Struck by flying object

ON 9-22-25 EMPLOYEE REPORTED HAVING A FOREIGN BODY IN EE'S (R) EYE FROM TORCHING HOLES ON A NEW CLAD INSTALL ON 9-19-25. THE FOREIGN BODY WAS REMOVED IN THE EMERGENCY ROOM ON 9-22-2025. EE WAS PRESCRIBED EYE DROPS AND WAS RELEASED TO FULL DUTY SAME DAY.

Fall to the walkway or working surface

On 11/9/23 Maintenance employee was standing on a platform not suitable to work attempting to adjust a head pulley guard. During the process the employee lost balance and began to fall backwards, the employee managed to catch themselves by using their right arm.

Employee was removing a guard from the head pulley. When doing so, EE dropped the guard on the top of left foot resulting in a contusion to EE's left foot.

Employee alleges that while shoveling material onto the conveyor, EE experienced a strain to EE's right shoulder. On October 1, 2021, the employee requested medical treatment; EE was treated at the clinic where EE was prescribed work restrictions.

Employee was attempting to reposition conveyor belt back on track and heard a pop in EE's shoulder. On 1/21/21 EE was diagnosed with a right shoulder strain and was prescribed physical therapy.

On 4/23/2019 employee received surgery for a lower left arm laceration. Employee was performing routine maintenance inside the crusher when EE's hand slipped causing EE's lower left arm to make contact with the socket resulting in a laceration.

Employee alleges pain in right shoulder. While entering telehandler, employee had left hand on the steering wheel and right hand on the back of the seat. When stepping in, employee felt pain and heard/felt a pop in right shoulder.

On July 24, 2018, EE requested a medical evaluation and was placed on restricted duty. On July 10, 2018, EE felt pain in groin while shoveling; EE was diagnosed with a right inguinal hernia.

Fall onto or against objects

On 1/25/18 employee was taken to the company clinic and received stitches for a facial laceration. Employee, while performing clean-up duties, tripped over a 6"" metal channel and fell to the ground. During the fall, the left side of EE's face struck another channel resulting in a laceration on the left cheek.

On 4/12/17, employee requested medical treatment and was prescribed chiropractic therapy by a physician. The employee alleged that ee hit head, while wearing hard hat, on a cross beam while walking under blade mill 1A in the process of greasing the plant equipment. EE was diagnosed with cervical and thoracic spine strains and was returned to full duty.

The employee requested medical treatment on 5/23/16, was taken to the occupational clinic and was placed on modified duty which is being accommodated. The employee alleged that pulling a rope for a pump float resulted in an annular tear at c7. The extent of the alleged injury is questionable.

On 10/2/15 the employee was taken to the emergency room for a laceration, received stitches and was placed on restricted duty. The head of a sharpened clamp rail bolt twisted in the employee's hand during a screen installation and cut through his glove, causing a laceration to his R-middle finger. He was returned to full duty on 10/5/15.

Drowning

Authorities believe the incident that occurred to be a trespasser's suicide and attempted murder involving a couple. The male driver drove their vehicle onto Vulcan property at high speed into a holding pond and drowned. The female passenger escaped and summoned help from a Vulcan employee. Deceased was 30 yrs. old, DOB unknown and per MSHA Denver, use 1-1-85.

On 6/4/2014, the employee was taken to the emergency room where he received stitches for an eyelid laceration. While the employee was using a chisel and hammer, the chisel dislodged and struck him in the face. The employee was diagnosed with a left eyelid laceration and was released to full duty on 6/5/14.

On 11/6/13 the employee was taken to the hospital and received stitches for a laceration. While grinding on a crusher rotor, the grinder kicked back and struck the employee in the mouth resulting in a laceration to the upper lip. The employee was released to full duty on 11/7/13.

Employee alleges: for 2 years repetitive movement of climbing stairs and walking has caused L-knee pain. He was released to full duty on 4/5/11.

On 9/21/2010 the employee was taken to the company clinic, was diagnosed with R-Elbow Inflammation and was returned to restricted duty. On 9/17/10 the employee reported that while attempting to remove a guard, he pulled a muscle in his R-Forearm and refused medical treatment at that time.

The employee went to the clinic on 4/26/10, was diagnosed with a splinter/paronychia (abscess), and was returned to full duty. On 4/26/10, the employee reported: On 4/20/10 he was allegedly reaching for tools that were in his tool storage tray and a wood sliver went under his R-thumb nail.

On 03/30/2010 employee requested medical treatment and was placed on modified duty for an injury that occurred on 03/29/2010. Employee reported that while removing a bearing with a torch, grease flashed up resulting in 2nd degree burns to the nose and right cheek. Modified duty was accommodated.

On 12/19/09, the employee was taken to the company clinic and sutures were applied to the injury site. While employee attempted to catch a falling section of conduit, the conduit landed on the L-finger, resulting in a laceration. The employee was returned to full duty.

EE WAS REMOVING A STEEL FRAME FROM A RAILING THAT WAS BEING INSTALLED ON A NEW PRODUCTION PLANT/THE FRAME WAS ATTACHED TO THE RAIL BY BOLTS AND TACK WELDS. AFTER REMOVING THE BOLTS THE TACK WELDS FAILED TO HOLD THE FRAME CAUSING IT TO FALL 6"" ON TO THE EE FOOT.EE INCURED A MINOR FRACTURE TO HIS BIG TOE ON HIS RIGHT FOOT. EE NOTED DISCOMFORT IN HIS RIGHT EYE AFTER THE END OF HIS SHIFT. HE REPORTED THE DISCOMFORT TO HIS SUPERVISOR ON 8-27-03 AND WAS SENT TO THE CLINIC. HE SPENT THE DAY INSTALLING TAIL GUARDS AND CLAIMED HE WAS WEARING SAFETY GLASSES AT ALL TIMES. IT WAS NOT NOTICEABLY WINDY.
