Fall onto or against objects

Injured employee was turning to acknowledge another person and when EE turned, EE's foot was against a piece of dunnage that steel was sitting on and EE lost EE's balance. EE began falling forward but was unable to catch self before landing on some large rocks on EE's left shoulder causing the injury.

Struck by falling object

The employee was in the process of cleaning material off the concrete pad under screen #3 at approximately 11:00 am. While setting the hose on the ground, debris that had accumulated on the structure above broke loose and fell to the ground, striking the employee in the back of the hardhat and their upper back. Employee sustained multiple injuries.

As the technician was removing rim components that were seated in the tire, they fell due to the tire being upright rather than being laid flat on the ground. As they fell the technician tried to catch the components injuring right shoulder.

Struck against a moving object

Haul Trucks were hauling ½""- rock from the Stone Plant to the Asphalt Plant. Approx. 10:40am, the operator of the empty haul truck was traveling S. toward Stone Plant. The empty truck appeared to travel outside its lane and collided with the cab of oncoming loaded haul truck traveling N. Both EEs were transported and admitted to hospital due to multiple injuries.

Injured worker was working at the edge of the building, tied off at the ridge. Injured worker went back to ridge area to get blocking material. When injured worker headed back down, IW stepped through a skylight and fell.

On 10/24/17, employee was walking into a tunnel & tripped over a conveyor belt laying on the ground. Employee attempted to catch self by bracing self to the wall with left hand. EE received First Aid treatment. Employee continued to experience pain & discomfort & decided to seek medical attention on 11/13/17 - diagnosed with a strained left shoulder & on restricted duty.

Employee's body shifted in the cab of haul truck when EE ran over a pot hole. A short time later EE experienced numbness in legs and was taken for medical treatment. EE is currently losing time.

IW was welding on a piece of equipment and as IW changed positions the welding torch flame went across his wrist between the glove and his shirt.

Employee was performing maintenance on conveyor using an electric drill. The drill jammed while in reverse motion catching his right hand between the drill handle and a fixed object. He sought medical attention and is currently on restricted duty.

Struck against stationary object

The injured was attempting to rotate an impact crusher rotor shaft to dislodge material that had it jammed using a pry bar. The pry bar slipped as he was pulling which caused his hand to impact the crusher housing. Received six stitches to left middle finger to close the wound.

Struck by flying object

A boulder too large to enter the crusher cavity was lodged on the feeder. The injured was using a steel cable to wrap around the boulder. The cable would then have been hooked to a loader bucket and removed from the feeder. When he attempted to take the slack out of the cable it slipped and hit him in the left eye.

Fall from machine

After washing drill, driller had trouble with his AC unit. After finishing the hole he was drilling, he climbed up to check on the problem. On his way down with his left foot on the track of the drill, his right pants leg snagged on a piece of metal causing him to lose his balance and fall to the ground. He landed on his left arm and shattered the bone just below the shoulder.

Employee was using a pry bar to help adjust a tail pulley on a stacker belt. His finger became wedged between the pry bar and the stacker belt framework, which resulted in a finger being broken.

Employee was trying to loosen and remove a tight nut. He was hitting on the handle of the box-end wrench with a small sledge hammer in an attempt to free the tight nut. The hammer deflected off the wrench, and the employee's finger was caught between the wrench and the hammer handle. Employee's right index finger was cracked as a result of the blow.

Employee was cutting off bolts with a cutting torch while wearing safety glasses. ee went to dr to have metal and rust removed from eye.

EE WAS UNLOADING METAL PIECES OF SHEETING, WHEN A PIECE FELL. HE JUMPED BACK AND HIT THE TRUCK. HE CAUGHT HIS NIPPLE RING ON THE SIDE OF THE TIRE TRUCK. EMPLOYEE WAS WEARING A T-SHIRT.

EE WAS LIFTING A WIRE SCREEN (APPROX. 4'X 6'), IT SLIPPED OUT OF HIS GRASP & WHEN HE REGRIPPED THE SCREEN, HE PINCHED HIS LEFT HAND, THE SKIN BETWEEN THE INDEX FINGER & THUMBS. HE RECEIVED 7 STITCHES. THE SCREEN IS LOCATED AT THE SECONDARY CRUSHER, TOWER #2.

EE WAS USING A PRY BAR TO DISLODGE A ROCK A SMALLER GRAGMENT OF A ROCK FLEW OUT AND STRUCK HIM IN THE CHIN. HE WAS WORKING IN THE FEEDER FOR A CRUSHER.

HE WAS REMOVING ROCKS FROM WABBLER FEEDER ON OF THE WABBLER BARS THAT HE WAS STANDING ON ROLLED OVER CAUSING HIS LEG TO SLIDE DOWN BETWEEN 2- WABBLER ROLLERS.

HE WAS ADJUSTING CHAIN ON WABBLE FEEDER DURING REPAIR HE WAS STANDING BESIDE THE FEEDER (4'AWAYHE WAS MOVING THE CHAIN ELECTRICALLY TO GET IN PLACE- WHEN MASTER LINK BROKE & HIT EE IN UPPER LEFT SHOULDER.
