Injured employee was tightening Pitman wedge bolts while using a knock out wrench to tighten bolts. When he struck wrench with hammer he pinched his left hand between the hammer and wrench causing a laceration to the left hand.
EE's left hand finger was crushed between pry bar and a metal skirt, while using a pry bar to clear a rock out of the plant feeder at conveyor #2, receiving ten stitches.

Employee was using a wrench attempting to break a bolt loose with body in full tension and body in prone position approx. 6-8 inches off the ground. Employee was replacing the master pin in the track on a Komatsu track hoe. A piece of the pin chipped off striking the forearm of the employee.
After cleaning out rail car; employee was using a metal bar to close the rail car door. He said something let loose in his lower back. The door was defective and hard to close.
The employee went to the company doctor on 7/10/2007 and was placed on restricted duty. While chipping a weld, using a chipping hammer, a piece of hot slag bounced into the employee's left eye, resulting in a corneal burn to L-eye.

After heating the plate injured, employee instructed co-worker to hit the plate with a hammer which caused a rock to become loose from a shelf underneath the crusher and strike employee's head above his right ear.

The drill tech was replacing a leaky fitting The drill tech stepped up onto the chain wrench to get a better reach. The wrench moved and he lost his balance. His knee struck the wrench during the fall injuring his knee. Knee strain possible level 1 trauma.
Employee was putting a section of new belting into a conveyor belt. While attempting to cut the old belt using a straight edge and razor knife along the straight edge, it slipped and cut his right index finger.

Employee was trying to loosen and remove a tight nut. He was hitting on the handle of the box-end wrench with a small sledge hammer in an attempt to free the tight nut. The hammer deflected off the wrench, and the employee's finger was caught between the wrench and the hammer handle. Employee's right index finger was cracked as a result of the blow.
EE crushed left thumb with a hammer used to loosen a bolt on the bed of the haul truck, requiring two stitches.
Employee was using a manual pipe bender to bed conduit for an electrical installation. The employee had his left foot on the foot pedal of the pipe bender and was placing downforce on the pedal to bend the conduit when he felt pain in his ankle and felt the ankle give out. The employee was transported to a local hospital by ambulance and diagnosed with a broken left ankle.
EE punctured the palm of his right hand while working on a water line to the stool in the employee bathroom. He washed it out and bandaged it. Later he developed an infection. On March 2nd the infection was removed. This is when the injury was reported.
Injured employee was holding a bearing tool device over his head to replace ride cylinder bearing on a Euclid fuel truck. A second employee was using a shop hammer to hit the tool to knock bearings in place. The handle on bearing tool broke and fell striking the injured employee in the mouth causing lacerations and loose teeth. The tool weighed app. 30 lbs.
Mechanic struck end of tooth with a hammer while replacing bucket tooth on 988G loader and piece of steel broke off tooth and hit him in the left eye.

He referred EE to surgeon, who placed EE on app. 6 wks restricted duty. Once surgeon's medical notes were rec'd (4/5/07) & reviewed it was determined this was a new hernia.
EE was cutting a belt with a utility knife. As EE was returning the knife for a second pass, the knife struck the edge of the belt, and his hand slid down the blade lacerating his middle finger. The laceration required sutures.
The employee was using a bar to dislodge material that was packed in a chute. He was pulling back on the bar and mashed his left thumb between the bar and the handrail on the catwalk. The injury resulted in 6 stitches.
Taking J-bolts out of the middle deck of a broken screen on wash tower, he caught right forearm on metal above.
The employee went to see the company doctor on 10/17/2006 and was returned to regular duty. A piece of metal became embedded in the employees L-cheek of face while chipping a rock out of the bolt hole. Stitches were applied.
EE went down to the booster pump on the dredge to check for a stuck rock. While attempting to unbolt the inspection plate off the pump, the locking mechanism on the 3/4" drive ratchet slipped causing the EE to hyperextend his left shoulder resulting in a strain. EE did not see a doctor until 9/18 adn MRI taken on 9/22.
EE was trying to dislodge a bearing from a shaft by using a hammer to tap on the bearing race as another employee heated the race. A small piece of metal broke off the race and entered EE forehead.
Employee was replacing v-belt on the HP 300 crusher. When he started tightening the v-belts with the jack shaft he strained his back. Employee did not seek medical attention until 8/31/06.
Employee opened inspection doors on the VSI crusher to inspect wear on the hammers, manufacturer does not have means of securing open door. The opened door fell when accidentally struck by a come-a-long chain falling on the employee's left foot resulting in a fracture.
