Accident type, without injuries

An unintentional roof fall has occurred in the 2 main section at 6:50am on 12-19-2014 the fall measured 18 feet wide by 50 feet long in # 5 entry and 18 feet wide by 20 feet long in the 5-6 entry the fall is approximately 15 feet high

Struck by falling object

Employee was changing scoop batteries when he lifted the lids on the batteries he placed on charge it slipped and struck him on the left hand causing a laceration to his middle finger resulting in 6 stitches

Miner was operating helper side of roof bolting machine when a piece of rock fell off of rib and struck the employee on the right leg

The man was picking up a water pump when it slipped in his hands causing a pain in his right elbow. The man continued to work regular duties until 8-15-13 at which time he went on restricted duty status.

Caught in, under or between a moving and a stationary object

The injured person didn't realize his foot was under the edge of the skid of lumber he was putting a come-along across as the forks were on the ground but tilted slightly upward. The operator was getting out of the machine to assist. He lowered the forks to ensure they were on the ground and they tilted forward onto the man's foot.

Struck by powered moving object

THE MAN WALKED FROM #7 ENTRY TO #5 ENTRY TO RECOVER HIS HAMMER AND ON THE WAY BACK HE CAME INBY IN #6 ENTRY. THE SHUTTLE CAR CAME AROUND THE CORNER AND CAUGHT HIM AGAINST THE RIB.

THIS WAS AN OUTBY INTAKE AREA, WHERE OTHER FALLS HAVE OCCURRED. THE AREA WAS BOLTED WITH 4' RESIN BOLTS AND THE FALLS ALL ARE 4' HIGH. IF 6' BOLTS WERE USED, THE FALLS--IN ALL LIKELIHOOD--WO ULDN'T HAVE HAPPENED.

THERE WAS A VERY LARGE MUD SLIP ACROSS THE INTERSECTION. IT WAS BOLTED WITH 6' FULLY GROUTED RESIN AND HAD 9' COMBINATION BOLTS SPOTTED BETWEEN THE T-5 CHANNELS. THE AREA HAD A ROW OF CRIBS NA RROWING WIDTHS AND DOWN TO 14'. NOTHING WAS NOTICED ON THE WEEKLY EXAMINATION 1 WEEK PRIOR.

A ROOF FALL OCCURRED AT THE FACE OF #3 ENTRY FROM THE FACE, WHICH WAS BOLTED WITH 6' FULLY GROUTED RESIN BOLTS WITH T-5 CHANNELS AND HAD ADDITIONAL 9' DOUBLE LOC BOLTS AS EXTRA SUPPORT. THERE WAS 2 CLAY VEINS THAT CONVERGED IN THE ROOF ABOVE THE ANCHORAGE AREA AND HAD A LARGE AMOUNT OF MUD IN THE INTERSECTION OF THE 2 CLAY VEINS.

EMPLOYEE WAS HELPING UNCOUPLE A SUPPLY TRAILER FROM THE SCOOP WHEN THE SCOOP MOVED AND THE TRAILER SLIPPED SIDEWAYS IN A MUDDY RUT.

Struck by falling object

WHILE ROOF BOLTING IN A HIGH SPOT THE EMPLOYEE WAS ADDING ANOTHER STEEL AND THE STEEL STAYED IN THE HOLE. WHILE HE WAS PUTTING THE OTHER STEEL IN PLACE, THE STEEL LODGED IN THE ROOF FELL STRIKING HIM ON HIS RIGHT THUMB.

A ROOF FALL OCCURRED IN THE INTERSECTION OF #4 TO #5 XCUT 25'INBY SURVEY STATION #419. LENGTH 70', WIDTH 18', HEIGHT 12-13'. A COAL SEAM CAME DOWN TO WITHIN 8 1/2' OF THE ROOF AND A LARGE UNDE TECTED SLIP CAUSED THE FAILURE. AREA WAS BOLTED WITH 6 & 8 FOOT FULLY GROUTED RODS AND 12 FOOT CABLE BOLTS.

WHILE MINING #4 ENTRY THE LEFT RIB ROLLED OUT EXPOSING A 3"" DIAMOND DRILL HOLE, USED FOR CORE INFORMATION. THE WATER ENTERED THE MINE FROM THE ABOVE C SEAM MINE. THERE WAS LITTLE EVIDENCE OF GROUT IN TH HOLE. WATER WAS REMOVED BY PUMPS ANDTHE HOLE GROUTED FROM THE BOTTOM.

WHILE CHECKING THE WATER TREATMENT FACILITY EMPLOYEE SLIPPED ON SOME ICE INJURING HIS WRIST IN THE FALL.

Struck by falling object

ROOF BOLTING IN A HIGH SPOT. ROCK FELL STRIKING THE EDGE OF THE CANOPY AND DEFLECTED AGAINST THE DRILL BOOM AND CAME UNDER THE DRILLERS CANOPY HITTING EMPLOYEE IN THE MOUTH.
