels from the hole, the steels separated and the coupler struck the man on the eyebrow. Report Filed for Medical Treatment Only.(Sutures)

The man was drilling a bolt hole into the roof near the rib to install a roof bolt. While lowering the drill head, a corner of the drill head struck the man on the upper leg.

A roof fall occurred just inby survey station 6486 in crosscut from 3-4 entry and extended through crosscut of 7-8 entry of the 3 Right section. The fall is approximately 20 feet wide x 250 long x 15 feet high.

A roof fall occurred in the #6 entry of Main A at survey station 17. The fall was approximately 30 feet long x 20 feet wide x 10 feet high.

The man was picking up tools at the tool sled. He stepped back and onto a drill steel laying on the bottom causing him to fall and twist his knee.

The man was lowering the roof bolter drill head to remove drill steels from the roof. The man reached for the drill steels before they had stopped spinning. His glove became caught on the drill steel coupler and twisted his hand.

The man was cleaning off the fender of a shuttle car when his hand scraped across a piece of metal strap and lacerated his hand.

The man was assisting a mechanic to tighten a scoop tire when the wrench slipped and the man fell onto the bottom twisting his ankle.

A man was cutting a piece of belting to make a splice. The injured person reached down in the area to make a measurement and put his hand in line with where the man was cutting the belt. The knife contacted the injured person's hand and lacerated it. Report filed for Medical Treatment Only.

The man was walking to the personnel carrier when he tripped in a rut and fell on his left shoulder. The man continued in a regular duty status until 8-8-13 when he went on a restricted duty status.

The man slipped and fell while walking in the mine and injured his shoulder when he attempted to stop his fall. The man continued to work his regular duties until 1-29-2013 at which time he went on a restricted duty status.

The man was carrying a box of roof bolter bits from the mantrip to the bolter when he felt a pain in his lower back.

A man was spraying a fire proofing material on the rib in a scoop charger when the nozzle plugged up. As he was cleaning the nozzle, the injured person was standing beside him. Some spray came out of the nozzle as he was cleaning it and went onto the man's safety glasses and a portion of the material went behind the glasses and into his eyes causing an irritation.

The man was walking when he slipped on a wet ledge and twisted his knee. The man performed his normal duties until 9-26-12 when he went on restricted duty.

The man was lifting a scoop tractor battery cover when it slipped out of his right hand and fell on his left hand.

The man was separating 2 roof drill steels that were stuck together. When the drill steels separated, the steel struck the man in the teeth. The man first treated for the incident on 6-15-12 and it was determined that the teeth were cracked and required dental repair. Report filed for Medical Treatment Only.

The man was removing a full box of resin from the top of the roof bolter. While reaching out and lifting the resin box, he felt a pain in his shoulder.

The man was pulling up the slack on the roof bolter cable when he slipped and fell backwards onto the bottom.

The man was tramming a mantrip when the wheels of the mantrip struck a rut in the mine floor causing the steering wheel to spin quickly. The steering wheel knob struck the man on the hand.

The man was walking in the #6 entry when a piece of roof scale struck him on the hand.

A roof fall occurred in the intersection of the 3 right #6 room 136 feet inby survey station 3675. The fall was approximately 22 feet long x 22 feet wide x 10 feet high.

A roof fall was discovered in the #1 entry, second intersection outby the 3 Right Mains faces, Spad 3558. The fall was approximately 30 feet long x 20 feet wide x 6-8 feet high.

The man was switching his seating location in a shuttle to change travel directions. While turning around he tripped and his hand struck the coal rib when he attempted to catch his balance.

The man was cleaning coal off the bottom with the continuous miner just outby the cut that was partially mined. As he was cleaning and positioning to enter the left side of the cut, the right side of the tail contacted him in the hip area.

Sperry Well Drilling Co. was drilling a well for a customer and drilled into the mine. Water entered the mine through the 6"" hole. The hole was plugged and grouted.

The man was riding on the personnel carrier with his legs resting on the front fender. While traveling through a check curtain, the man's foot caught on the curtain and caused his leg to hyper extend.

The man was installing a roof bolt in the roof. The bolt bent while he was installing it. When the man began to pull on the bolt, he reported that he felt pain in his back.

While drilling roof, the man drilled the first steel into the roof. He lowered the roof bolter head to insert the second steel into place. The first steel fell out of drill hole and struck the man on the hand.

The man was installing a drill head on a roof bolter when he pinched his hand between the drill head and the frame of the machine.

The man was shoveling coal around the belt feeder when he strained his lower back.

The man was re-positioning a portable rock dusting machine and developed pain in his back.

The man was walking to the personnel carrier when he slipped on wet bottom, fell and twisted his knee.

A roof fall was discovered in an outby area in the #1 entry at station number 2809. The fall was approximately 60 feet long x 18 feet wide x 6 feet high.

The man was pushing a roof bolt into a drill hole in the roof when he felt a pain in his left elbow.

The man was repairing a roof bolting machine. As he was stepping off the machine, he slipped and fell causing pain in his knees.

The man was tramming a personnel carrier through an airlock door that was opened. While tramming the carrier, the door started to swing closed and struck the man on the leg.

The man was crawling beside the scoop tractor. When he slid his hand along the scoop bucket, he lacerated his finger on a sharp burr on the bucket.

The man was stacking cement blocks when a block fell and struck the man on the left hand.

The man was pulling a shuttle car cable when he slipped and fell to the bottom.

The man was walking in the section when his left leg gave out from under him.

The man was using a pry bar to align the hole to insert a boom jack pin. While pulling on the pry bar it broke and the bar struck him on the cheek and eye.

The man was lifting a cable cross-over ramp to place on the roof bolting machine when he felt a pain in his back.

Heavy rain overcame a diversion ditch and the water ran into the mine's surface pit sump. As a precaution because of the continuing heavy rain, mine personnel were instructed to evacuate the mine at approximately 11:15 am. At approximately 01:17 pm, the heavy rain and water from the diversion ditch overcame the mine pit sump pumps and water entered the primary escapeway.

The man was roof bolting when a drill steel became lodged in the roof. As he was attempting to dislodge it, the drill steel fell from the drill steel hole and struck the man on the thumb. Report filed for medical treatment only.

The man had his hand on the boom arm of the roof bolter and as the boom raised he caught he finger between the boom arm and canopy.

The man reported pain in his right lumbar and right knee. The man reported off on 7-29-10 with a non work related medical problem. On 8-4-10 he completed a work incident report for his knee and back regarding a stated 7-27-10 incident. On 8-16-10 a medical examination indicated that the lumbar and knee pain were work related.

The man was walking through the section when he slipped and fell forward. He attempted to break his fall with his arms and jammed his arms into the bottom. The man continued to work regular duty until 09/07/2010 when he reported off work.

The man was repairing the water line on the continuous miner. While using the banding tool, his hand slipped and his finger was cut on the banding. He received 5 stitches in his finger. Report filed for medical treatment only.

The man was walking through the section conducting an examination when he felt a pain is his back.

The man was tramming the roof bolter out of a working place from the operator's compartment. The man had his arm resting on the corner of the compartment. As he turned the machine, it moved close to the rib and he pinched his arm between the machine and the rib.

The man was tramming a shuttle car through a crosscut and had the shuttle car get too close to the coal rib. While attempting to tram the car away from the rib, the man caught his arm between the rib and shuttle car.

The man was cutting the rubber jacket on a phone line when the knife slipped and lacerated his thumb which required stitches. Report filed for medical treatment only.

The man was lifting on the top belt to install belt structure when he felt a pain in his left groin area.

The man was removing a J-hook from a roof bolt plate when he caught his hand against the plate and lacerated his hand.

The man was walking out of the outside shop when he slipped and fell to the ground.

The man attempted to remove by hand a drill steel stuck in the mine roof without success. While preparing to add a second steel into the bolter chuck to raise into the stuck steel, the man inadvertently raised the drill head and caught his finger between the chuck and the drill steel.

Due to a break in the sump discharge line, an overflow from the surface sump entered the alternate escapeway and adjacent entries to the left of this escapeway. Workers were withdrawn from the mine.

EE was walking in the section when he struck his hard hat against a J channel supported from the roof.

The man was riding on the mantrip into the section when he struck his hard hat against the roof.

EE started to walk along side of the roof bolter while the machine was being trammed and the EE got his leg caught between the rear of the machine and the rib.

The man was cleaning a shuttle car and a panel lid fell onto top of man's foot.

The man was prying a roof strap with a drill steel and the steel slipped striking him on the forehead.

The man was tramming a ram car when the canopy caught a roof strap. The damaged strap struck the man on the hand causing a laceration.

The man was hanging a cable against the roof, when he felt a pain in his right knee.

The man was striking the drill head of the roof bolter with a hammer to dislodge a wrench when a piece of metal flew and struck the man in the eye. (The man is off work for additional treatment to his eye. No new incident occurred. Off effective 12-12-09.)

The man reported pain in his knee. He stated no specific trauma occurred. The person continued to work until 5-15-09 when he sought medical treatment and reported off work.

The man was pulling miner cable when he felt a pain in his back. The man continued to work until 4/16/09 at which time he reported off work.
