ON 9-22-25 EMPLOYEE REPORTED HAVING A FOREIGN BODY IN EE'S (R) EYE FROM TORCHING HOLES ON A NEW CLAD INSTALL ON 9-19-25. THE FOREIGN BODY WAS REMOVED IN THE EMERGENCY ROOM ON 9-22-2025. EE WAS PRESCRIBED EYE DROPS AND WAS RELEASED TO FULL DUTY SAME DAY.
On 11/9/23 Maintenance employee was standing on a platform not suitable to work attempting to adjust a head pulley guard. During the process the employee lost balance and began to fall backwards, the employee managed to catch themselves by using their right arm.
Employee was removing a guard from the head pulley. When doing so, EE dropped the guard on the top of left foot resulting in a contusion to EE's left foot.
Employee was looking up to adjust some rollers, when a foreign matter fell into EE's right eye. The following day the employee reported the incident and requested medical treatment.
Employee alleges that while shoveling material onto the conveyor, EE experienced a strain to EE's right shoulder. On October 1, 2021, the employee requested medical treatment; EE was treated at the clinic where EE was prescribed work restrictions.
Employee was attempting to reposition conveyor belt back on track and heard a pop in EE's shoulder. On 1/21/21 EE was diagnosed with a right shoulder strain and was prescribed physical therapy.
On 4/23/2019 employee received surgery for a lower left arm laceration. Employee was performing routine maintenance inside the crusher when EE's hand slipped causing EE's lower left arm to make contact with the socket resulting in a laceration.
Employee alleges pain in right shoulder. While entering telehandler, employee had left hand on the steering wheel and right hand on the back of the seat. When stepping in, employee felt pain and heard/felt a pop in right shoulder.
On July 24, 2018, EE requested a medical evaluation and was placed on restricted duty. On July 10, 2018, EE felt pain in groin while shoveling; EE was diagnosed with a right inguinal hernia.
On 1/25/18 employee was taken to the company clinic and received stitches for a facial laceration. Employee, while performing clean-up duties, tripped over a 6"" metal channel and fell to the ground. During the fall, the left side of EE's face struck another channel resulting in a laceration on the left cheek.
On 4/12/17, employee requested medical treatment and was prescribed chiropractic therapy by a physician. The employee alleged that ee hit head, while wearing hard hat, on a cross beam while walking under blade mill 1A in the process of greasing the plant equipment. EE was diagnosed with cervical and thoracic spine strains and was returned to full duty.
The employee requested medical treatment on 5/23/16, was taken to the occupational clinic and was placed on modified duty which is being accommodated. The employee alleged that pulling a rope for a pump float resulted in an annular tear at c7. The extent of the alleged injury is questionable.
On 10/2/15 the employee was taken to the emergency room for a laceration, received stitches and was placed on restricted duty. The head of a sharpened clamp rail bolt twisted in the employee's hand during a screen installation and cut through his glove, causing a laceration to his R-middle finger. He was returned to full duty on 10/5/15.
Authorities believe the incident that occurred to be a trespasser's suicide and attempted murder involving a couple. The male driver drove their vehicle onto Vulcan property at high speed into a holding pond and drowned. The female passenger escaped and summoned help from a Vulcan employee. Deceased was 30 yrs. old, DOB unknown and per MSHA Denver, use 1-1-85.
On 6/4/2014, the employee was taken to the emergency room where he received stitches for an eyelid laceration. While the employee was using a chisel and hammer, the chisel dislodged and struck him in the face. The employee was diagnosed with a left eyelid laceration and was released to full duty on 6/5/14.
On 11/6/13 the employee was taken to the hospital and received stitches for a laceration. While grinding on a crusher rotor, the grinder kicked back and struck the employee in the mouth resulting in a laceration to the upper lip. The employee was released to full duty on 11/7/13.
Employee went to Company clinic on 3/29/11 and was placed on restricted duty. He voluntarily retired effective 3/31/11 and requested medical treatment 3 days prior to his last day on the job. Employee alleges: for 2 years repetitive movement of climbing stairs and walking has caused L-knee pain. He was released to full duty on 4/5/11.
On 9/21/2010 the employee was taken to the company clinic, was diagnosed with R-Elbow Inflammation and was returned to restricted duty. On 9/17/10 the employee reported that while attempting to remove a guard, he pulled a muscle in his R-Forearm and refused medical treatment at that time.
The employee went to the clinic on 4/26/10, was diagnosed with a splinter/paronychia (abscess), and was returned to full duty. On 4/26/10, the employee reported: On 4/20/10 he was allegedly reaching for tools that were in his tool storage tray and a wood sliver went under his R-thumb nail.
On 03/30/2010 employee requested medical treatment and was placed on modified duty for an injury that occurred on 03/29/2010. Employee reported that while removing a bearing with a torch, grease flashed up resulting in 2nd degree burns to the nose and right cheek. Modified duty was accommodated.
On 12/19/09, the employee was taken to the company clinic and sutures were applied to the injury site. While employee attempted to catch a falling section of conduit, the conduit landed on the L-finger, resulting in a laceration. The employee was returned to full duty.
On 11/9/09 the employee was taken to the company clinic for ALLEGED R-knee pain and was returned to full duty. On 12/22/09, the company physician faxed a letter indicating a limited duty work status effective 12/16/09. The employee alleges he was injured at work but does not cite a specific event.
While removing north screw of blade mill BM-1 using come-alongs, the shaft shifted unexpectedly and employee's right leg was caught between the flights of the north and south screws. A root cause was stored energy not controlled.
Employee went to company clinic on 1/3/08 and was returned to full duty. On 3/4/08 employee returned to the clinic and was placed on restricted duty. The employee was inspecting the plant before start-up and slipped on stairs resulting in a left knee sprain.
The employee went to see the company doctor on 9/12/2006 and was placed on restricted duty. The employee ruptured his R-shoulder tendon by lifting an end of a rail road tie onto the forklift.
The employee went to see the company clinic on 5/4/2006 and was returned to full duty. While adjusting the scrubber, the employee struck L-index finger with the hammer which caused a laceration that required sutures.
THE EE WENT TO SEE THE COMPANY DR ON 2/14/06 AND WAS TAKEN OFF WORK. THE EE ALLEGES PHYSICAL ASSAULT AND SEXUAL HARRASSMENT CAUSING STRESS. (ALLEGED)
Employee went to see the company doctor on 5/12/2005 and was placed on regular duty. Employee went back to see company doctor on 5/16/2005 and was placed on restricted duty. Due to a previous and ongoing injury to his R-knee, employee began to favor his L-knee, resulting in an alleged strain to his L-knee.
Employee went to see company doctor on 11/4/2004 and was placed on restricted duty. Employee was climbing up the side of the jaw crusher to look inside, he slipped off the step and fell to the catwalk below, which resulted in a contusion to the left ribs and left arm.
Employee went to see company doctor on 10/27/04 and was placed on modified duty. When putting together shelf units, the top shelf fell and fractured employee's right thumb.
EMPLOYEE CLIMBED ONTO THE DOZER ON TRACKS, LOST BALANCE AND FELL OFF DOZER.
EMPLOYEE WENT TO SEE A DOCTOR ON 1/16/04 AND WAS PLACED ON RESTRICTED DUTY UNTIL 1/19/04 DUE TO A FRACTURE TO HIS TOE. EMPLOYEE WAS TURNING OVER A SKID STEER BUCKET TO INSTALL A NEW BOTTOM PIE CE AND BUCKET DROPPED ON HIS LEFT FOOT CAUSING A CONTUSION TO LEFT FOOT AND A FRACTION TO HIS LEFT TOE.
EE WAS PULLING ON A ""TAG LINE"" DURING THE INSTALLATION OF NEW CONVEYOR EQUIPMENT. THE LINE CAME LOOSE CAUSING THE EE'S ARM TO SNAP BACK AND INJURE RIGHT SHOULDER.
EE SPENT SEVERAL HOURS USING AN 8 LB SLEDGE HAMMER BEATER TO DRIVE IN CONNECTING PINS ON NEW CONSTRUCTION. AFTER SEVERAL HOURS OF REPETATIVE MOTION. THE EE ADVISED HIS FOREMAN THAT HIS RIGHT E LBOW WAS SORE. THE EE WAS RETURNED TO NORMAL DUTY BY THE DR WITH NO LOSS OF TIME.
EE WAS REMOVING A STEEL FRAME FROM A RAILING THAT WAS BEING INSTALLED ON A NEW PRODUCTION PLANT/THE FRAME WAS ATTACHED TO THE RAIL BY BOLTS AND TACK WELDS. AFTER REMOVING THE BOLTS THE TACK WELDS FAILED TO HOLD THE FRAME CAUSING IT TO FALL 6"" ON TO THE EE FOOT.EE INCURED A MINOR FRACTURE TO HIS BIG TOE ON HIS RIGHT FOOT. THE EE WAS ABLE TO CONTINUE WORKING HIS NORMAL DUTIES NO LO
EE NOTED DISCOMFORT IN HIS RIGHT EYE AFTER THE END OF HIS SHIFT. HE REPORTED THE DISCOMFORT TO HIS SUPERVISOR ON 8-27-03 AND WAS SENT TO THE CLINIC. HE SPENT THE DAY INSTALLING TAIL GUARDS AND CLAIMED HE WAS WEARING SAFETY GLASSES AT ALL TIMES. IT WAS NOT NOTICEABLY WINDY.
WHILE INSTALLING BEARING ASSEMBLY IN CRUSHER THE MOUNTING PLATE FELL AND CAUSED A LACERATION TO EMPLOYEE'S LEFT MIDDLE FINGER. HE WENT TO SEE COMPANY PHYSICIAN ON 1/7/03; STITCHES WERE APPLIED AND HE WAS RETURNED TO FULL DUTY.
WHILE LOOSENING THE RETURN ROLLER WITH AN OPEN-END WRENCH, THE WRENCH WAS CAUGHT IN THE CONVEYOR BELT CAUSING A LACERATION TO THE RIGHT HAND. EE WENT TO SEE COMPANY DOCTOR ON 10/10/02, BUTTE RFLY BANDAGES (STERI STRIPS) WERE APPLIED AND EE WAS RETURNED TO FULL DUTY.
EMPLOYEE WENT TO SEE COMPANY PHYSICIAN ON 10-9-02, STITCHES WERE APPLIED AND EMPLOYEE WAS RETURNED TO FULL DUTY. WHILE USING A RAZOR KNIFE TO CUT RUBBER LINERS, EE RECEIVED A LACERATION IN LEF T RING FINGER.
EMPLOYEE WENT TO SEE COMPANY PHYSICAIN ON 12/19/02 AND WAS PLACED ON RESTRICTED DUTY. WHEN WALKING THROUGH TRUCK SHOP TO TURN ON LIGHTS, EMPLOYEE STEPPED ON KING NIPPLE LAYING ON FLOOR AND SPR IANED HIS R-KNEE.
EE WENT TO SEE CO DR ON 9-5-02 AND WAS TAKEN OFF FOR ONE DAY AND PLACED ON RESTRICTED DUTY. EE HAS A LUMBAR STRAIN FROM RUNNING LOADER AND DOZER ALLEGED.
ADJUSTING A WATERVALVE, EMPLOYEE STEPPED DOWN ONTO A PLATFORM & TWISTED HIS RIGHT KNEE. HE WENT TO SEE COMPANY DOCTOR ON 8/29/02 & WAS PLACED ON RESTRICTED DUTY.
LARGE PIECE OF CLAY FELL ONTO EE'S LOWER BACK & LOWER LT. LEG WHILE CLEANING MACHINERY WITH PRY BAR. INJURING LOWER BACK AND BREAKING SMALLER BONE IN LOWER LT. LEG.
EE WENT TO SEE COMPANY DR ON 2-25-02 AND WAS PLACED ON MODIFIED DUTY. SITTING IN HAUL TRUCK WAITING TO BE LOADED, OPERATOR OF FRONT END LOADER ALLEGEDLY DUMPED LOAD VIOLENTLY IN HAUL TRUCK CAU SING THORACIC/CERVICAL STRAIN TO DRIVER OF HAUL TRUCK ALLEGED.
EE WENT TO CO. DR. ON 12-3-01 AND WAS TAKEN OFF WORK. EE STEPPED IN MUD AND HEEL DROPPED CAUSING LEFT CALF MUSCLE STRAIN. (ALLEGED)
EE WENT TO DR CO DR ON 11-16-01 AND WAS PLACED ON MODIFIED DUTY. EE WAS STEPPING OFF FROM LOADER ON UNEVEN GROUND, TOOK STEP AND HEARD LEFT HIP POP.
EE WENT TO GO SEE CO DR ON 11-16-01 AND WAS PLACED ON MODIFIED DUTY. EE WAS MEASURING COUNTER WEIGHT WHEN TAPE MEASURE WAS PULLED BY CONVEYOR BELT CAUSING A LACERATION TO LEFT MIDDLE FINGER.
EMPLOYEE WENT OT SEE DOCTOR ON 10-11-01 AND WAS TAKEN OFF WORK. PUTTING A BRACKET ON A PIECE OF CONDUIT, THE EMPLOYEE LIFTED THE CONDUIT WITH A SCREWDRIVER WHICH SLIPPED, RESULTING IN THE COND UIT COMING DOWN AND SMASHING THE EMPLOYEE'S RIGHT FINGER.
EMPLOYEE WENT TO SEE HIS DOCTOR ON 8/17/01 AND WAS TAKEN OFF WORK. A PIECE OF METAL IMBEDDED INTO EMPLOYEE'S R-EYE WHILE GRINDING ON A LOADER BUCKET.
INJURY WAS REPORTED AS A FIRST AID ON 6/20/01 & BECAME MEDICAL ON 9/27/01. THE EMPLOYEE WENT TO THE COMPANY CLINIC ON 9/27/01, WAS RETURNED TO FULL DUTY, X-RAYS WERE TAKEN AND WERE POSITIVE. WHILE CLIMBING UP ON LADDER, EMPLOYEE SLIPPED OFF STEP AND FELL ONTO BUTTOCKS.
EMPLOYER WAS NOTIFIED ABOUT THE INJURY ON JULY 5, 01. EMPLOYEE WENT TO SEE THE COMPANY DOCTOR ON 6-22-01 AND WAS PLACED ON RESTRICTED DUTY. WHILE PULLING HOSE THROUGH THE MUD, EMPLOYEE FELT A POP IN HIS GROIN AREA.
FIRST AID REPORT WAS FILED ON 4/27/01 AND ON 2/11/02. IT BECAME NECESSARY FOR THE EMPLOYEE TO SEE OUR COMPANY DOCTOR WHO RELEASED HIM TO MODIFIED DUTY. WHILE LIFTING A HEAVY STEEL PLATE, EMPL OYEE STRAINED LEFT GROIN AREA.
EE WENT TO CO. DR. ON 4-26-01 AND WAS PLACED ON RESTRICTED DUTY. CLEANING OUT THE CONE CRUSHER WITH A BAR, EE STRAINED THE RIGHT SIDE OF GROIN.
EMPLOYEE WENT TO SEE A DOCTOR ON 11-8-00 AND WAS PLACED ON RESTRICTED DUTY. OBSERVING DOZER PUSH MATERIAL INTO DOZER TRAP AND AS AIR WAS DISPLACED FROM FEEDER, IT CARRIED AIRBORNE PARTICLES WH ICH ENTERED EMPLOYEE'S RIGHT EYE.
EMPLOYEE WENT TO SEE DOCTOR ON 8-17 AND WAS PLACED ON RESTRICTED DUTY. WHEN EMPLOYEE LEANED BETWEEN CONVEYER BELT TO FREE FROZEN IDLER, SLEEVE OF OVERALLS WAS GRABBED AND ARM WAS PULLED BETWEE N IDLER AND BELT AND BELT WHICH CAUSED A LACERATION TO RIGHT FOREARM.
EMPLOYEE WENT TO DOCTOR ON 7-25-00 AND WAS PLACED OM TEMPORARY DISABILITY. EMPLOYEE DROVE OFF DESIGNATED ROUTE TO AN AREA ONLY ALLOWED FOR DOZERS, TRUCK ROLLED ON TO THE SLOPE AND EMPLOYEE BUM PED KNEE ON STEERING COLUMN.
EE WENT TO SEE DR ON 5/23/00, STITCHES WERE APPLIED AND EE WAS RETURNED TO FULL DUTY.
EE WENT TO SEE DR ON 1/27/00 AND WAS PLACED ON MODIFIED DUTY. WHILE LOWERING PUMP INTO ROCK TUNNEL, EE WAS PUSHING AGAINST PUMP WITH RIGHT HAND UNDERNEATH PUMP. PUMP HUNG UP ON SOME OBJECT AND DROPPED ONTO EE'S HAND.
